GLP-1 Agonists in Orthopedic Surgery: A Narrative Review of Bone Health and Surgical Implications.

Glucagon-like peptide-1 (GLP-1) receptor agonists, commonly used for glycemic control in patients with type 2 diabetes and for weight loss in obese patients, have been increasingly used due to their effectiveness in treating these conditions and in reducing cardiovascular events. Yet evidence is limited surrounding their impact on bone health and on patients undergoing orthopedic procedures. This narrative review explores the mechanisms of action of GLP-1 agonists, their effects on bone health, and the implications of their use in perioperative patents undergoing orthopedic surgery, with an em…

Surgery in patients with hemophilia: is thromboprophylaxis mandatory?

Patients with hemophilia are considered low risk for thromboembolic complications. However, in the presence of risk factors for thrombosis, … for hemophilia patients the use of venous thrombosis prophylaxis during major surgeries, including orthopedic procedures remains controversial. For the majority of the patients the use of gradated compression stockings and early mobilization can be sufficient to prevent venous thromboembolism. The use of anticoagulant prophylaxis should be considered just for patients …

The correlation between biomechanical experiments and clinical studies in orthopedics: a systematic review.

PURPOSE: To evaluate the clinical relevance of biomechanical experiments by conducting a systematic review of orthopedic studies that compared treatments with both a biomechanical experiment and a clinical study. METHODS: A systematic search of MEDLINE/PUBMED was performed to identify orthopedic studies that included both a biomechanical experiment and a clinical study to compare treatments. The association between a difference in the outcome of the biomechanical experiment and a difference in the outcome of the clinical study was evaluated. RESULTS: There were 23 studies that were included.

Caught in the Crossroads: Damage Control Orthopedics versus Early Total Care for Polytrauma in the Indian Scenario.

Polytrauma patients are defined by the Berlin Consensus as those with severe injuries across multiple regions and require complex, adaptable treatment approaches to address both immediate and long-term needs of the patient, while considering the resource availability. This review evaluates early total care (ETC), which attempts early definitive fixation, against damage control orthopedics (DCO), which focuses on temporary stabilization to reduce physiological strain from surgery. While ETC promotes early mobilization, DCO reduces risks and subsequent complications associated with the "second h

Reporting bias in orthopaedic science-Are we telling the whole story?

Orthopaedic congresses and journals are dominated by positive results, creating the impression that modern surgical techniques yield uniformly excellent outcomes. In reality, complications, suboptimal results, and abandoned techniques remain underreported-a reflection of outcome reporting bias (ORB) and publication bias. Professional incentives, reputational concerns, and competitive pressures favour success stories while discouraging the publication of negative findings. Yet, these data are crucial for scientific progress and patient care, offering lessons that help avoid repeated mistakes. T

Validity Evidence for Procedure-specific Competence Assessment Tools in Orthopaedic Surgery: A Scoping Review.

INTRODUCTION: Competency-based training requires frequent assessment of residents' skills to determine clinical competence. This study reviews existing literature on procedure-specific competence assessment tools in orthopaedic surgery. METHODS: A systematic search of eight databases up to May 2023 was conducted. Two reviewers independently assessed validity evidence and educational utility of each assessment tool and evaluated studies' methodological quality. RESULTS: Database searching identified 2,556 unique studies for title and abstract screening. Full texts of 290 studies were reviewed

Genitourinary complications in orthopaedic surgery.

Genitourinary complications following orthopaedic intervention are uncommon but well-described occurrences and exist on a spectrum of severity. These complications vary depending on the anatomic location and surgical approach, with surgery of the spine, hip, and pelvis of particular concern. Injuries to the urinary tract may present acutely with urinary retention or hematuria. However, they often have a delayed presentation with severe complications such as urinary fistula and recurrent infection. Delayed presentations may place the onus of timely and proper diagnosis on the orthopaedic provid

Analysis of orthopedic surgery-related incidents in operating rooms using a nationwide incident reporting database.

BACKGROUND: Patient safety is crucial in high-risk specialties such as orthopedic surgery due to the significant incidence of preventable adverse events. Analyzing extensive databases of orthopedic surgery-related incidents in operating rooms is vital for enhancing medical safety and identifying targeted interventions. This study analyzed orthopedic surgery-related incidents in operating rooms using a nationwide incident reporting database in Japan to identify risk factors associated with severe harm. METHODS: We extracted orthopedic surgery-related incidents in the operating room from the Ja

MR imaging of the postoperative knee.

Advances in orthopedic and arthroscopic surgical procedures of the knee such as, knee replacement, ligamentous reconstruction as well as articular cartilage and meniscus repair techniques have resulted in a significant increase in the number of patients undergoing knee arthroscopy or open surgery. As a consequence postoperative MR imaging examinations increase. Comprehensive knowledge of the normal postoperative MR imaging appearances and abnormal findings in the knee associated with failure or complications of common orthopedic and arthroscopic surgical procedures currently undertaken is cruc

French surgical experience in the role 3 medical treatment facility of KaIA (Kabul International Airport, Afghanistan): the place of the orthopedic surgery.

INTRODUCTION: In 2009, the French took command of the Medical Hospital (MH) or Role 3 Hospital at KaIA (Kabul International Airport) within the framework of its role in the military mission Operation Pamir in Afghanistan. The goal of this study was to analyze the volume of orthopedic surgical activity for the last four years, to identify its specificities and to improve training of military orthopedic surgeons. HYPOTHESIS: Orthopedic surgery is the most important activity in the field and surgeons must adapt to situations and injuries that are different from those encountered in France. PATI

Trends in venous thromboembolic events and anticoagulation prescriptions for orthopaedic surgery of the shoulder.

BACKGROUND: There is a scarcity of literature discussing anticoagulation prescribing patterns after upper extremity orthopedic surgery. The purpose of this study is to use a large national database to identify trends in adverse events and anticoagulation prescribing following orthopedic shoulder surgery. MATERIALS AND METHODS: The TriNetX Research Network was used to identify all patients undergoing orthopedic shoulder surgery between January 2004 and December 2022. Patient information was stratified by year and type of surgery including arthroplasty, arthroscopy, soft tissue procedures, bony

Case Volume Benchmarks During Residency and Fellowship Training for Pediatric Orthopedic Surgeons.

Higher case volumes correlate with improved clinical outcomes across surgical specialties. This study establishes case volume benchmarks after completion of pediatric orthopedic fellowship training. Case logs for orthopedic surgery residents and pediatric orthopedic fellows at accredited programs were analyzed (2017-2018 to 2020-2021). Case volumes for pediatric orthopedic surgery were compared using parametric tests. Case logs from 3,000 orthopedic surgery residents and 149 pediatric orthopedic fellows were analyzed. There was an increase in total pediatric cases among orthopedic surgery resi
