Large Cell Neuroendocrine Carcinoma of the Colon With Carcinomatosis Peritonei.

Colorectal large-cell neuroendocrine carcinomas (NECs) are extremely rare and have very poor prognosis compared to adenocarcinomas. A 74-year-old man presented with abdominal pain, diarrhea and hematochezia. The histopathologic report of colonoscopic biopsy performed at a local clinic was a poorly differentiated carcinoma. An abdominopelvic computed scan revealed irregularly enhanced wall thickening at the sigmoid colon with regional fat stranding and lymphnode enlargement. He underwent a laparoscopic high anterior resection with selective peritonectomy for peritoneal carcinomatosis, intraoper

Large Cell Neuroendocrine Carcinoma of the Extrahepatic Bile Duct.

Primary neuroendocrine tumors originating from the extrahepatic bile duct are rare. Among these tumors, large cell neuroendocrine carcinomas (NECs) are extremely rare. A 59-year-old man was admitted to Sanggye Paik Hospital with jaundice that started 10 days previously. He had a history of laparoscopic cholecystectomy, which he had undergone 12 years previously due to chronic calculous cholecystitis. Laboratory data showed abnormally elevated levels of total bilirubin 15.3 mg/dL (normal 0.2-1.2 mg/dL), AST 200 IU (normal 0-40 IU), ALT 390 IU (normal 0-40 IU), and gamma-glutamyl transferase 1,2

Multimodality therapy for large cell neuroendocrine carcinoma of the thymus.

A case of large cell neuroendocrine carcinoma of the thymus successfully treated with chemoradiation, followed by extended resection under cardiopulmonary bypass, is reported. A 44-year-old man diagnosed with thymic large cell neuroendocrine carcinoma received induction chemoradiation therapy (3 cycles of cisplatin/etoposide and 45 Gy of hyperfractionated radiation) because of invasion to the aortic arch and pulmonary trunk. After radiographic partial response was noted, a radical resection under cardiopulmonary bypass was performed. Pathologic examination revealed no viable cells in the tumor

[Two case reports of neuroendocrine cell carcinoma of the stomach].

We encountered 2 cases of endocrine cell carcinoma of the stomach. One patient had a type 3 tumor in the cardia, and the other patient had a submucosal tumor in the posterior wall of the gastric angle, both of which were detected by upper gastrointestinal endoscopy. The first patient underwent total gastrectomy(D1+No.7, 19) with cholecystectomy. Microscopic histological examination fortuitously revealed a tumor thrombus in the vessel of the gallbladder. Eight months after the operation, a paraaortic lymph node recurrence was detected, and the patient died 28 months after the operation. The sec

Fast-growing large cell neuroendocrine carcinoma of mediastinum.

Neuroendocrine carcinomas combine a heterogeneous group of tumors occurring in lungs on a rare occasion, and in some cases, they appear to have extraordinary quick growth and extrapulmonary localization. In this case we present a 42-year-old patient who underwent a right upper lobectomy for emphysema, and 6 months later, the tumor developed again into a giant neuroendocrine carcinoma of the mediastinum.

[A rare case of large cell neuroendocrine carcinoma of the thymus].

A 46-year-old man presented with chest pain at a local hospital in July 2007. Chest computed tomography (CT) showed a 48-mm mass in the anterior mediastinum. CT-guided percutaneous tumor biopsy demonstrated large cell neuroendocrine carcinoma of the thymus. He was referred to our hospital in August 2007. Because the tumor had already progressed to stage IVb according to the Masaoka classification of thymic epithelial tumors, the patient was treated with combination chemotherapy of cisplatin and irinotecan, which achieved a partial response. However, the tumor relapsed in February 2008. He died

Neuroendocrine small cell carcinoma of the breast: report of a case.

A rare case of neuroendocrine small cell carcinoma (SmCC) of the breast is reported. A 51-year-old postmenopausal woman noticed a nodule approximately 3 cm in diameter in her right upper breast. Histologically, the tumor consisted of small ovoid to pleomorphic cells with hyperchromatic nuclei, and a large central area was occupied by acellular amorphous tissue. Extensive lymphatic permeation was seen around the tumor. Invasive and in situ ductal carcinoma foci were not observed in and surrounding the tumor. Immunohistochemically, estrogen and progesterone receptors and HER2/neu were all negati

Primary large cell neuroendocrine carcinoma of the urinary bladder.

Reports of primary large cell neuroendocrine carcinomas of the urinary bladder are few; we identified only 2 cases in the literature. Both of these cases involved male patients with rapid progression of disease culminating in death with widespread metastases. We report a case of primary large cell neuroendocrine carcinoma of the bladder, with an admixed minor element of adenocarcinoma, in an 82-year-old man. This solitary lesion arose in a bladder diverticulum lateral to the left ureteric orifice. Two attempts at transurethral resection were unsuccessful at achieving local control. The patient

Neuroendocrine cell proliferations in gastric antral vascular ectasia.

Gastric antral vascular ectasia is a recently recognized cause of chronic gastrointestinal blood loss. Immunohistochemical and electron-microscopic studies were performed on the surgically resected antrum of a patient with antral vascular ectasia. These revealed extraepithelial and intraepithelial neuroendocrine cell proliferations. Staining for a wide range of neural and endocrine markers demonstrated that these cells contain large quantities of 5-hydroxytryptamine and vasoactive intestinal polypeptide. Gastric vascular ectasia is of unknown etiology but has been associated with achlorhydria

Metastatic neuroendocrine lesions in diffuse idiopathic pulmonary neuroendocrine cell hyperplasia (DIPNECH): a narrative review and case series.

OBJECTIVE: Diffuse idiopathic pulmonary neuroendocrine cell hyperplasia (DIPNECH) is a rare entity characterised by the proliferation of neuroendocrine cells in the respiratory epithelium. DIPNECH is a pre-neoplastic condition that can progress to localised or metastatic neuroendocrine tumours (NETs). The understanding of DIPNECH remains limited, and although the European Neuroendocrine Society has recommendations for its management, there are currently no established guidelines. METHOD: This study involves a case series of three patients with DIPNECH progressing to metastasis, as well as a n…

Diffuse idiopathic pulmonary neuroendocrine cell hyperplasia and the associated lung neuroendocrine tumors: clinical experience with a rare entity.

BACKGROUND: Normal adult lungs contain pulmonary neuroendocrine cells (PNECs). PNEC hyperplasia may be either reactive or idiopathic, and the idiopathic type is defined as diffuse idiopathic PNEC hyperplasia (DIPNECH). It is believed that DIPNECH is a neuroendocrine proliferative process associated with carcinoid tumors. The available data regarding this rare condition are very limited. The objective of the current study was to describe the clinical, radiologic, and pathologic characteristics of patients with DIPNECH and the effect of various therapeutic modalities on patient well being. METH…

Neuroendocrine ampullary tumors: spectrum of disease including the first report of a neuroendocrine carcinoma of non-small cell type.

Primary neuroendocrine tumors of the ampulla of Vater are rare. These tumors, like those of the respiratory tract and remainder of the gastrointestinal tract, appear along a histologic spectrum of increasing malignancy from low-grade carcinoid tumors to high-grade small cell undifferentiated cancers. We herein describe the characteristics of these ampullary neoplasms and report the first case of a neuroendocrine carcinoma of non-small cell type, which represents the middle of this histologic continuum. This tumor seems to follow a more aggressive course than the carcinoid tumor but less aggres

Combined larynx large cell neuroendocrine and squamous cell carcinoma: a case report.

Laryngeal cancer ranks third among the most common head and neck neoplasms. The most common histological subtype is squamous cell carcinoma, and neuroendocrine tumors are rare. An even rarer entity is a composite tumor with both these histologies. This case reports a metastatic combined carcinoma of squamous cells and large neuroendocrine cells, presenting favorable response to treatment with a total laryngectomy followed by adjuvant therapy including chemo-, radio-, and immunotherapy.

Genetic and immunohistochemical profiling of small cell and large cell neuroendocrine carcinomas of the breast.

Neuroendocrine carcinomas (NEC) of the breast are exceedingly rare tumors, which are classified in the WHO system as small cell (SCNEC) and large cell (LCNEC) carcinoma based on indistinguishable features from their lung counterparts. In contrast to lung and enteropancreatic NEC, the genomics of breast NEC have not been well-characterized. In this study, we examined the clinicopathologic, immunohistochemical, and genetic features of 13 breast NEC (7 SCNEC, 4 LCNEC, 2 NEC with ambiguous small versus large cell morphology [ANEC]). Co-alterations of TP53 and RB1 were identified in 86% (6/7) SCNEC

Automated Analysis of Proliferating Cells Spatial Organisation Predicts Prognosis in Lung Neuroendocrine Neoplasms.

Lung neuroendocrine neoplasms (lung NENs) are categorised by morphology, defining a classification sometimes unable to reflect ultimate clinical outcome. Subjectivity and poor reproducibility characterise diagnosis and prognosis assessment of all NENs. Here, we propose a machine learning framework for tumour prognosis assessment based on a quantitative, automated and repeatable evaluation of the spatial distribution of cells immunohistochemically positive for the proliferation marker Ki-67, performed on the entire extent of high-resolution whole slide images. Combining features from the fields

Interleukin-10 Induces Expression of Neuroendocrine Markers and PDL1 in Prostate Cancer Cells.

Interleukin-10 (IL10) is best studied for its inhibitory action on immune cells and ability to suppress an antitumour immune response. But IL10 also exerts direct effects on nonimmune cells such as prostate cancer epithelial cells. Elevated serum levels of IL10 observed in prostate and other cancer patients are associated with poor prognosis. After first-line androgen-deprivation therapy, prostate cancer patients are treated with androgen receptor antagonists such as enzalutamide to inhibit androgen-dependent prostate cancer cell growth. However, development of resistance inevitably occurs and
