Osteogenesis Imperfecta, a heterogenous group of inherited disorders, is characterized by both bone fragility and low bone mass. Since the Sillence classification of the disease, significant progress has been made in understanding the molecular, histological and genetical pathogenesis of the disease, and it led to expanded nosology. The multidisciplinary approach including rehabilitation, medications and surgery, may significantly improve the quality of life and prolong life expectancy.

Osteogenesis imperfecta (OI) is a kind of genetic osteoporosis, whose clinical severities vary from patients to patients. In type I OI, some patients appear normal without investigating bone mineral density (BMD) by DXA. In severe OI children, pamidronate infusion therapy is effective, however it is still uncertain when to stop the pamidronate therapy without having the problems of bone fracture healing. Also, the appropriate therapy for adult and post-menopoause OI patients remains to be clarified in future.

OBJECTIVE: To discuss the case of a 42-yr-old weightlifter with osteogenesis imperfecta.
CLINICAL FEATURES: The patient had bilateral acute elbow pain and a previous history of more than 35 fractures of the spine and extremities.
INTERVENTION: There is no current treatment for osteogenesis imperfecta. Treatment objectives were designed to minimize pain, improve range of motion, and decrease stress on the elbow joints. Nutritional supplementation was used to help maintain bone density. The elbow pain improved with treatment, and the patient has had no new fractures in the last 6 years.

Distraction osteogenesis was used to correct congenital hand deformities in 12 digits. The patients' ages at the time of distraction ranged between 2.5 and 7 years. A total of nine patients underwent lengthening of 12 bones. The average period of distraction was 31.1 +/- 17.6 days. The average length of distraction was 23.6 +/- 7.3 mm. This review describes the technique of distraction osteogenesis, outlining its application and current developments for the correction of craniofacial anomalies. Also described are some of the difficulties and complications which can arise with speculation on how this may be overcome. Finally, there is an exploration of how this technique might be routinely used to deal with orthognathic, orthodontic and alveolar problems.

Osteogenesis imperfecta (OI) is a rare, inherited condition in which not only the bones are abnormally brittle but also abnormalities in the ligamentous structures, of the eye, ear and skin coexist. Three members of the same family experienced the pathologic fractures with squaring deformity, but no blue sclerae or deafness. They presented with a relatively benign course treated conservatively.

Operative findings and results of stapedectomy in 32 patients (43 ears) with osteogenesis imperfecta are presented. The stapes footplates were in many ears thick and of a very soft consistency. In all the operated ears we found the stapes footplates fixated in the oval window. Brittle and atrophic stapes crura were a common finding, but in only 5 ears fractures of the crura were observed.
