Comitant strabismus etiology: extraocular muscle integrity and central nervous system involvement-a narrative review.

Strabismus is not a condition in itself but the consequence of an underlying problem. Eye misalignment can be caused by disease, injury, and/or abnormalities in any of the structures and processes involved in visual perception and oculomotor control, from the extraocular muscles and their innervations to the oculomotor and visual processing areas in the brain. A small percentage of all strabismus cases are the consequence of well-described genetic syndromes, acquired insult, or disease affecting the extraocular muscles (EOMs) or their innervations. We will refer to them as strabismus of periph

Intramuscular Nerves of the Inferior Rectus Muscle: Distribution and Characteristics.

PURPOSE: Knowledge of the distribution of intramuscular nerves of the extraocular muscles is crucial for understanding their function. The purpose of this study was to elucidate the intramuscular distribution of the oculomotor nerve within the inferior rectus muscle (IRM) using Sihler's staining. METHOD: Ninety-three IRM from 50 formalin-embalmed cadavers were investigated. The IRM including its branches of the oculomotor nerve was finely dissected from its origin to the point where it inserted into the sclera. The intramuscular nerve course was investigated after performing Sihler's whole-mo

Lateral Rectus Myectomy Combined With Medial Rectus Plication for Complete Oculomotor Nerve Palsy: A Case Report With Detailed Postoperative Course.

BACKGROUND: Lateral rectus myectomy has been reported as an effective and simple surgical option for large-angle paralytic exotropia. We report a case of complete oculomotor nerve palsy treated with lateral rectus myectomy combined with medial rectus plication, focusing on serial changes in ocular alignment and abduction limitation. CASE REPORT: A woman in her 20s developed left oculomotor nerve palsy following surgery for an intracranial tumor. At presentation, visual acuity was 20/16 and 20/100, with temporal hemianopia and a central scotoma in the right and left eyes, respectively. She exh

[Significance of the "V" syndrome with double "up shoot". Insufficiency of the two superior oblique muscles in craniofacial malformations].

Craniofacial malformations (telorbitism, craniofaciostenosis, plagiocephalia, Franceschetti's syndrome) are frequently associated with oculomotor anomalies. The most common vertical anomaly is the existence of a "V" syndrome with double "up shoot", with deficiencies in the two superior oblique muscles. Several mechanisms may be involved in the origin of this anomaly, related to the anatomical orbital malformation: orbital torsion with antimongoloid clefts responsible for a syndrome of macular pseudo-ectopia with extorsion of the 4 rectus muscles; plagiocephalia responsible for sagitallization

Loss of MAFB Function in Humans and Mice Causes Duane Syndrome, Aberrant Extraocular Muscle Innervation, and Inner-Ear Defects.

Duane retraction syndrome (DRS) is a congenital eye-movement disorder defined by limited outward gaze and retraction of the eye on attempted inward gaze. Here, we report on three heterozygous loss-of-function MAFB mutations causing DRS and a dominant-negative MAFB mutation causing DRS and deafness. Using genotype-phenotype correlations in humans and Mafb-knockout mice, we propose a threshold model for variable loss of MAFB function. Postmortem studies of DRS have reported abducens nerve hypoplasia and aberrant innervation of the lateral rectus muscle by the oculomotor nerve. Our studies in mic

Prognosis of oculomotor palsy in patients with aneurysms of the posterior communicating artery.

In patients with symptomatic aneurysms of the posterior communicating artery, the prognosis of oculomotor palsy mainly depends on the interval between the onset of palsy and the time of operation, and furthermore on the degree of preoperative deficit and the development of the cranial nerve lesion. The incidence of ultimately complete or incomplete palsy is the same in cases with subarachnoid haemorrhage and without rupture ("warning symptom"). In many cases, an initially incomplete paresis develops to a complete ocular palsy within eight days. Ptosis is generally the first symptom, and it fre

Dynamic Imaging of the Eye, Optic Nerve, and Extraocular Muscles With Golden Angle Radial MRI.

PURPOSE: The eye and its accessory structures, the optic nerve and the extraocular muscles, form a complex dynamic system. In vivo magnetic resonance imaging (MRI) of this system in motion can have substantial benefits in understanding oculomotor functioning in health and disease, but has been restricted to date to imaging of static gazes only. The purpose of this work was to develop a technique to image the eye and its accessory visual structures in motion. METHODS: Dynamic imaging of the eye was developed on a 3-Tesla MRI scanner, based on a golden angle radial sequence that allows freely s

Cranial nerves III, IV, and VI.

Movements of the eye are produced by six extraocular muscles innervated by three cranial nerves: the oculomotor (III), the trochlear (IV), and the abducens (VI). These cranial nerves are discussed together because of the interrelated nuclear origins, neural pathways, and motor functions. The normal anatomic pathway of these three nerves is presented. The clinical and pathologic manifestations of lesions producing both isolated and complex palsies of these nerves are discussed along with imaging correlation.

[Oculo-orbital topometry studied by X-ray computed tomography in strabismic children].

Scanner examination of the oculo-orbital region of 20 squinting children shows, by mean of precise measures, in a first part abnormalities of orbital development (these abnormalities are always moderate). In a second part, this examination demonstrate asynergies of the horizontal oculomotor muscles, particularly an isolated hypermotricity of one medial ocular muscle in patent induction movements. Despite of irradiation of the crystalline lens this examination can be helpful in a preoperatory check-up.

Incomitant strabismus: does extraocular muscle form denote function?

The paradigm that an "underacting" extraocular muscle is always atrophic or hypoplastic and that an overacting extraocular muscle should always be enlarged leads to inconsistencies with clinical observations. These include findings of "overacting" inferior oblique muscles, superior rectus muscle overaction or contracture syndrome, and normal extraocular muscle diameters in patients with apparent superior oblique muscle palsy, among other clinical entities. These inconsistencies can be reconciled if one accepts the possibility that extraocular muscle contractile activity may reflect a change in

Statomotor system in the marine mollusk Clione limacina.

1. In the marine mollusk Clione limacina the "statomotor system" (named by analogy with the oculomotor system) has been found. This system includes a muscle that is directly attached to the statocysts connecting them with each other and with the inner surface of the body. 2. The statocyst muscle consists of four electrically coupled, mononuclear cells. Statocyst muscle cells do not generate spike-like potentials but only excitatory junctional potentials. 3. The motor input to the statocyst muscle correlates with the activity of the locomotor generator. This suggests that in the soft-bodied Cli

[Isolated oculomotor nerve palsy caused by mesencephalic hemorrhage].

A 62-year-old man with a history of diabetes mellitus, hypertension and liver damage has sudden diplopia on Nov. 4, 1992. Ocular movements of this right eye were normal, but his left eyelid was completely ptotic, and left pupil was dilated and nonreactive to light. In primary position, his left eye deviated outward, and could not move to any direction. He was diagnosed as having total oculomotor nerve palsy of the left side without any other neurological signs or symptoms; his consciousness level was alert and mental state was normal. The present case showed normal facial sensation, no facial

[Ocular movement and nystagmus: basics and clinical diagnosis].

BACKGROUND: Eye movements are one of the most complex motor functions of the central nervous system (CNS). Eye movement disorders including nystagmus occur in diseases of the CNS and the vestibular system. A systematic clinical examination often allows a topodiagnostic classification of the lesion. OBJECTIVE: The basics of eye movements, the role of the cerebellum, the clinical examination of the oculomotor system, and the most important forms of nystagmus and their diagnostic implications are described. MATERIALS AND METHODS: A literature analysis assessing eye movements, cerebellar control

[Partial recovery of the oculomotor nerve after section and repair during the excision of a tumor].

The authors report the case of a patient having suffered a section of the oculomotor nerve during the excision of a tumor located in the tentorium incisura. The nerve was immediately sutured (end to end anastomosis). After 18 months, a partial recovery of the function of the nerve was seen clinically and on EMG. Ptosis had disappeared and adduction of the eye reappeared. An analysis of available data on the suture of oculomotor nerves is then given: reparation of the third nerve in animals and man give variable and partial results. Partial recovery and aberrant regeneration are explained by th
