We administered anesthesia for a 20-month-old boy with non-bullous ichthyosiform erythroderma who underwent orthopedic syndactyly repair on two occasions. Anesthetic considerations include the difficult fixation of the tracheal tube and iv cannulas and the risk of hypothermia. Intravenous access placement was also difficult due to the hyperkeratosis and the deformity of extremities. In the first surgery, pharyngeal temperature fell to 34.4 degrees C despite the use of the 37 degrees C circulating water mattress and room temperature set to 26 degrees C. It gradually returned to 36 degrees C dur

Surgical instrumentation for the correction of adolescent idiopathic scoliosis (AIS) is a complex procedure involving many difficult decisions (i.e. spinal segment to instrument, type/location/number of hooks or screws, rod diameter/length/shape, implant attachment order, amount of rod rotation, etc.). Recent advances in instrumentation technology have brought a large increase in the number of options. Despite numerous clinical publications, there is still no consensus on the optimal surgical plan for each curve type. The objective of this study was to document and analyse instrumentation conf

Beals syndrome (congenital contractural arachnodactyl) is a genetic disorder of the connective tissue phenotypically related to Marfan syndrome. It is characterised by dolichostenomelia, arachnodactyly, multiple joint contractures, crumpled ears, hypoplastic muscles and scoliosis. The latter, the most important clinical feature of this rare condition, presents in the infantile and juvenile age group and has a tendency to rapid progression. Bracing often fails to control the scoliosis and surgery is the recommended treatment. We present our experience of two cases managed with the paediatric Is

Olecranon fractures are common upper-extremity fractures amenable to various surgical techniques. The authors conducted a systematic review and meta-analysis to evaluate surgical management strategies for olecranon fractures in adult patients. Two investigators independently screened articles from records obtained from MEDLINE, Embase, SPORTDiscus, CINAHL, and the Cochrane Library databases. The authors compared tension-band wiring (TBW) with plate fixation (PF) for the outcomes of complications and hardware removal. Subgroup analyses were conducted comparing randomized controlled trials with

Advancement in orthopedics have been increasing rapidly. The most important advances have been in fixation. With time, metallic hardware will begin to be replaced by materials that become one with the body. This progress will not only aid in the repair process it will allow permanent and improved reinforcement of the fixated region. Biointegrative technology is a promising new generation of materials capable of achieving this goal. Over time, it is expected that plates, screws, pins, interference screws, and even possibly joint replacements will incorporate into patients' bodies, negating the

Pediatric supracondylar humerus fractures are the most commonly encountered type of elbow fractures in children that require surgical fixation. Many pediatric supracondylar humerus fractures can be treated with closed reduction and percutaneous skeletal fixation. In difficult fractures, adjunct pin techniques, such as joystick wires and leverage pins, can be used to help attain a satisfactory and stable reduction before an open approach is used. After the fracture is reduced, optimal pinning, with the use of either crossed or lateral-entry techniques, and fixation that achieves maximal spread

OBJECTIVE: To approach the clinical curative effect of treatment of the disease of vertebral column by the arc-track private lock pedicle orthopedics fixation system (ALPF II).
METHODS: Eighty-six cases were treated by useing ALPF II. The average age of the cases was 36.8 years (range from 18 to 69 years),the male was 39 cases and the female was 37 cases. The results were evaluated and all the cases were followed-up.
RESULTS: All the patients were followed-up for 9 to 30 months. The amelioration of patients nerve function, spinal motion, low back pain and leg pain was 94.1%, 65.9%, 92.1% and

A new technique is presented for the treatment of comminuted intraarticular fractures of the base of the thumb metacarpal. A triangular Hoffmann external radius and first- and second-metacarpal fixator is used. The reduction is maintained by prolonged distraction and abduction of the first metacarpal bone. The application of the technique in three cases is reported.

Problems in the diagnosis, reduction, and maintenance of reduction of dislocations of the tarsometatarsal joints are discussed. Many different patterns may present due to the anatomy of the foot and ankle and the different mechanisms of injury. Closed reduction and maintenance of reduction may be difficult to achieve, and open reduction may be required. Fixation with multiple K wires is useful in maintaining reduction. The possibility of vascular compromise should be kept in mind. Although anatomical reduction should be the goal of treatment, this does not always assure a painless foot. Arthro

OBJECTIVE: Traumatic long bone fractures require precise reduction and stable fixation to achieve optimal outcomes during open reduction and internal fixation (ORIF). Conventional bone-holding instruments are often associated with prolonged operative time, increased blood loss, reliance on surgical assistance, and higher complication rates. This study aimed to evaluate the clinical effectiveness of a newly designed orthopedic repositioning device in reducing operative time, intraoperative blood loss, postoperative pain, and complications compared with conventional ORIF techniques.

Our goals in writing this article are to facilitate understanding of issues related to (1) why anterior fixation for thoracolumbar fractures are an important tool for managing these injuries, (2) when to perform these as a single procedure, or in combination with other procedures such as vertebrectomy and/or posterior stabilization and fusion, (3) to appreciate the biomechanical and design-related issues of available systems, and (4) what the clinical outcomes are following these procedures.

The Hartshill frame is method for segmental osteosynthesis of the spine. It is attached at each vertebral level by sub-laminal metal wires. This frame therefore ensures immediate stability and avoids the need for a post-operative corset. It enables correction of frontal deformities (scoliosis) whilst retaining sagittal physiological curves (lumbar lordosis--dorsal kyphosis). It ensures the fixation of posterior spinal instability (destruction of posterior ligaments and bone tissue, spondylolisthesis, etc.). The Hartshill frame can thus be seen to be a posterior brace. It has been used in 92 pa

The two important fractures of the trapeziometacarpal joint are Bennett's fracture and Rolando's fracture. Bennett's fracture is a fracture-dislocation of the joint. The preferred treatment is closed reduction and percutaneous pinning of the fracture with additional fixation through the trapeziometacarpal joint verified by anteroposterior (AP) and lateral roentgenograms. If the fracture is not reducible, and open reduction through the thenar approach with Herbert screw fixation is recommended. Rolando's fracture is a comminuted T- or Y-shaped fracture through the base of the first metacarpal.
