Cervical osteomyelitis following tonsillectomy.

We present a case of fatal cervical osteomyelitis following an elective tonsillectomy in a previously fit young man. Following induction of general anaesthesia, and prior to surgery, the patient received bilateral glossopharyngeal nerve blocks with 0.5% bupivacaine and adrenaline 1:200,000. The initial recovery was uneventful but persistent throat and neck pain developed at home which was diagnosed as a throat infection and possible hyperextension injury of the neck. It is impossible to say how much the dissection of chronically infected tonsils or the infiltration of local anaesthetic into or…

[Osteomyelitis from Cryptococcus neoformans].

A forty-three-year old patient suspected of having a bone tumor in his left femur, was submitted to the hospital. However, osteomyelitis caused by Cryptococcus neoformans has been demonstrated by culture. The history of the patient revealed a long-term therapy with corticosteroids due to sarcoidosis. The osteomyelitis was treated with fluconazole (200 mg daily p.o.) for three months. Under this therapy the infection resolved.

Cryptococcus neoformans vertebral osteomyelitis.

A 67-year-old previously healthy woman presented with low back pain of 2 months duration and daily fever of 39 degrees C for 3 weeks. CT sca … w-up of 2 years. Since this case, as well as most of the previously described cases of cryptococcal osteomyelitis, were in normal hosts, cryptococcal osteomyelitis should be considered in the differential diagnosis even in a normal host, and therefore, prior to possible invasive diagnostic procedures, cryptococcal antigen in the serum should be determined.

Case report: Nocardia osteomyelitis.

Osteomyelitis is an unusual manifestation of infection with Nocardia spp, and most cases result from direct extension from a primary lung or cutaneous lesion. The authors report what they believe to be the first case of Nocardia vertebral osteomyelitis occurring postoperatively, and review the literature on the 19 previously reported cases of Nocardia osteomyelitis due to hematogenous dissemination or direct inoculation, excluding those occurring from direct extension of a mycetoma or primary lung infection.

Torulopsis glabrata vertebral osteomyelitis.

A case of Torulopsis glabrata osteomyelitis of the thoracic spine producing spinal cord compression and myelopathy is reported. The patient displayed several of the predisposing factors to T. glabrata infection, including diabetes mellitus, a history of abdominal surgery, and intravenous catheterization with hyperalimentation. The patient was successfully managed with surgical decompression, debridement, and curettage of the affected bone, postoperative immobilization, and systemic amphotericin B therapy. Her pain was relieved, spinal cord function recovered, and the vertebrae healed uneventfu…

[Osteomyelitis after malleolar fractures].

Over a ten-year period, 485 patients suffering from malleous fracture were operated at the Department for Orthopaedic Surgery and Traumatology in Novi Sad. Postoperative infection appeared in ten cases (2.06%). Eight of these patients primarily had open fractures, and two of them had closed ones. Osteosynthesis of the lateral malleous had most frequently been treated by the use of panel and screws. Staphylococcus aureus was isolated as symptom of the infection in eight cases. In treatment of malleous osteomyelitis, the debridement of focus of infection together with fixation material extractio…

[Osteomyelitis following BCG vaccination].

Typical for the case history of a patient with the very rare vaccination-induced osteomyelitis is a relatively long interval between the BCG-vaccination and the initial clinical symptoms. The course of the disease is undramatic. Pronounced changes are frequently visible in x-rays and, with proper surgical and tuberculostatic therapy, the prognosis is good as well. The morphological changes in cases of BCG-osteomyelitis cannot be distinguished from those caused by tuberculosis. The same causal factors as for other possible BCG-complications are considered pathogenically significant for its deve…

Primary subacute epiphyseal osteomyelitis.

In eight children with primary subacute osteomyelitis of a femoral or tibial epiphysis, the only complaints were pain and limp. Plain roentgenograms and tomograms showed a well defined lytic lesion in the epiphysis and no evidence of any connection to the metaphysis. Although the bone of the epiphysis was involved in every instance, and the lesion extended to the articular cartilage in most patients, the cartilage itself was not damaged. When the lesions were curetted, six were sterile on routine and anaerobic cultures and on culture for tuberculous and fungal organisms, and Staphylococcus was…

A Case of Odontoid Osteomyelitis.

Odontoid osteomyelitis is a rare infectious disease that manifests as fever and posterior neck pain, while crowned dens syndrome is a relatively common inflammatory disorder with similar signs and symptoms. We describe the case of a 90-year-old woman presenting with fever, posterior neck pain, throat pain, and headache. Crowned dens syndrome was initially diagnosed based on the clinical picture and calcification around the odontoid process on cervical spine CT. However, the diagnosis was revised to odontoid osteomyelitis following the detection of Staphylococcus aureus in blood cultures that w…

Tuberculosis Osteomyelitis of the Wrist.

Wrist Mycobacterium tuberculosis (TB) complex osteomyelitis is rare, with polymicrobial TB osteomyelitis even more uncommon. The authors describe an unusual case of polymicrobial TB wrist osteomyelitis. The case patient presented with a 2.5-year history of 2 insidiously growing nodules on his wrist. He underwent debridement, and tissue cultures grew methicillin-resistant Staphylococcus aureus, Enterococcus faecalis, and, later, TB complex. He was started on vancomycin, rifampin, isoniazid, pyrazinamide, and ethambutol with improvement in symptoms. This case emphasizes the importance of a broad…

Model of Osteomyelitis in Rats.

We developed a rat model of chronic osteomyelitis of the femur caused by Staphylococcus aureus. The clinical, histomorphological, and laboratory signs were compared in rats with osteomyelitis (infected) and control animals receiving saline. On day 21 after infection, clinical, histomorphological, and laboratory signs of chronic osteomyelitis were found in experimental rats.

The Epidemiology of Osteomyelitis in Children.

Pediatric osteomyelitis remains challenging to treat. Detailed epidemiological data are required to estimate future developments. Therefore, we aimed to analyze how the incidence has changed over the last decade depending on age, gender, osteomyelitis subtype, and anatomical localization. Cases were quantified for patients aged 20 years or younger, using yearly reported ICD-10 diagnosis codes from German medical institutions for the time period 2009 to 2019. Incidence rates of osteomyelitis increased by 11.7% from 8.2 cases per 100,000 children in 2009 to 9.2 cases per 100,000 children in 2019…

Osteomyelitis of the Glenohumeral Joint.

A 60-year-old man presented to the emergency department with constant right shoulder pain of insidious onset that had lasted for 1 month. Based on the patient's history, the physical therapist's recommendations, and initial radiographs, the physician ordered magnetic resonance imaging, which revealed shoulder osteomyelitis and myositis. J Orthop Sports Phys Ther 2019;49(11):865. doi:10.2519/jospt.2019.9101.

Xanthogranulomatous Osteomyelitis of the Tibia.

Background: Xanthogranulomatous osteomyelitis (XO) is a rare chronic inflammatory process that is histologically characterized by the presence of foamy macrophages, histiocytes, and plasma cells. Radiologic and gross examinations can mimic malignancy, so definitive diagnosis should be made by histopathologic evaluation. Case Report: A 15-year-old male presented with pain in the proximal right leg for 2 weeks prior to admission. The patient had a history of leg trauma 3 years prior that was responsive to as-needed analgesics. Laboratory data revealed increased erythrocyte sedimentation rate and…

Aspergillus Osteomyelitis of Clivus.

A 64-year-old man presented with a 2-month history of headache. The patient had a history of 30 years of diabetes. Magnetic resonance imaging revealed a lesion involving the sphenoid sinus and clivus. Surgery was performed through a transsphenoidal approach. Pathology suggested the diagnosis of the presence of Aspergillus. The patient received voriconazole treatment for 8 weeks, and 3-month follow-up demonstrated no progression. Aspergillus osteomyelitis of the skull base is uncommon, and accurate preoperative diagnosis is difficult. However, when there is a clear history of diabetes, a diagno…
