Nerve conduction studies through landmark based recording and effects of hand length.

As per existing guidelines, the distance between stimulator and recording electrodes in nerve conduction studies (NCS) should be the same (fixed) in all the subjects, i.e., it should not be based on anatomical landmarks. However, there are no studies comparing fixed distance recordings with landmark based NCS. We postulated that hand length can influence the NCS parameters in fixed distance recordings and this can be nullified using landmark based recordings. To test this theory, we performed NCS in 48 normal subjects as per standard guidelines (standard protocol) and then compared it to NCS w

Somatosensory evoked potentials in chronic alcoholics with spasticity.

Somatosensory evoked potentials to median and bilateral tibial nerve stimulation were investigated in eight chronic alcoholics with spasticity, 12 patients with alcoholic polyneuropathy, and 11 normal subjects. Central conduction velocities from the third lumbar vertebra to the fifth cervical vertebra and from the 12th thoracic vertebra to the fifth cervical vertebra were significantly lower in the chronic alcoholics with spasticity than in the alcoholic polyneuropathy group and in the healthy nonalcoholic group. The result indicates that chronic alcoholics with spasticity have conduction dist

Electrophysiologic study demonstrating triple antegrade AV nodal pathways in patients with spontaneous and/or induced supraventricular tachycardia.

Ten patients are described with two discrete discontinuities in AV nodal conduction curves suggesting triple antegrade AV nodal pathways. This represents approximately 6% of patients seen in this laboratory with dual AV nodal pathways. Patients ranged in age from 18 to 63 (mean +/- SD, 48 +/- 15 years). Six of the 10 patients had organic heart disease and four did not. The effect of cycle length on triple pathways could be analyzed in 8 of 10 patients who had atrial extrastimulus testing at two or more cycle lengths. Three of these eight patients had triple pathways at all tested cycle lengths

Neurodynamics Is an Effective Intervention for Carpal Tunnel Syndrome.

Clinical Scenario: Carpal tunnel syndrome (CTS) is the most common entrapment neuropathy of the upper-extremity. Due to the involvement of the median nerve, long-term compression of this nerve can lead to hand dysfunction and disability that can impact work and daily life. As such, early treatment is warranted to prevent any long-term damage to the median nerve. Conservative management is utilized in those with mild to moderate CTS. Neural mobilizations can aid in the reduction of neural edema, neural mobility, and neural adhesion while improving nerve conduction. Clinical Question: Is neurody

Spatial Heterogeneity in Myelin Sheathing Impacts Signaling Reliability and Susceptibility to Injury.

Axons in the mammalian brain show significant diversity in myelination motifs, displaying spatial heterogeneity in sheathing along individual axons and across brain regions. However, its impact on neural signaling and susceptibility to injury remains poorly understood. To address this, we leveraged cable theory and developed model axons replicating the myelin sheath distributions observed experimentally in different regions of the mouse central nervous system. We examined how the spatial arrangement of myelin affects propagation and predisposition to conduction failure in axons with cortical v

[Diagnostic significance of the H-reflex in traumatic lesions of the spinal cord].

The study of H-reflexes, their latent periods and M-responses involved 69 patients with trauma to the spinal cord. The results showed that these parameters were normal in 39 patients. The remaining patients lacked H- or M-responses, which was indicative of inhibition of neural and muscular activity due to disorders of hemodynamic and metabolic processes in the spinal cord or in muscles of the lower extremities. The study of latent periods and the speed of impulse conduction along the peripheral nerve revealed that after an injury to the spinal cord some patients presented impairment of impulse

Median nerve impairment in leprosy: how does it differ from the classic carpal tunnel syndrome?

BACKGROUND: Carpal tunnel syndrome (CTS) has already been described as a possible form of neural leprosy presentation. However, the median nerve can be involved in this neuropathy in proximal segments and, sometimes, with an asymmetric impairment of the digital branches. OBJECTIVE: To detail the pattern of median nerve impairment through nerve conduction study (NCS) and ultrasound evaluation. METHODS: This cross-sectional study comprises 15 primary neural leprosy (PNL) patients and 14 patients with CTS who underwent peripheral nerve ultrasonography and NCS evaluation. RESULTS: From the tota

Effects of Visual Deprivation on Remodeling of Nodes of Ranvier in Optic Nerve.

Oligodendrocytes, the myelinating cells of the CNS, promote rapid action potential conduction along axons. Changes in the geometry of gaps between myelin segments, known as nodes of Ranvier, affect the conduction speed of neuronal impulses and can ultimately alter neural synchronization and circuit function. In contrast to synaptic plasticity, much less is known about how neural activity may affect node of Ranvier structure. Recently, perinodal astrocytes have been shown to remodel nodes of Ranvier by regulating thrombin proteolysis, but it is not known whether neural activity influences this

Nf2 Mutation in Schwann Cells Delays Functional Neural Recovery Following Injury.

Merlin is the protein product of the NF2 tumor suppressor gene. Germline NF2 mutation leads to neurofibromatosis type 2 (NF2), characterized by multiple intracranial and spinal schwannomas. Patients with NF2 also frequently develop peripheral neuropathies. While the role of merlin in SC neoplasia is well established, its role in SC homeostasis is less defined. Here we explore the role of merlin in SC responses to nerve injury and their ability to support axon regeneration. We performed sciatic nerve crush in wild-type (WT) and in P0SchΔ39-121 transgenic mice that express a dominant negative Nf

Evaluation of Auditory Evoked Potentials in Neuromyelitis Optica Spectrum Disorder.

BACKGROUND: Neuromyelitis optica spectrum disorder (NMOSD) is a recurrent demyelinating neuroinflammatory disorder of the central nervous system. This study aimed to evaluate the neural conduction and sound information processing in individuals with NMOSD, through the analysis of auditory brainstem responses (ABRs) and cortical auditory evoked potentials (CAEPs). METHODS: Twenty-four individuals (9-21 years old) were distributed into two groups: the study group (SG), diagnosed with NMOSD, and the control group, healthy individuals. Their ABRs and CAEPs were evaluated, and the results were exa…

Doppler sonography for the diagnosis of carpal tunnel syndrome: a critical review.

INTRODUCTION: Doppler sonography may detect increased intraneural blood flow of the median nerve in carpal tunnel syndrome (CTS). The purpose of this review is to critically evaluate the literature about the diagnostic value of increased intraneural flow detected by sonography in CTS Methods: Systematic review of studies published between 1985 and 2013. RESULTS: The 7 studies we found had considerable differences in study design and had methodological shortcomings. Doppler sonography had a median sensitivity of 72% (range, 41-95%) and a median specificity of 88% (range, 71-100%). Most studies
