Pupil abnormalities in leprosy usually result from chronic iritis with loss of stroma, iris miosis, a sluggish reaction to light, and poor dilation in response to anticholinergic mydriatics. We report two patients with long-standing lepromatous leprosy who developed tonic pupils characterized by mydriasis, absence of reaction to light and hypersensitivity to weak cholinergic solution. Penalization includes all those methods apart from total occlusion which disturb the central vision of one eye by favouring the other eye. Penalization has a sensory action on functional amblyopia; moreover it also has a motor action, influencing the non-accommodative element of spasm in esotropia. Optical penalization is the most classical method. However, one should also be aware of penalization with partial occlusion or fogging, penalization with mydriatics, and penalization with sectors, which are very often used in association with the classical technique.

Fluorescein staining is helpful in diagnosis. Many conjunctival foreign bodies can be removed by swab or irrigation, without anesthesia. Cycloplegics can prevent painful ciliary spasm. A surgical opening may be required for deeply embedded objects. Topical anesthesia is required for corneal foreign body removal. Caution: x-rays must be made to rule out intraocular foreign bodies in cases of flying metal. Rust rings must be removed. Mydriatics or cycloplegics can precipitate glaucoma. Corneal abrasions are easily infected. Secondary iritis may follow deeply embedded foreign bodies.

Patients with acquired immunodeficiency syndrome (AIDS) can develop severe uveitis. Although infectious and autoimmune causes must always be considered, drug induced uveitis is also an important etiology. The secondary rise of i.o. pressure in uveitis may lead to a true secondary glaucoma or to hypertensive uveitis. The etiology of the endogenous inflammation does not seem to play a role. Pathogenetically the occlusion of the pupil with the formation of iris bombé and the obliteration of the chamber angle by exudate are important factors, while the hypersecretion of aqueous humor plays a minor role. Medical treatment consists in mydriatics and steroids. Surgical treatment depends on the pathogenetic mechanism and consists either in sector iridectomy or a filtering procedure.

The inheritance of congenital miosis in 2 pedigrees is described. The inheritance was autosomal dominant in one and autosomal recessive in the other. A series of 131 successive hospitalized cases of traumatic hyphaema after blunt injury of the eye were treated with complete bed-rest for 6 days and double eye-patching but without any topical medication. Seven patients (5.3%) had a secondary bleeding. The visual acuities of 115 patients were tested, and 105 regained their normal vision, i.e. 1.0 or over. These results are no better than those reported in series treated with moderate bed-rest for 3-4 days combined with topical administration of corticosteroids and mydriatics. PURPOSE: To describe a case of meningococcemia with anterior uveitis.
METHODS: Observational case report.
RESULTS: A 38-year-old woman developed meningococcal septicemia caused by Neisseria meningitidis type B. During her admission, she had pain in her left eye, inflammatory cells, and a fibrinous exudate in the anterior chamber and multiple posterior synechiae, all in the context of an anterior uveitis.
