Myoglobin (Mb) levels in pooled urine samples were investigated and compared in patients with different types of hereditary progressive muscular dystrophies (MD). The samples were taken before and after physical exercise. The Mb levels in the patients were significantly higher than in controls under both resting and exercise conditions. The formation of separate clusters of Mb values enabled us to distinguish patients with different types of MD according to the clinical diagnosis. Myoglobin can be found in excess in the urines of some patients with acute myocardial infarction. To test the specificity of this finding, urine specimens were analyzed from 39 patients with provisional diagnosis of myocardial infarction by means of a hemagglutination-inhibition technique with prepared antisera to monkey myoglobin. Muscle carnitine palmityltransferase activity, measured by three different methods, was very low (0 to 20 percent of controls) in a patient with a familial syndrome of recurrent myoglobinuria. Reports of electrocardiographic abnormalities in association with myoglobinuria have been sparse and have included conduction disturbances, ST segment shifts, and T-wave changes. In many instances, these changes were noted in patients with underlying heart disease. We report a case of a 34-year-old woman with ST segment depression and T-wave inversion in the inferolateral leads during the acute episode of myoglobinuria. The possible association between the use of pneumatic trousers and myoglobinuria was investigated. One hundred and four victims of multiple trauma were entered into a study over a three-month period. Patients who suffered serious injuries to two or more organ systems qualified for entrance into the study.
