PURPOSE: Undescended testicle after groin surgery is a condition traditionally approached through an inguinal incision with en bloc mobilization of the spermatic cord and external oblique fascia, and extensive dissection of the proximal spermatic vessels. We report on a single surgeon series of orchiopexies after prior inguinal surgery approached through a single scrotal incision.

PURPOSE: Laparoscopic orchiopexy is a safe operation. However, the bladder can be injured during creation of the transperitoneal tunnel for the cryptorchid testis. We reviewed our experience with this complication.

BACKGROUND AND PURPOSE: The Fowler-Stephens orchiopexy is performed laparoscopically both as a single-stage as well as a two-stage procedure. We hypothesize that with the right technique, the single-stage procedure is just as successful as the two-stage procedure.
METHODS: The records of all boys who underwent a laparoscopic Fowler-Stephens orchiopexy (LFSO) for a nonpalpable testis at our institution from August 1995 to February 2007 were reviewed. Office charts and operative reports were reviewed for all patients.

PURPOSE: Laparoscopic orchiopexy is extremely effective for treating patients with nonpalpable testis. Study of fecundation ability following unilateral experimental cryptorchism as a function of the timing of orchiopexy. Two groups of 45 Wistar rats underwent unilateral mechanical cryptorchism and early and late prepuberal posterior orchyopexy, respectively. A third group of 45 rats underwent a dummy intervention. Mating taking place in subgroups of 15 animals at different ages showed that in the experimental animal, and therefore in any non-dysgenetic test, orchiopexy performed at the prepuberal period allows preservation of fecundation ability.

Early pubertal albino rats aged 20 days were made bilaterally cryptorchid by surgery, and groups with 5, 15, 25, and 35 days of cryptorchidism were submitted to bilateral orchiopexy. Fertility of all rats 110 days of age was tested by mating with female rats. Despite histological changes in the germinal epithelium of 35-day cryptorchid testes, orchiopexy restored fertility.

Fifty-six patients treated surgically for cryptorchidism between 1950 and 1975 were assessed for fertility. Their age at the birth of their first child was used as the parameter. A direct comparison with the normal population of different regions was available from the General Register Office of Scotland, obviating the need to use female statistics for comparison. The results showed that only very late surgical treatment of unilateral cryptorchidism affected fertility.
