Anterior cervical osteophyte is a rare cause of dysphagia usually occurring in the elderly. We report two cases in which the anterior cervical osteophytes impinged upon the esophagus, and we describe their surgical management. Initial investigation of a case of dysphagia should be directed to the common causes; however, in the elderly and in those with disorders of the cervical spine an anterior cervical osteophyte may be the cause of dysphagia.

An uncommon cause of progressive dysphagia may be large cervical osteophytes impinging upon the cervical esophagus. Dysphagia may be due to the mechanical mass effect of a large anterior osteophyte, trapping the esophagus opposite a fixed point such as the cricoid cartilage, or a local inflammatory reaction resulting in cricopharyngeal spasm. Diffuse idiopathic skeletal hyperostosis (DISH) is a condition that causes abnormal bone growth at the sites of ligament insertion, mainly in the spine. It is of unknown etiology and usually affects older males. It is often asymptomatic, but it can sometimes cause dysphagia if it affects the anterior cervical spine. BACKGROUND: Our aim was to investigate the clinical and manofluorographic findings of patients with anterior cervical osteophyte (ACO) dysphagia before and after surgery.
METHODS: Chart review including manofluorography (MFG) data of patients undergoing ACO removal was undertaken.

Osteophytes are bony outgrowths usually found in the context of osteoarthritis and diffuse idiopathic skeletal hyperostosis (DISH). Even if they are usually asymptomatic, they may present with complications such as spinal stenosis, myelopathy and radiculopathy. We examined the in vivo effect of benoxaprofen on the degeneration of articular cartilage after injection of sodium iodoacetate into the guinea pig knee.
