Atypical periodic paralysis and myalgia: A novel RYR1 phenotype.

OBJECTIVE: To characterize the phenotype of patients with symptoms of periodic paralysis (PP) and ryanodine receptor (RYR1) gene mutations. … LUSIONS: RYR1 mutations can cause late-onset atypical PP both with and without associated myopathy. Myalgia and cramps are prominent features. The McManis test may be a useful diagnostic tool to indicate RYR1-associated PP. We propose that clinicopathologic features suggestive of RYR1-related disorders should be sought in genetically undefined PP cases an …

[Fever, skin changes, myalgia--from early symptom to diagnosis].

New clinical symptoms in the context of rheumatic inflammatory system diseases require the exact knowledge of the anamnestic symptom-characteristic. Fever, skin changes and myalgia are frequent and unspecific symptoms that need a particularly broad-based interdisciplinary approach to identify potential causes. The attending physician can thereby avoid unnecessary, expensive and somewhat stressful tests. This article is intended to give important advice for everyday practice in step-wise diagnostics.

Proposed medications for taxane-induced myalgia and arthralgia (Review).

Taxanes inhibit the disassembly of microtubules, which are involved in mitosis and axoplasmatic transport, and may cause the degeneration of peripheral, mainly small, sensory nerves. Peripheral neurotoxicity is a dose-limiting side-effect of taxanes. Neuroprotective agents may aid in the reduction of neurotoxicity, thus allowing the intensification of cytostatic therapy in patients. An increasing number of medications for the prevention of taxane-induced arthralgia and myalgia are becoming available to oncology teams. The most widely studied medications include so-called analgesics such as Sha…

Post-epidemic eosinophilia-myalgia syndrome associated with L-tryptophan.

Eosinophilia-myalgia syndrome (EMS) is characterized by subacute onset of myalgias and peripheral eosinophilia, followed by chronic neuropathy and skin induration. An epidemic of EMS in 1989 was linked to consumption of L-tryptophan that had originated from a single source. Following the ban by the Food and Drug Administration (FDA) on the sale of L-tryptophan, the incidence of EMS declined rapidly. Moreover, no new cases have been described since the FDA ban was lifted in 2005. We report the clinical, histopathologic, and immunogenetic features of a new case of L-tryptophan-associated EMS, al…

Vitamin D deficiency, myositis-myalgia, and reversible statin intolerance.

OBJECTIVE: In 150 hypercholesterolemic patients, unable to tolerate ≥1 statin because of myositis-myalgia, selected by low (<32 ng/ml) serum 25 (OH) vitamin D, we prospectively assessed whether vitamin D supplementation with resolution of vitamin D deficiency would result in statin tolerance, free of myositis-myalgia. RESEARCH DESIGN AND METHODS: We studied 74 men, 76 women, median age 60, 131 white, 17 black and 2 other. On no statins, 50,000 units of vitamin D was given twice a week for 3 weeks, and then continued once a week. After 3 weeks on vitamin D, statins were restarted. Patients wer…

Mechanisms of statin-induced myalgia assessed by physiogenomic associations.

OBJECTIVE: We investigated genetic variants predictive of muscular side effects in patients treated with statins. We utilized a physiogenomic approach to prototype a multi-gene panel correlated with statin-induced myalgia. BACKGROUND: Statin-induced myalgia occurs in ∼10% of lipid clinic outpatients. Its clinical manifestation may depend in part upon gene variation from patient to patient. METHODS: We genotyped 793 patients (377 with myalgia and 416 without) undergoing statin therapy at four U.S. outpatient clinic sites to evaluate 31 candidate genes from the literature for their association…

Uncommon clinical pattern of FMF: protracted febrile myalgia syndrome.

Familial Mediterranean fever (FMF) is a genetic multisystem disorder of unknown etiology characterized by recurrent episodes of fever and pain due to acute inflammation of the peritoneum, synovia, or pleura. Up to 25% of patients with FMF report muscle pain. Myalgia may be a spontaneous pattern, exercise-induced pattern, or protracted febrile myalgia syndrome (PFMS). PFMS is characterized by severe paralyzing myalgia, high fever, abdominal pain, diarrhea, arthritis/arthralgia, and transient vasculitic rashes mimicking Henoch-Schonlein purpura. The episodes last for 4-6 weeks, except in those p…

Papular mucinosis in L-tryptophan-induced eosinophilia-myalgia syndrome.

Five patients with the L-tryptophan-related eosinophilia-myalgia syndrome had a generalized eruption of flesh-colored papules. In all patients, histologic examination revealed a focal accumulation of mucin in the upper mid dermis, associated with increased dermal cellularity. The mucin was composed predominantly of hyaluronic acid, with small amounts of sulfated acid mucopolysaccharides. The cells within the lesion were fibroblasts. The lesions slowly regressed after L-tryptophan was discontinued. Proposed explanations for the L-tryptophan-related eosinophilia-myalgia syndrome have centered on…

Tryptophan toxicity: a pharmacoepidemiologic review of eosinophilia-myalgia syndrome.

Tryptophan, an essential amino acid commercially available as a dietary supplement, has been implicated in the development of a new and potentially fatal clinical entity: eosinophilia-myalgia syndrome (EMS). EMS reached epidemic proportions in the US in late 1989 and early 1990, with 1536 cases and 27 deaths reported as of August 1990. Features of the syndrome include intense, debilitating myalgias and marked peripheral eosinophilia. Vasculitis, neuropathy, and pulmonary involvement also may be observed but are not pathognomonic. Death typically ensues from ascending polyneuropathy with result…

[Epidemic leg myalgia: a complication of influenza-like infections].

Following an influenza-like illness four patients developed severe pains in the calf muscles. All patients showed elevated levels of serum-creatinin-phosphokinase (CPK) and Coxsackie-B-virus-titers; with the exception of electromyographic abnormalities the other laboratory findings were normal. Because of the diagnosis, myalgia cruris epidemica, only symptomatic treatment was performed. Differential diagnostic and aetiological aspects are discussed.

Myalgia and biochemical changes following intermittent suxamethonium administration. Effects of alcuronium, lignocaine, midazolam and suxamethonium pretreatments on serum myoglobin, creatinine kinase and myalgia.

One hundred gynaecological patients for laparoscopy divided into five groups were studied to determine the effects of a number of pretreatments on serum myoglobin, creatinine kinase and myalgia following intermittent suxamethonium administration. One group acted as controls, while the other groups were given intravenous pretreatments of alcuronium 2 mg, midazolam 0.15 mg/kg, lignocaine 1.5 mg/kg and suxamethonium 7 mg. Serum myoglobin was determined by radio-immunoassay. The mean increases in the control group were 167 micrograms/litre myoglobin at 20 minutes and 196 IU creatinine kinase at 24…

A Pediatric Case of Hypokalemic Periodic Paralysis With Fatigue and Myalgia.

Periodic paralysis is a rare genetic disorder characterized by recurrent episodes of sudden skeletal muscle weakness. Various factors can trigger this condition, including cold temperatures, abnormal potassium levels, physical activity, and consumption of certain foods. Periodic paralysis is considered benign and usually manifests as muscle weakness. Adult patients with hypokalemic periodic paralysis often experience symptoms such as myalgia and fatigue, whereas myalgia and fatigue are less common in children. We encountered a pediatric patient with hypokalemic periodic paralysis who presented…

The role of inflammatory markers in Temporomandibular Myalgia: A systematic review.

Approximately 10 % of the general population is affected by temporomandibular disorder (TMD) pain. Diagnosis of myogenous TMD pain (i.e., TM myalgia) may be challenging, while an adequate assessment of this pain is crucial to establish an adequate management strategy. We aim to analyze if there is a relation between inflammation and TM myalgia, and if we can identify and measure inflammatory markers in patients suffering from this condition. An electronic literature search was conducted from inception up to July 14 2022 through the databases PubMed, Cochrane Library, Web of Science, and Embase…

Flecainide-induced myalgias and weakness: a rare adverse reaction.

Flecainide is a class 1C antiarrhythmic, which is known to cause several cardiac and non-cardiac adverse reactions. We report a case of a 62-year-old woman with atrial fibrillation who presented with generalised myalgias and intermittent chest heaviness after being started on flecainide. She had undergone her third cardiac ablation 1 week prior to presentation and was subsequently started on flecainide 150 mg two times per day. On physical examination, she had diminished reflexes and diffuse calf tenderness. Her blood tests demonstrated an increased erythrocyte sedimentation rate and C reactiv…

Severe Disabling Myalgia as an Initial Presentation of Polyarteritis Nodosa.

BACKGROUND: Polyarteritis nodosa (PAN) is a form of necrotizing vasculitis affecting medium or small blood vessels with multiorgan involvement. Although myalgia is a clinical feature of PAN, severe disabling myalgia as the initial presentation is rarely noted. CASE PRESENTATION: We present a case of 54-year-old male with recently detected chronic kidney disease admitted with progressive severe disabling muscular pains predominantly over calves with constitutional symptoms for seven weeks. He was weak to mobilize out of the bed. Later, he developed a vasculitic rash, unilateral ulnar claw, and…
