Surgical management of anal incontinence.

The complex nature of the continence mechanism is reflected in the difficulties experienced in the surgical management of patients with faecal incontinence. Apparent anatomical abnormalities may be identified but the functional outcome following surgery may be unsatisfactory. It is therefore important to improve both the selection of patients as well as the surgical procedures themselves. Selection of patients for appropriate treatment should be based on clinical findings as well as anorectal physiology tests. Most patients referred for assessment fall into one of six categories: incontinence

Simple operative management of ingrown toenail using bipolar diathermy.

Ingrowing toenails (IGTN) cause significant discomfort for children and adults alike. Where conservative treatment fails, a surgical approach is usually adopted. Many surgical procedures have been described with varying complexity and outcome. We report a novel, simple technique which involves wedge excision of the ingrowing nail, and bipolar diathermy of the nail bed. Three-hundred and fifty-three procedures were carried out on 302 patients during the study period. Our re-operation rate for recurrence is 9.9%, which compares favourably with other techniques, and which lends itself to utilisat

Transposition flap and skin graft techniques for optimizing anastomosis coverage.

BACKGROUND: This study explores a novel technique combining transposition flaps with Split-Thickness Skin Grafts (STSG) to enhance anastomosis site coverage in free tissue transfer surgeries, particularly in lower limb reconstructions using Thoracodorsal Artery Perforator (TDAP) free flap procedures. This method aims to alleviate tension at the anastomosis sites, a common issue affecting reconstructive surgery success. METHODS: A retrospective analysis was conducted on 20 patients who underwent TDAP free flap procedures. These cases were chosen due to the anticipated tension at anastomosis si

Dental Management of Persons With Haemophilia Receiving Emicizumab Prophylaxis.

The use of emicizumab prophylaxis among persons with haemophilia A (PwHA), both with and without inhibitors, has been increasing significantly in recent years. Nevertheless, clear recommendations remain lacking regarding the necessity and extent of perioperative adjunctive systemic haemostatic therapy for surgical procedures. Given the high frequency of dental interventions, minor oral surgical procedures, are of particular clinical relevance for these persons. This paper provides a review of the literature along with a retrospective analysis of a case series involving 10 PwHA who underwent in

Utilization Rates of Diagnostic and Therapeutic Vascular Procedures Among Patients Undergoing Lower Extremity Amputations in a Rural Community Hospital: A Clinicopathological Correlation.

BACKGROUND: Significant geographical variations exist in amputation rates and utilization of diagnostic and therapeutic vascular procedures before lower extremity amputations in the United States. The purpose of this study was to evaluate the rates of diagnostic and therapeutic vascular procedures in the year prior to amputation in a contemporary population and correlate with pathological findings of the amputation specimens. METHODS: A retrospective analysis was conducted of non-traumatic amputations from 2011 to 2017 at a rural community hospital. We reviewed the proportion of patients unde

Multiple facial suspensions in protracted facial palsy.

Primary objective of multidirectional tractions of the facial muscles for protracted facial paralysis is to restore the facial symmetry. The facial suspension by multiple fascial strips in conjunction with other complemental minor surgical procedures realizes these complex tractions. Deformities requiring surgical procedures, regionally classified, are indicated for surgery as follows: 1) drooping of the eyebrow and upper eyelid, and loss of frontal creases, 2) lagophthalmos and ptosis of the lower eyelid, and 3) drooping of the cheeks and lips, and loss of the nasolabial fold. The multifascia…

Use of Phenol as a Local Anaesthetic for Adult Grommet Insertion in Resource-Limited Settings: A Preliminary Report.

BACKGROUND: Grommet insertion is one of the most commonly performed minor surgical procedures in otolaryngological practice. For such minor procedures in the outpatient, local anaesthetics are preferred; this is even more so in adults especially for grommet insertion. This study described our experience with the use of phenol as a local anaesthetic agent for grommet insertion in adults. METHODS: Phenol was used as a local anaesthetic agent that was applied topically for grommet insertion in adult patients as outpatient procedures between January and September 2018 in two tertiary hospitals. Dedicated Chalazion Clinic as a Tool for Early Surgical Education in Ophthalmology Residency.

Objective  Ophthalmology residency programs aim to improve resident surgical teaching through increased surgical exposure over a 4-year period. Resident-centric surgical clinics across various surgical specialties have been established to help develop surgical autonomy and experience. We present the first demonstration of a resident-centric chalazion incision and drainage clinic (chalazion clinic) in an ophthalmology residency with the goal of increasing early surgical exposure to residents. Design  The chalazion clinic was founded in July 2019. It is a once weekly procedure clinic conducted b

The efficacy of the surgical delay procedure in pedicle TRAM breast reconstruction.

Although experimental evidence suggests that the preliminary surgical delay procedure physiologically improves the circulation of the TRAM flap, there are no published controlled studies evaluating the clinical outcomes associated with this procedure. The purpose of this study was to evaluate the efficacy of the surgical flap delay versus no delay in consecutive patients having pedicle TRAM breast reconstruction. This was a retrospective cohort study of breast cancer patients at the University of Michigan who had pedicle TRAM reconstruction between January 2004 and March 2008. Prior to Septemb

Perioperative complications of minor and major reinterventions for peripheral arterial disease.

IntroductionPatients with peripheral arterial disease (PAD) frequently require reinterventions after lower-extremity revascularization (LER) to maintain perfusion. Current Society for Vascular Surgery guidelines define reinterventions as major or minor based on the magnitude of the procedure. While prior studies have compared primary LER procedures of different magnitudes, similar studies for reinterventions have not been performed. The objective of this study is to compare perioperative outcomes associated with major and minor reinterventions.MethodsPatients undergoing LER for PAD at a tertia

Endoscopic orchidectomy and salpingohysterectomy of pigeons (Columba livia): an avian model for minimally invasive endosurgery.

To evaluate a minimally invasive endosurgical system in birds, endoscopic orchidectomy and salpingohysterectomy were evaluated in 11 male and 14 female pigeons (Columba livia). Anesthesia was maintained by using isoflurane delivered by a pressure-cycle ventilator and produced good to excellent anesthesia during 96% of procedures. Endosurgery was performed with a 2.7-mm telescope system, 3-mm human pediatric laparoscopy instruments, and a 4.0-MHz radiofrequency device. Mean +/- SD surgery times for bilateral orchidectomy and salpingohysterectomy procedures were 39 +/- 18 minutes and 34 +/- 15 m
