Pupil abnormalities in leprosy usually result from chronic iritis with loss of stroma, iris miosis, a sluggish reaction to light, and poor dilation in response to anticholinergic mydriatics. We report two patients with long-standing lepromatous leprosy who developed tonic pupils characterized by mydriasis, absence of reaction to light and hypersensitivity to weak cholinergic solution. OBJECTIVE: To evaluate the efficacy and side effects of oral pilocarpine for the treatment of ocular symptoms in patients with primary Sjögren's syndrome (SS).
METHODS: A 12 week, single centre, randomised controlled study was performed. Twenty nine patients were randomly assigned to receive oral pilocarpine (5 mg twice a day), 28 only artificial tears, and 28 inferior puncta occlusion. The clinical findings in a 16-year-old boy with unilateral isolated pupillary paralysis in a case of herpes zoster ophthalmicus is described because of the extreme rarity of the condition. BACKGROUND/PURPOSE: Retinal detachment has previously been reported in association with topical miotic use for the treatment of glaucoma. Pilocarpine hydrochloride 1.25% was recently approved by the Food and Drug Administration for the treatment of presbyopia, with no reports of associated retinal detachments in the clinical trial data.

The action of Timolol, which lowers IOP in open-angle glaucoma, was investigated in patients with aphakic glaucoma and, with simultaneous administration of a miotic, in patients with narrow-angle glaucoma. An ab-interno goniotomy-like operation for disinserting the ciliary muscle from the scleral spur in cynomolgus monkeys is described. Following postoperative miotic and corticosteroid treatment, the eyes did well clinically and the muscle reattached to the sclera posterior to the spur. OBJECTIVE: To compare the action of miosis of 1% pilocarpine liposome with 1% pilocarpine solution in rabbits.
METHODS: 18 white rabbits were randomly divided into 3 groups. Test group received 1% pilocarpine liposome, positive control group received 1% pilocarpine solution, negative control group received liposome. In a prospective study 155 consecutive patients undergoing penetrating keratoplasty were examined for postoperative increase in intraocular pressure to determine the frequency and duration of intraocular pressure spikes, risk factors, and response to treatment. Patients were maintained on preoperative glaucoma medications except miotics. Additional medications to lower intraocular pressure perioperatively were not given. Uncertainty exists regarding accommodative and age changes in lens diameter and thickness in humans and monkeys.
