Self-regulated frequent power pinch exercises: A non-orthotic technique for the treatment of old mallet deformity.

The utilization of an orthotic device to treat a mallet finger injury is common practice. This author describes a different approach to treating patients with an old mallet finger injury. The incorporation of frequent, self-regulated exercises without the use of an orthosis is described.--Victoria Priganc, PhD, OTR, CHT, CLT, Practice Forum Editor.

Is helmet therapy effective in positional plagiocephaly? A systematic review and proposal for an updated management algorithm.

Positional plagiocephaly, also known as deformational or non-synostotic plagiocephaly, is a common cranial deformity in infancy resulting from sustained external mechanical forces acting on the developing skull. Its prevalence has increased in recent decades, paralleling widespread adoption of supine sleep positioning recommendations. Although generally considered a benign condition, uncertainty persists regarding optimal management strategies, particularly the role and timing of cranial orthotic therapy. Conservative approaches, including repositioning techniques and physical therapy, are fre

Dynamic estimation of soft tissue stiffness for use in modeling socket, orthosis or exoskeleton interfaces with lower limb segments.

Modeling the interface between the lower limb segments and a socket, orthosis or exoskeleton is crucial to the design, control, and assessment of such devices. The present study aimed to estimate translational and rotational soft tissue stiffness at the thigh and shank during daily living activities performed by six subjects. Smooth orthogonal decomposition (SOD) was used on skin marker trajectories and fluoroscopy-based knee joint kinematics to compute stiffness coefficients during squatting, sitting and rising from a chair, level walking, and stair descending. On average, for all subjects an

Tremor Control Devices for Essential Tremor: A Systematic Literature Review.

BACKGROUND: There is a growing interest in nonpharmacological approaches for essential tremor (ET), including tremor cancelation devices. However, the true efficacy of such devices in ET remains unclear. METHODS: A systematic literature review was conducted using standardized criteria regarding efficacy and comfortability. Devices focused on design or experimental testing in which tremor was simulated in a robot were excluded. RESULTS: Out of 324 articles initially identified, 12 articles were included. Orthoses using biomechanical loading and neuromodulation with electrical stimulation, and

[Thomas George Morton metatarsalgia].

Plantar interdigital neuroma, also called Morton neuroma, neuralgia, or metatarsalgia, is a perineural fibrosis that induces severe intermittent pain and paresthesia, most frequently between the third and fourth metatarsal heads. This compression syndrome or entrapment neuropathy involves one branch of the common plantar digital nerves. Diagnosis is based essentially on clinical examination. Imaging techniques may be useful in the diagnosis of atypical cases and postoperative recurrences. In most case, conservative treatment (such as orthotic shoes or devices or injections) is successful. When

[Cogenital femoral defect. Indication, therapy and complication management].

Congenital femoral deficiencies remain a surgical challenge. There is no common classification: both radiological and clinical methods are recommended. Depending on the severity of the deformity, reconstructive and lengthening techniques are performed, while accompanying deformations are taken into consideration. In addition, amputation or fusion techniques together with orthotic devices are used. A total of 35 patients (37 extremities) with congenital femoral deficiencies have been treated at the Orthopaedic Hospital Vienna-Speising, from 1982 to 1998. 24 extremities were treated with reconst…

Do current upper limb orthotic classification systems help clinicians choose and design effective orthoses? A scoping review with expert interviews.

BACKGROUND: Orthosis (orthotic) fabrication is an essential part of the treatment plan for many upper extremity conditions. PURPOSE: We aim to comprehensively identify the current body of evidence about the purpose, structure, scope, and application of available orthotic classification systems. Our secondary aim was to know if the current classification systems can be used as a decision guide for clinicians. STUDY DESIGN: A scoping review. METHODS: A scoping review of research studies identified through data-based and gray literature was conducted to determine studies that addressed classif

Study of the compression behavior of functionally graded lattice for customized cranial remodeling orthosis.

Deformational plagiocephaly is a head deformity that occurs in newborns, treated in severe cases with helmets named cranial remodeling orthoses (CRO). Current CROs can fail to adapt to head growth, causing excessive pressure sores and other complications, and may lead to poor clinical results. In this work, we experimentally and numerically study the compressive behavior of a functionally graded lattice that may be used in future work as a potential inner lining for a CRO with a customized density distribution. This work is divided into five stages. First, we describe the design of all lattice

[A comparison of ankle fixation between two kinds of low-temperature plastic anterior ankle foot orthoses].

The low-temperature ankle foot orthosis (AFO) was designed in the anterior leaf type, called anterior AFO, to meet the need of indoor barefoot walking. It could be easily molded and remolded to fit the foot, as well as to adjust the position of the foot and ankle. Two kinds of low-temperature plastic anterior ankle foot orthoses, anterior encased and anterior direct molding ankle foot orthoses, were compared to analyze the effectiveness of ankle fixation in plantar flexion and inversion. Force was applied to each of the anterior AFOs at the anterior-middle and anterior-lateral sides to simulat

Cerebrovascular accidents.

CVA comprises a large number of clinical entities, depending on the site of infarction in the brain. Accurate evaluation of deficits in the patient's sensory and/or motor systems and the patient's intellectual status are paramount in establishing realistic rehabilitation goals. With respect to the motor system, two types of voluntary movement may occur. These include synergistic or pattern movement and selective movement. Spasticity in the affected lower extremity may result in a variety of lower-extremity deformities and contractures. Those most commonly encountered include hip flexion and ad

Gait analysis. Alterations in support phase forces using supportive devices.

This study documents changes in momentary distribution of forces under the foot, comparing barefoot gait to that with heel cups, medial arch supports, and low-dye taping. Cholesterol crystal force plate analysis and a computerized Kistler force platform were used in the study. Low-dye taping or a heel cup significantly diminished the duration of forces under the midfoot, medializing the instant center of forces. A medial arch support shifted the instant center of forces laterally, though it did not diminish the duration of forces under the arch. The relationship between these alterations of fo
