Neurosurgical management for chronic and end-of-life pain in children: A systematic review.

INTRODUCTION: Chronic and end-of-life pain in children is underreported and undermanaged. Current guidelines for pediatric chronic pain include medical and interventional modalities; however, the inclusion of neurosurgical treatments is uncommon and inconsistent. This systematic review presents the literature, and we provide recommendations for the role of neurosurgical procedures in treating chronic and end-of-life pain in children. METHODS: A systematic review was conducted in accordance with the Preferred Reporting Items for Systematic Review and Meta-analyses (PRISMA) guidelines using thr

Evidence of amyloid-β cerebral amyloid angiopathy transmission through neurosurgery.

Amyloid-β (Aβ) is a peptide deposited in the brain parenchyma in Alzheimer's disease and in cerebral blood vessels, causing cerebral amyloid angiopathy (CAA). Aβ pathology is transmissible experimentally in animals and through medical procedures in humans, such as contaminated growth hormone or dura mater transplantation in the context of iatrogenic prion disease. Here, we present four patients who underwent neurosurgical procedures during childhood or teenage years and presented with intracerebral haemorrhage approximately three decades later, caused by severe CAA. None of these patients carr

Characterizing the unmet need for pediatric neurosurgical care.

BACKGROUND: Specialized pediatric neurosurgical care is often limited or absent in resource-limited countries. This study aims to characterize the unmet need for pediatric neurosurgery by characterizing the burden and outcomes of pediatric neurosurgical disease in children at a high-volume surgical center in Pakistan. METHODS: We conducted a retrospective chart review of all pediatric patients admitted for any indication requiring a neurosurgical consultation between 2010 and 2021 at a tertiary care hospital in Karachi, Pakistan. We described the scope and volume of neurosurgical procedures p

Clinical feature and outcome of pediatric cerebrovascular disease: a neurosurgical series.

To investigate the clinical features and surgical outcome of pediatric cerebrovascular disease (CVD), we retrospectively reviewed 280 children (up to 15 years of age) who underwent neurosurgical procedures for CVD between 1979 and 1998. Open surgery (n=448), endovascular procedures (n=22), and stereotactic radiosurgery (n=14) were the main neurosurgical procedures adopted. Clinical features and surgical outcomes were described according to the etiology of the CVD and the number of years of management. The mean duration of follow-up was 34 months. The mean age was 7.1 years, and the male-to-fem

Meningitis due to Enterobacter aerogenes subsequent to resection of an acoustic neuroma and abdominal fat graft to the mastoid.

Meningitis is an uncommon complication of neurosurgical procedures, with an incidence of 1.1% to 2.5%. Although unusual, the frequency of nosocomial Gram-negative meningitis appears to be increasing. Gram-negative meningitis has been documented following disruption of the dura-arachnoid barrier secondary to trauma or surgery. The association of Gram-negative bacillary meningitis with neurosurgical procedures was first reported in the 1940's. Wolff et al. described the association between Enterobacter species and post-neurosurgical infection. More recently, risk factors for nosocomial Enterobac

[Rupture of a cerebral aneurysm and pregnancy: apropos of a case of neurosurgical intervention during pregnancy].

The authors report a case of an aneurysm of the posterior cerebral artery that was operated on after 28 weeks of amenorrhoea without any sequelae for the mother or the fetus. The article gives precise details of the neurosurgical procedures carried out in this pregnant woman and how and what neurosurgical anaesthesia was administered while the aneurysm was cured and the pregnancy continued. Finally the obstetrical and neurosurgical care is discussed. The neurosurgeons decided on the operation in view of the neurological findings. Once the neurosurgical decision has been made, the obstetrical p

The incidence of postoperative cerebrospinal fluid leakage after elective cranial surgery: a systematic review.

Cerebrospinal fluid (CSF) leakage is a major complication after elective neurosurgical procedures. The aim of this systematic literature review is to summarize the incidence rates of postoperative cerebrospinal fluid leakage for neurosurgical procedures, classified by surgical approach. The Pubmed, Cochrane, Embase, and Web of Science databases were searched for studies reporting the outcome of patients undergoing elective neurosurgical procedures. The number of patients, surgical approach, and indication for surgery were recorded for each study. Outcomes related to CSF leakage such as clinica

Utility of Routine Preoperative Urinalysis in the Prevention of Surgical Site Infections.

OBJECTIVE: Preoperative assessment is important for neurosurgical risk stratification, but the level of evidence for individual screening tests is low. In preoperative urinalysis (UA), testing may significantly increase costs and lead to inappropriate antibiotic treatment. We prospectively evaluated whether eliminating preoperative UA was noninferior to routine preoperative UA as measured by 30-day readmission for surgical site infection in adult elective neurosurgical procedures. METHODS: A single-institution prospective, pragmatic study of patients receiving elective neurosurgical procedure

Expanded applications of knotless tissue control devices in neurosurgical cranial and spinal applications.

OBJECTIVES: Effective wound closure is essential to neurosurgical procedures. Historical rates of neurosurgical wound complications range from 7 to 14 %. Knotless suturing with barbed sutures may offer effective closure rates but has had limited evaluation of safety in neurosurgical applications. METHODS: Consecutive patients undergoing cranial, spinal, and peripheral nerve neurosurgical procedures from 12/2022 until 7/2024 were included in a quality improvement initiative. A retrospective review of demographics, past medical history, and operative and postoperative variables was undertaken.

Written risk disclosure in elective neurosurgical consent: A retrospective observational study with descriptive postoperative outcomes.

BACKGROUND: Informed consent is a fundamental ethical and legal requirement in neurosurgical practice, particularly given the high risk of postoperative morbidity and medico-legal litigation. Written documentation of material risk disclosure provides objective evidence that valid consent has been obtained. However, real-world data on neurosurgical consent documentation practices remain limited in Malaysia. METHODS: A retrospective observational study was conducted at a tertiary referral center, reviewing elective neurosurgical procedures performed in 2022. Electronic medical records and scann

The value of cerebrospinal fluid lactate levels in diagnosing CSF infections in pediatric neurosurgical patients.

PURPOSE: Diagnosis of cerebrospinal fluid (CSF) infections in patients following neurosurgical procedures can be challenging. CSF lactate (LCSF) has been shown to assist in differentiating bacterial from non-bacterial meningitis in non-neurosurgical patients. The use of lactate in diagnosing CSF-related infections following neurosurgical procedures has been described in adults. The goal of this study was to describe the role of LCSF levels in diagnosing CSF-related infections among neurosurgical children. METHODS: We retrospectively collected data for all pediatric patients treated at a large

Emerging technology in intracranial neuroendoscopy: application of the NICO Myriad.

Improvement in fiber optics and imaging paved the way for tremendous advancements in neuroendoscopy. These advancements have led to increasingly widespread use of the endoscope in neurosurgical procedures, which in turn incited a technological revolution leading to new approaches, instruments, techniques, and a diverse armamentarium for the treatment of a variety of neurosurgical disorders. Soft-tissue removal is often a rate-limiting aspect to endoscopic procedures, especially when the soft tissue is dense or fibrous. The authors review a series of cases involving patients treated between Aug

A meningitis case due to Stenotrophomonas maltophilia and review of the literature.

Stenotrophomonas maltophilia (formerly Xanthomonas maltophilia) is a Gram-negative bacillus increasingly associated with serious nosocomial infections. Here, we describe a 30-year-old male patient who developed meningitis associated with this organism after several neurosurgical procedures. A review of the literature revealed only 15 previous reports. Most cases were associated with neurosurgical procedures. Antimicrobial therapy is complicated by multiple drug resistance of the organism, and trimethoprim-sulfamethoxazole is the recommended agent for treatment.

Current State of Brazilian Neurosurgery: Evaluation of Burden of Care and Case Volume.

BACKGROUND: Low- and middle-income countries (LMICs) bear a heavier burden of neurosurgical diseases than high-income countries. Brazil, a growing middle-income country, holds promise as a health care market. However, there exists a lack of information to characterize the state of neurosurgical practice and access to care in Brazil. This study aimed to characterize neurosurgical practice in Brazil and identify barriers to care. METHODS: A collaborative survey was developed with Brazilian neurosurgeons and distributed by the Brazilian Society of Neurosurgery. The survey gathered demographic in

[Is it possible to prevent diabetic ketoacidosis at diagnosis of pediatric type 1 diabetes? Lessons from the COVID-19 pandemic].

INTRODUCTION: Of the many chronic painful conditions, trigeminal neuralgia (TN) affecting the orofacial region needs the particular attention of physicians and surgeons, especially those specialising in the maxillofacial region. Treatment protocols for the management of classic TN include pharmacology and surgical intervention. Oral and maxillofacial surgeons have traditionally employed the peripheral neurectomy in the surgical management of TN. This review aims to evaluate the efficacy of peripheral neurectomy in the management of TN with regard to (a) the relief of symptoms in comparison wit

Neurosurgical management of occult spinal dysraphism associated with OEIS complex.

INTRODUCTION: OEIS complex has been described as a combination of defects consisting of omphalocele (O), exstrophy of the cloaca (E), imperforate anus (I), and spinal defects (S). As the first three defects are life-threatening and treated on a priority basis, neurosurgical intervention for spinal defects is deferred until recuperation from abdominogenital repair. However, the best timing for neurosurgical operation has not been precisely described. MATERIALS AND METHODS: We reviewed our neurosurgical management of three cases (case 1: myelomeningocele; cases 2 and 3: terminal myelocystocele)

Exoscopic resection of tentorial meningiomas: Technical note from the exoscope era.

BACKGROUND: Tentorial meningioma resection is among the most challenging neurosurgical procedures. Various surgical approaches must be considered, including subtemporal, lateral suboccipital, occipital interhemispheric supratentorial, and supracerebellar transtentorial (SCTT) techniques. However, performing tentorial meningioma resections in the SCTT approach using operative microscopy often requires awkward positioning for the neurosurgeon. In this study, we report two cases of tentorial meningioma resection and discuss the utility and limitations of the 4KHD 3D operating room layout, showing

Implications of a neurosurgical intervention in a patient with a surgically repaired hypoplastic left heart syndrome.

Hypoplastic left heart syndrome (HLHS) accounts for 4-9% of congenital heart disease in children. The mortality rate among children with HLHS undergoing cardiac repair is well documented, but comparable data for noncardiac surgical procedures are not well known. Medically intractable epilepsy is one of those problems, which arises as a complication of cardiovascular surgery in HLHS and necessitates neurosurgical intervention. There is no published knowledge about neurosurgical procedures performed in children with HLHS in the English literature. Thus, we present a 10-year-old boy who developed
