Dose prediction of organs at risk in patients with cervical cancer receiving brachytherapy using needle insertion based on a neural network method.

OBJECTIVE: A neural network method was employed to establish a dose prediction model for organs at risk (OAR) in patients with cervical cancer receiving brachytherapy using needle insertion. METHODS: A total of 218 CT-based needle-insertion brachytherapy fraction plans for loco-regionally advanced cervical cancer treatment were analyzed in 59 patients. The sub-organ of OAR was automatically generated by self-written MATLAB, and the volume of the sub-organ was read. Correlations between D2cm3 of each OAR and volume of each sub-organ-as well as high-risk clinical target volume for bladder, rect…

Urinary Organs-at-Risk for Radiation Therapy Following Radical Prostatectomy: Contouring Guidelines on Behalf of the Francophone Group of Urological Radiation Therapy (GFRU).

PURPOSE: Urinary toxicity following radical prostatectomy (RP) and postoperative radiation therapy (RT) includes urinary incontinence and vesicourethral anastomosis strictures. With the increasing use of stereotactic body RT (SBRT), dose escalation, and reirradiation of the prostate bed (PB), standardization of the definition of urinary organs-at-risk (OARs) in the post-RP setting is needed. This work aimed to provide a comprehensive review of the anatomic and physiopathological changes occurring after RP, as well as a consensus on urinary OAR delineation for prostate cancer external beam RT i

Benefit of using motion compensated reconstructions for reducing inter-observer and intra-observer contouring variation for organs at risk in lung cancer patients.

BACKGROUND AND PURPOSE: In lung cancer patients, accuracy in contouring is hampered by image artefacts introduced by respiratory motion. Wit … hese scans were reconstructed as average and motion compensated datasets. 5 observers contoured the organs at risk (trachea, oesophagus, proximal bronchial tree, heart and brachial plexus) for each patient and each reconstruction. Contours were compared against a STAPLE volume with distance to agreement metrics. Intra-observer variation was assessed by redelinea …

Transfer learning for auto-segmentation of 17 organs-at-risk in the head and neck: Bridging the gap between institutional and public datasets.

BACKGROUND: Auto-segmentation of organs-at-risk (OARs) in the head and neck (HN) on computed tomography (CT) images is a time-consuming component of the radiation therapy pipeline that suffers from inter-observer variability. Deep learning (DL) has shown state-of-the-art results in CT auto-segmentation, with larger and more diverse datasets showing better segmentation performance. Institutional CT auto-segmentation datasets have been small historically (n < 50) due to the time required for manual curation of images and anatomical labels. Recently, large public CT auto-segmentation datasets (n

Urinary Organs at Risk for Prostate Cancer External Beam Radiation Therapy: Contouring Guidelines on Behalf of the Francophone Group of Urological Radiation Therapy.

PURPOSE: The occurrence of genitourinary (GU) toxicity is a common adverse event observed after external beam radiation therapy (EBRT) for prostate cancer (PCa). Recent findings suggest that the dose delivered to specific urinary organs at risk (OARs) such as the ureters, bladder trigone, and urethra is involved in the development of GU toxicity. METHODS AND MATERIALS: A multidisciplinary task force including 3 radiation oncologists, a uroradiologist, and a urologist was created in 2022. First, OARs potentially involved in GU toxicity were identified and discussed. A literature review was per…

Machine learning-based detection of aberrant deep learning segmentations of target and organs at risk for prostate radiotherapy using a secondary segmentation algorithm.

Objective. The output of a deep learning (DL) auto-segmentation application should be reviewed, corrected if needed and approved before being used clinically. This verification procedure is labour-intensive, time-consuming and user-dependent, which potentially leads to significant errors with impact on the overall treatment quality. Additionally, when the time needed to correct auto-segmentations approaches the time to delineate target and organs at risk from scratch, the usability of the DL model can be questioned. Therefore, an automated quality assurance framework was developed with the aim …

Deep inspiration breath-hold radiation therapy in left-sided breast cancer patients: a single-institution retrospective dosimetric analysis of organs at risk doses.

BACKGROUND: Radiotherapy can induce cardiac injury in left-sided breast cancer cases. Cardiac-sparing irradiation using the deep inspiration breath-hold (DIBH) technique can achieve substantial dose reduction to vulnerable cardiac substructures compared with free breathing (FB). This study evaluated the dosimetric differences between both techniques at a single institution. METHODS: From 2017 to 2019, 130 patients with left-sided breast cancer underwent breast-conserving surgery (BCS; n = 121, 93.1%) or mastectomy (ME; n = 9, 6.9%) along with axillary lymph node staging (n = 105, 80.8%), foll

Deep Learning for Per-Fraction Automatic Segmentation of Gross Tumor Volume (GTV) and Organs at Risk (OARs) in Adaptive Radiotherapy of Cervical Cancer.

BACKGROUND/HYPOTHESIS: MRI-guided online adaptive radiotherapy (MRI-g-OART) improves target coverage and organs-at-risk (OARs) sparing in radiation therapy (RT). For patients with locally advanced cervical cancer (LACC) undergoing RT, changes in bladder and rectal filling contribute to large inter-fraction target volume motion. We hypothesized that deep learning (DL) convolutional neural networks (CNN) can be trained to accurately segment gross tumor volume (GTV) and OARs both in planning and daily fractions' MRI scans. MATERIALS/METHODS: We utilized planning and daily treatment fraction setu

Proton Treatment Techniques for Posterior Fossa Tumors: Consequences for Linear Energy Transfer and Dose-Volume Parameters for the Brainstem and Organs at Risk.

PURPOSE: In proton therapy of posterior fossa tumors, at least partial inclusion of the brainstem in the target is necessary because of its proximity to the tumor and required margins. Additionally, the preferred beam geometry results in directing the field distal edge toward this critical structure, raising concerns for brainstem toxicity. Some treatment techniques place the beam's distal edge within the brainstem (dose-sparing techniques), and others avoid elevated linear energy transfer (LET) of the proton field by placing the distal edge beyond it (LET-sparing techniques). Hybrid approache

Will weight loss cause significant dosimetric changes of target volumes and organs at risk in nasopharyngeal carcinoma treated with intensity-modulated radiation therapy?

This study aimed to quantify dosimetric effects of weight loss for nasopharyngeal carcinoma (NPC) treated with intensity-modulated radiation therapy (IMRT). Overall, 25 patients with NPC treated with IMRT were enrolled. We simulated weight loss during IMRT on the computer. Weight loss model was based on the planning computed tomography (CT) images. The original external contour of head and neck was labeled plan 0, and its volume was regarded as pretreatment normal weight. We shrank the external contour with different margins (2, 3, and 5mm) and generated new external contours of head and neck.

[Active detection of patients with tuberculosis of urinary organs from increased risk groups at the polyclinics of the general therapeutic-and-prophylactic network].

To timely detect tuberculosis of urinary organs (TUO), the authors have proposed a procedure for detecting the disease in increased risk groups. By analyzing 272 cases with TUO and 12 control persons, they established the most important risk factors. The authors have developed a procedure for stratification of contingents and differential measures in accordance with the risk (three degrees). The procedure has shown to be effective.

Comparing Organs at Risk Sparing Between Intensity-Modulated Radiotherapy and Pencil-Beam Scanning Plans Based on Disease Location for Hodgkin and Non-Hodgkin Lymphoma.

PURPOSE: Radiotherapy (RT) is often used as a combined modality therapy for patients with Hodgkin (HL) and aggressive Non-Hodgkin Lymphoma (aNHL). RT options include photon intensity-modulated radiotherapy (IMRT) or proton therapy (PT). As PT is a limited and costly resource, it is important to identify cases in which PT may best benefit in order to offer providers and patients the best possible information for decision-making. Accordingly, the purpose of this study was to assess the difference in OAR sparing between photon IMRT and pencil-beam scanning (PBS) based on disease location using th

Linear accelerator selection: impact on feasibility dose-volume histograms and practicality in dose reduction for organs at risk during prostate volumetric-modulated arc therapy.

BACKGROUND: The quality of volumetric-modulated arc therapy (VMAT) plans depends on the planner's expertise. A feasibility dose-volume histogram (FDVH) may help reduce planning variability. MATERIALS AND METHODS: This study evaluated the impact of two linear accelerators - Halcyon and TrueBeam - on FDVH in prostate VMAT planning and their effectiveness in reducing doses to organs at risk (OARs). VMAT plans were generated for 30 patients with prostate cancer, with planning target volume excluding the rectum (PTV-R) and OAR contours created by three planners. An FDVH was created, and plans were…
