Vending machines for reducing harm associated with substance use and use disorders, and co-occurring conditions: a systematic review.

BACKGROUND: To stem rising fatal overdoses and other substance use/use disorder (SUD)-related harms, communities are turning to low-barrier harm reduction strategies, such as harm reduction-focused vending machines (VMs) that distribute naloxone, fentanyl test strips, and other harm reduction-related items. This systematic review aims to synthesize literature on VMs for SUD-related harm reduction. METHODS: Four databases (Embase, Cochrane, PubMed, MEDLINE) were searched from their inception through November 29, 2023. References of identified eligible articles and pertinent prior reviews were

Independent community pharmacists' attitudes and intentions toward dispensing buprenorphine/naloxone for opioid use disorder.

BACKGROUND: Buprenorphine/naloxone (BUP/NX) for opioid use disorder (OUD) is associated with positive health outcomes; however, challenges accessing prescribed BUP/NX at community pharmacies have been identified. OBJECTIVE: The theory of planned behavior was applied to determine whether independent community pharmacists' attitudes toward dispensing BUP/NX for OUD predict intentions to dispense. METHODS: A 40-item survey was administered to 185 Texas Community Pharmacy Enhanced Services Network pharmacists. The survey assessed intentions to dispense BUP/NX (3 items), attitudes toward BUP/NX (

Who is using take-home naloxone? An examination of supersavers.

BACKGROUND: As the opioid overdose crisis persists and take-home naloxone (THN) programmes expand, it is important that the intervention is targeted towards those most likely to use it. We examined THN program participants to 1) describe those that return for refills, specifically those that reported multiple use (supersavers), and 2) to examine what rescuer characteristics were associated with higher rates of THN use. METHODS: This study included a cohort of consenting THN recipients from June 2014-June 2021 who completed initial and refill questionnaires from a widespread program in Norway.

QUANTITATIVE AND QUALITATIVE ANALYSIS OF CONSUMPTION OF NARCOTIC DRUGS AND PSYCHOACTIVE SUBSTANCE BY THE BENEFICIARIES OF THE CENTER FOR MENTAL HEALTH AND PREVENTION OF ADDICTION LLC IN 2013-2017.

Our research aims to produce qualitative and quantitative analysis of the use of narcotic drugs and psychoactive substances in 2013-2017 and their impact on drug abuse in the country. We studied 1519 medical cards of hospitalized beneficiaries. According to the obtained results, 'pharmacy' drug addiction is still widespread in Georgia. According to our data, it is hardly possible to determine, whether drug addicts consume the agents obtained at Georgian pharmacy network, or use the smuggled psychoactive substances. Regrettably, the consumption of opiates- 'Black tar' and heroin has increased a

A mapping review of take-home naloxone for people released from correctional settings.

BACKGROUND: People released from correctional settings are at an elevated risk of opioid overdose death in the weeks immediately following release. However, it is not well understood how this population, as a particularly high-risk group, is included in, and benefits from take-home naloxone (THN) programs. The objective of this review is to map research into THN for people released from correctional settings in order to identify further research needs. METHOD: We searched electronic databases, grey literature, and conference abstracts for reports on THN for people in or released from correcti

Are take-home naloxone programmes effective? Systematic review utilizing application of the Bradford Hill criteria.

BACKGROUND AND AIMS: Fatal outcome of opioid overdose, once detected, is preventable through timely administration of the antidote naloxone. Take-home naloxone provision directly to opioid users for emergency use has been implemented recently in more than 15 countries worldwide, albeit mainly as pilot schemes and without formal evaluation. This systematic review assesses the effectiveness of take-home naloxone, with two specific aims: (1) to study the impact of take-home naloxone distribution on overdose-related mortality; and (2) to assess the safety of take-home naloxone in terms of adverse

Public Perspectives on Expanding Naloxone Access to Reverse Opioid Overdoses.

BACKGROUND: Opioid overdoses can often be reversed with naloxone hydrochloride. Past studies on attitudes toward expanded naloxone access have surveyed drug users, family members, and providers. OBJECTIVES: To explore how the general public perceives take-home or nonmedical first-responder access to naloxone to reverse opioid overdoses. Additionally, little is known about how support for expanded access is related to participant's endorsement of Individualism and Just World Belief-colloquially people getting what they deserve and deserving what they get. METHODS: Lay participants completed a

Buprenorphine in the treatment of opioid addiction: opportunities, challenges and strategies.

INTRODUCTION: Buprenorphine follows the success of methadone as another milestone in the history of treatment for opioid addiction. Buprenorphine can be used in an office-based setting where it is clearly effective, highly accepted by patients and has a favorable safety profile and less abuse potential. However, the adoption of buprenorphine treatment has been slow in the USA. AREAS COVERED: This article first reviews the history of medication-assisted opioid addiction treatment and the current epidemic opioid addiction, followed by a review of the efficacy, pharmacology and clinical prescrip

A systematic review and meta-analysis of naltrexone implants for the treatment of opioid dependence.

INTRODUCTION AND AIMS: Naltrexone implants are used to treat opioid dependence, but their safety and efficacy remain poorly understood. We systematically reviewed the literature to assess the safety and efficacy of naltrexone implants for treating opioid dependence. DESIGN AND METHODS: Studies were eligible if they compared naltrexone implants with another intervention or placebo. Examined outcomes were induction to treatment, retention in treatment, opioid and non-opioid use, adverse events, non-fatal overdose and mortality. Quality of the evidence was assessed using the Grading of Recommend

Agonist treatment in opioid use: advances and controversy.

Opioid dependence is a chronic relapsing condition which requires comprehensive care; pharmacological agents form the mainstay of its long term treatment. The two most popular approaches are the harm reduction method using agonists and the complete abstinence method using antagonists. Currently, particularly from the harm minimization perspective and the low feasibility of an abstinence based approach, there is an increasing trend toward agonist treatment. The use of buprenorphine has gained popularity in view of its safety profile and the availability of the buprenorphine-naloxone combination

Current and potential pharmacological treatment options for maintenance therapy in opioid-dependent individuals.

Opioid dependence, manifesting as addiction to heroin and pharmaceutical opioids is increasing. Internationally, there are an estimated 15.6 million illicit opioid users. The global economic burden of opioid dependence is profound both in terms of HIV and hepatitis C virus transmission, direct healthcare costs, and indirectly through criminal activity, absenteeism and lost productivity. Opioid agonist medications, such as methadone and buprenorphine, that stabilize neuronal systems and provide narcotic blockade are the most effective treatments. Prolonged provision of these medications, define

[Addiction].

The news in addiction medicine for 2010 include somatic, neuroscientific as well as psychotherapeutic aspects. First are considered the risks of cardiac arythmy with methadone as long as the racemate form is prescribed in Switzerland. Then the neurosciences bring their usual novelties in the field of the addictions, this year in relational neuroscience and in the relationship between trauma and addiction. At last a contribution bridges the notion of low threshold treatment with the psychodynamic approach.

Anesthesia for patients on buprenorphine.

Opioid abuse is a devastating, costly, and growing problem in the United States, and one for which treatment can be complicated by barriers such as access to care and legal issues. Only 12% to 15% of the opioid-dependent population is enrolled in methadone maintenance programs. A significant breakthrough occurred with passage of the Drug Addiction Treatment Act of 2000 (DATA 2000). For the first time in approximately 80 years, physicians could legally prescribe opioid medications for the treatment of opioid addiction. The opiate, so designated, was buprenorphine (Subutex).

Buprenorphine for office-based treatment of patients with opioid addiction.

The Drug Addiction Treatment Act of 2000 (DATA 2000) was established to create a new paradigm for medication-assisted treatment of opiate addiction in the United States. Before enactment of DATA 2000, the use of opioid medications to treat opioid addiction was permissible only in federally approved treatment programs, ie, methadone clinics. The only medications permitted were Schedule II drugs (eg, methadone and l-a-acetylmethadol [LAAM]), which could only be dispensed, not prescribed. Under provisions of DATA 2000, qualified physicians in a medical office and other appropriate settings outsid

[Pharmacologic criteria for medical substitution in opiate dependence].

Our goal was to establish new pharmacological criteria for a drug to be used in the treatment of opioid dependence. We propose the following six pharmacodynamic and pharmacokinetic criteria: (i) the same pharmacodynamic properties as the drug being substituted; (ii) a long duration of action (minimum 24 hours, not requiring several daily doses) in order to prevent fluctuations in effect and especially withdrawal symptoms; (iii) few euphoric effects together with a minimal reinforcing effect for the drug itself and other drugs; (iv) oral or sublingual administration without any special affinity
