Systemic diseases involving the orbit.

A variety of systemic pathological processes may involve the eye and the surrounding orbital adnexae. CT and MRI of the orbit have become useful clinical adjuvants, not only in establishment of the diagnosis, but also in suggesting the appropriate clinical treatment, sometimes preventing needless biopsy. Grave's disease, Wegener's granulomatosis, and sarcoidosis may all present initially with orbital disease before the onset of systemic manifestations. Idiopathic inflammatory disease of the orbit and lymphoproliferative disease of the orbit continue to remain radiological and clinical challeng

Cystic schwannoma of the orbit.

Cystic schwannoma of the orbit is unreported in the world literature. A 54-year-old Caucasian man presented to our clinic with a 5 year history of progressive right-sided proptosis and diplopia. A large paramedian cystic mass displacing the right globe downwards and laterally was demonstrated. From the clinical and radiological features, the presumptive diagnosis of right frontal sinus mucocoele was made. However, the excisional biopsy of the lesion revealed the rare diagnosis of cystic schwannoma, arising from a branch of the first division of the trigeminal nerve. Post-operatively, the patie

[Vascular tumors of the orbit].

The author presents 536 cases of vascular tumors of the orbit. The most common orbital vascular lesions are cavernous hemangioma, capillary hemangioma, and lymphangioma. The hemangiopericytoma are less frequent than the three other types. The clinical and paraclinical characteristics of the most frequent tumors are analyzed and the various possibilities of treatment are discussed.

Pyoderma gangrenosum of the orbit.

Pyoderma gangrenosum is an uncommon chronic ulcerative condition, the aetiology of which is poorly understood. Ophthalmic involvement is rare. The case presented in this paper involves destruction of the orbital contents with subsequent perforation of the eye despite conventional therapy. Evisceration was performed with the patient being subjected to hyperbaric oxygen therapy pre- and post-operatively, with resultant cessation of the disease process.

Hematic cyst of the orbit.

We describe a case of a 44-year-old man, presenting with progressive proptosis of the right eye. Computed Tomographic (CT)-examination showed a cystic mass in the lacrimal gland fossa of the right orbit. The lesion was removed in toto. Histological examination showed evidence of old and recent bleedings and a chronic inflammatory reaction with numerous foreign-body giant cells without epithelial lining. The diagnosis of an orbital hematic cyst was established. The pathologic and pathogenetic features of this lesion are discussed. The hematic cyst should be borne in mind as a possible cause of

[Rhinogenous phlegmons of the orbit].

During the past 15 years we treated at the ENT department and ophthalmological department of the hospital with policlinic type III in Nitra 10 patients with orbital phlegmons or an abscess of the eyelid after an eye injury treated and cured by antibiotic treatment and external drainage. Twelve patients had a rhinogenic phlegmon or abscess in the orbit (one patient had a dentogenic aetiology of the inflammation). This group of 12 patients were treated in addition to antibiotics in different combinations also surgically (in one case also by drainage of the abscess from a lateral quadrant of the

[Arteriovenous malformation of the orbit].

Arteriovenous malformation is a rare vascular disease of the orbit that has to be differentiated from arteriovenous fistulas (e.g., of the carotid sinus), arterial aneurysms, the varices and the angiomas. It develops in the presence of an underlying constitutional maldifferentiation of the vascular bed but usually only becomes manifest in early adulthood. Typical clinical signs are exophthalmos, often with pulsation and thrill; episcleral and conjunctival vascular congestion; secondary glaucoma; and, to varying degrees in the late stage, restricted movement with double images as well as functi

Benign osteoblastoma of the orbit.

Benign osteoblastoma is a rare lesion of the orbit--only five cases have been described previously. The usual location is in the vertebral column and to a lesser extent in the skull. Although it is an invasive lesion, recurrences have not been reported after en bloc excision. The authors report a 10-year-old boy who presented with upper lid swelling, proptosis, and hypophthalmia. A computed tomography (CT) scan showed a calcified mass in the superior orbit which had eroded through the roof, and magnetic resonance imaging (MRI) showed a well-circumscribed mass between the periorbita and dura. A

Penetrating injuries to the orbit.

Although penetrating orbital wounds are an uncommon entity they are often associated with vision and life-threatening complications. By careful attention to the history and physical signs of the injured patient and the use of modern computed tomography (CT) scan imaging, the physician will be better able to make an accurate analysis and prognosis of the problem at hand as well as a well-planned therapeutic approach.

Subacute zygomycosis of the orbit.

A zygomycotic (phycomycotic) orbital cellulitis developed in a healthy 9-year-old boy. Because of the involved tissue's unfamiliar histopathologic features, an initial diagnosis of eosinophilic granuloma was made and appropriate treatment was delayed for more than six months. The patient's slowly progressive form of zygomycosis was clinically and morphologically similar to that described in previously recorded cases. All three cases occurred in otherwise healthy children living within the United States. Their involved tissues had mixed histopathologic features of chronic granulomatous entomoph

Subperiosteal abscess of the orbit.

Seven cases of subperiosteal abscess (SPA) of the orbit are reported. Anatomic relationships shared by the orbits, paranasal sinuses, and facial venous system explain the rapid development of SPA after periorbital infection. Computed tomography and echography allow distinction between SPA and other stages of orbital inflammation that are often indiscriminately grouped under the rubric of orbital cellulitis. These techniques suggest that elevated orbital pressure is the primary, and potentially reversible, mechanism of visual impairment produced by SPA. Difficulty in accurately determining the

[Computer tomography of the orbit].

CAT scanning of the orbit is greatly facilitated by the distinctly different absorption characteristics of the various orbital tissues. Intraorbital tumours can be defined accurately with respect to size, topographic position and extent. Differentiation between the benign or malignant character of orbital tumours is less well achieved by CAT scanning than by sonography; therefore sonography is needed as a complementary diagnostic modality.

Lymphoid pseudotumour of the orbit.

A retrospective study of 55 patients with orbital lesions diagnosed on clinical grounds as cases of inflammatory pseudotumor was carried out to discover the relationship, if any, between the clinical and histological features and the response to treatment. It was found that few clinical parameters had a bearing on the subsequent progress of the lesion but B-scan ultrasonography was useful in identifying the lymphoproliferative mass. In 28 cases wherein histological material was available, the presence of follicles with germinal centres was found to indicate a good prognosis, whereas the presen

Vascular problems of the orbit.

Vascular neoplasms and malformations of the orbit are unusual and fascinating problems. The benign capillary and cavernous hemangiomas are the most common vascular tumors and are classified separately because of distinct clinical and pathologic differences. The capillary hemangiomas progress rapidly in early childhood but have a tendency to regress. The cavernous hemangiomas occur later in life, progress slowly, and often require surgical excision. The orbital aneurysm, venous malformation, arteriovenous malformation, and carotid-cavernous fistula all may cause exophthalmos, visual loss, and o
