Fixed drug eruption in hands caused by omeprazole.

OBJECTIVE: Omeprazole is one of the most widely prescribed gastric antisecretory drugs. It is generally well tolerated and significant adverse reactions occur rarely. The objective of this report is to describe a case of fixed drug eruption that occurred during omeprazole treatment. CASE REPORT: A 37-year-old white female patient admitted with epigastric pain and heartburn symptoms. An upper gastrointestinal endoscopy revealed reflux esophagitis and the patient was given 20 mg b.i.d. omeprazole. She developed dark-red coloration on her hands, at the fourth day of treatment, which has been def…

Interaction of omeprazole with enteric-coated salicylate tablets.

OBJECTIVE: Enteric-coated tablets are designed to resist gastric fluids and to disrupt and dissolve in the alkaline medium of the small intestine. Main objective of the present study was to investigate whether the increase in gastric pH due to omeprazole treatment alters the release rate of a drug from enteric-coated formulation. To this end, we have compared the single dose pharmacokinetics of a single-unit enteric-coated salicylate to that of uncoated acetylsalicylic acid tablets in the presence and absence of omeprazole treatment. METHODS: Study was carried out according to 4 x 4 Latin squ…

A comparison of omeprazole and placebo for bleeding peptic ulcer.

BACKGROUND: The role of medical treatment for patients with bleeding peptic ulcers is uncertain. METHODS: We conducted a double-blind, placebo-controlled trial in 220 patients with duodenal, gastric, or stomal ulcers and signs of recent bleeding, as confirmed by endoscopy. In 26 patients the ulcers showed arterial spurting, in 34 there was active oozing, in 35 there were nonbleeding, visible vessels, and in 125 there were adherent clots. The patients were randomly assigned to receive omeprazole (40 mg given orally every 12 hours for five days) or placebo. The outcome measures studied were fur…

Growth of the parathyroid glands in omeprazole-treated chickens.

BACKGROUND: Omeprazole, a long-acting inhibitor of gastric acid secretion, is able to increase the circulating concentrations of gastrin. Daily treatment with high doses of omeprazole cause sustained hypergastrinemia. Long-standing hypergastrinemia can be expected to exert numerous effects in the body. For instance, gastrin has been proposed to promote growth in the digestive tract and pancreas. The present study is concerned with the effect of omeprazole on parathyroid glands in the chicken. METHODS: Chickens were treated with omeprazole (400 mumol/kg/day) in methylcellulose (2.5 ml/kg) for …

Comparison of omeprazole and nizatidine in the treatment of duodenal ulcers.

This study compares the efficacy and side effects of omeprazole and regular- and double-dose nizatidine in the treatment of duodenal ulcers. Duodenal ulcer healing rates in these three groups (omeprazole, 20 mg qd; nizatidine, 300 mg hs and 600 mg hs) were 81.8%, 19% and 30%, respectively, after two weeks of therapy; and 90.5%, 70% and 84.2%, respectively, after four weeks of treatment. Omeprazole had a significantly better healing rate than nizatidine, 300 mg or 600 mg, after two weeks of treatment (p < 0.01), but not after four weeks of treatment. Omeprazole relieved the ulcer pain sooner th…

Omeprazole in post-gastrocystoplasty metabolic alkalosis and aciduria.

The use of segments of stomach for bladder augmentation is gaining popularity in pediatric urology due to favorable muscular and secretory properties. However, in a renal failure patient who underwent gastrocystoplasty a high level of acid production within the bladder associated with persistent hypergastrinemia was noted leading to severe systemic metabolic alkalosis. This condition was unresponsive to standard acid-inhibiting or neutralizing therapies but it was treated successfully with omeprazole, a proton-pump inhibitor recently introduced for treatment of peptic ulcer disease.

Omeprazole: a new approach to gastric acid suppression.

Omeprazole, one of a new group of antisecretory drugs, is a substituted benzimidazole that does not exhibit the anticholinergic or histamine H2 antagonistic properties of drugs such as cimetidine. This agent suppresses gastric acid secretion by inhibiting the proton pump mechanism, thereby blocking the final step of acid secretion. Omeprazole is significantly more effective than the histamine H2 receptor antagonists in eliminating acid secretion; thus, it may be beneficial in patients who are resistant to these agents. Omeprazole is indicated for severe erosive esophagitis, gastroesophageal re…

Comparison of omeprazole with ranitidine in the treatment of reflux oesophagitis.

The amount and concentration of acid in the refluxed gastric contents are considered to be the major factors in the development and progression of reflux oesophagitis. Omeprazole, with its pronounced inhibition of gastric acid secretion, should therefore be a most effective drug for the treatment of patients with this disease. In an international clinical programme, the effects of treatment with placebo, ranitidine, 150 mg b.d. and omeprazole in doses ranging from 20 mg to 60 mg once daily, have been compared in patients with erosive and/or ulcerative oesophagitis. One placebo-controlled study…

Omeprazole in the short-term treatment of reflux oesophagitis.

Studies with short-term administration of omeprazole in patients with erosive or ulcerative reflux oesophagitis have demonstrated that strong inhibition of gastric acid secretion will become the therapy of choice for these patients. However, because reflux oesophagitis relapses rapidly when short-term treatment is discontinued, these patients are candidates for maintenance treatment. In most cases, long-term treatment will also require strong inhibition of gastric acidity. Until more information on the long-term safety of strong inhibition of gastric acid secretion is available, this therapeut…

Safety experience from long-term treatment with omeprazole.

Omeprazole was administered for up to 6 years in 859 patients for 'prevention of relapse in patients with poorly responsive peptic ulcer or reflux oesophagitis'. The pattern of adverse events reported during long-term treatment was similar to the adverse-event profile in short-term treatment with omeprazole (n = 2,818), ranitidine (n = 1,572) and cimetidine (n = 891). Omeprazole had essentially the same adverse-event profile as the two H2-receptor antagonists. The adverse-event profile for omeprazole during long-term treatment did not differ from that seen during short-term treatment with eith…

Omeprazole: effective, convenient therapy for Zollinger-Ellison syndrome.

The acute and long-term effects of omeprazole on gastric acid secretion were examined in 11 patients with Zollinger-Ellison syndrome. Basal gastric acid secretion was inhibited by 50% 3 h after a single 60-mg dose of omeprazole and 78% 4 h after administration of omeprazole. Patients were treated with a single daily dose of omeprazole, and the dose requirement was defined as the lowest dose of omeprazole that would reduce gastric acid secretion to less than 10 mEq/h during the last hour before the next dose. The mean daily dose requirement was 70 mg (range 20-160 mg). Ten of the 11 patients we…
