Breaks in a mechanical heart valve prosthesis with resulting acute valvar insufficiency are extremely rare. The case is reported of a 49-year-old man who, two-and-a-half years after implantation of a mitral valve prosthesis, suddenly went into acute cardiogenic shock, after having been in good health since the valve replacement. We examined M-mode echocardiograms on 35 patients with catheterization-proven mitral stenosis and normal sinus rhythm to determine whether the presence or absence of an A wave on the mitral echogram predicted mild versus severe mitral stenosis. Mitral valve area (MVA) was determined by the Gorlin formula. Presence of a mitral A wave was defined as 2 mm or greater anterior motion (after a well-defined F point) of the anterior mitral leaflet. Mitral valve prolapse (MVP) results from the systolic movement of a portion(s) or segment(s) of the mitral valve leaflet(s) into the left atrium during left ventricular (LV) systole. Over a 5-year period, 1,292 patients had operation on their native mitral valves. Ischemia was the cause of mitral insufficiency in 84 patients (6.5%). Sixty-five patients (77.4%) had mitral valve repair. Mean age was 66 +/- 10 years; 35 patients (53.8%) were women. Transcatheter mitral valve replacement (TMVR) using the SAPIEN platform has been performed in failed bioprosthetic valves (valve-in-valve), surgical annuloplasty rings (valve-in-ring), and native valves with mitral annular calcification (MAC) (valve-in-MAC). Experience over the past decade has identified important challenges and solutions to improve clinical outcomes. In this review, we discuss the indication, trend in utilization, unique challenges, procedural planning, and clinical outcomes of valve-in-valve, valve-in-ring, and valve-in-MAC TMVR.
