Polycystic ovary syndrome: chemical pharmacotherapy.

INTRODUCTION: Polycystic ovary syndrome (PCOS) is the most common reproductive endocrine disease among women of childbearing age. The clinical features are heterogeneous and vary in intensity. Hirsutism, menstrual disorders and infertility are the most frequent conditions observed; however, long-term complications (dyslipidemia, hypertension, cardiovascular disease, type 2 diabetes mellitus, endometrial cancer) are also often described. Each disorder may be managed by tailored strategies, employing sequential or combined pharmacological and/or non-pharmacological treatment. AREAS COVERED: The…

Metabolic Syndrome: Polycystic Ovary Syndrome.

Polycystic ovary syndrome (PCOS) is a heterogeneous condition characterized by androgen excess, ovulatory dysfunction, and polycystic ovaries. It is the most common endocrinopathy among women of reproductive age, affecting between 6.5% and 8% of women, and is the most common cause of infertility. Insulin resistance is almost always present in women with PCOS, regardless of weight, and they often develop diabetes and metabolic syndrome. The Rotterdam criteria are widely used for diagnosis. These criteria require that patients have at least two of the following conditions: hyperandrogenism, ovul…

[Natural history of ovary cancer].

Ovary cancer is a disease charged of paradigms and a serious health problem. It's important to know its natural history, because has a multifactor origins, and understanding its behavior since risk factors until patient's death because metastatic disease is a challenger for oncology group. In this work we made a bibliographic, analytic review that brings up concepts related to its origin, evolution, risk factors, preclinical horizon, and clinical symptoms until the death of patient.

Genetics of polycystic ovary syndrome.

The etiology of polycystic ovary syndrome (PCOS) has been difficult to determine because its features are heterogeneous, and its origin may also be heterogeneous. Twin studies suggest that its etiology is strongly heritable and genetic approaches are rapidly uncovering new regions of the genome that appear to confer risk for PCOS. Recent genome-wide association studies in Han Chinese women with PCOS demonstrate 11 genetic loci that are associated with PCOS. The variants identified are in regions that contain genes important for gonadotropin action, genes that are associated with risk for type …

Obesity and polycystic ovary syndrome.

Obesity is now a major international health concern. It is increasingly common in young women with reproductive, metabolic and psychological health impacts. Reproductive health impacts are often poorly appreciated and include polycystic ovary syndrome (PCOS), infertility and pregnancy complications. PCOS is the most common endocrine condition in women and is underpinned by hormonal disturbances including insulin resistance and hyperandrogenism. Obesity exacerbates hormonal and clinical features of PCOS and women with PCOS appear at higher risk of obesity, with multiple underlying mechanisms li…

Metabolomics in polycystic ovary syndrome.

The association of androgen excess with abdominal adiposity, insulin resistance and metabolic derangements characterize many patients with PCOS. However, the mechanisms underlying these associations are not entirely understood, indicating the need for discovery of the origin of these metabolic alterations, and of new metabolic biomarkers for PCOS. This review summarizes the metabolites and metabolic pathways associated with PCOS according to recent metabolomic studies. PCOS-associated metabolites were involved mostly in carbohydrate, fat, and protein metabolism. Obesity, hyperinsulinemia and t

Androgens and polycystic ovary syndrome.

Polycystic ovary syndrome (PCOS) is a mainly hyperandrogenic disorder and is possibly the most frequent endocrinopathy in premenopausal women. Androgen excess is the primary defect in PCOS, because ovarian theca cells secrete increased amounts of androgens even after several passes in primary culture. Excessive androgen amounts might favor the visceral deposition of body fat in affected women, resulting in insulin resistance, compensatory hyperinsulinism and further androgen excess. This vicious circle starts early during life in women with PCOS, even during fetal development, manifests clinic…

[Polycystic ovary syndrome--genetic considerations].

The Polycystic Ovarian Syndrome is one of the most frequent of the endocrinopathies, affecting in average 8% of the woman's that are in to reproductive age. It was first described by Chereau and Rokitanski, further associated with the amenorrhea encountered in the Stein and Levanthal syndrome, exposed by the Irving F, Stein and Michael Leventhal, in 1935. In many aspects this is a systemic disease also interfering with the process of reproduction. The studies of the genetic factor implication, by relating with the specialty literature, converging with a series of hypotesys launch over the year

[Polycystic ovary syndrome and comorbidity].

Polycystic ovary syndrome (PCOS) is the most frequent endocrinological disorder among women in the reproductive age. It is by its very nature a heterogeneous juxtaposition of clinical and biochemical features. In women with PCOS, the most common clinical manifestations - obesity, hirsutism and oligomenorrhoea - are associated with insulin resistance, dyslipidaemia, hypertension, vascular dysfunction involving chronic inflammation and also reduced cardiopulmonary function. The focus of this overview is therefore on PCOS comorbidity with the metabolic syndrome, type 2 diabetes and cardiovascular…

Polycystic ovary syndrome and autoimmunity.

Polycystic ovary syndrome (PCOS) is characterized by laboratory and/or clinical features consisting of hyperandrogenism with chronic anovulation and is currently one of the most common endocrinopathies in women of fertile age. PCOS is associated with a variety of endocrine and metabolic disturbances. It was demonstrated that the prevalence of autoimmune thyroiditis is high among these patients. Recent studies reveal a higher incidence of autoantibodies such as anti-histone, anti-dsDNA presented in systemic autoimmune disease, however their clinical significance is still unknown. According to r…

Androgens and polycystic ovary syndrome.

PURPOSE OF REVIEW: Polycystic ovary syndrome (PCOS) is a common complex endocrine genetic disorder, which involves overproduction of androgens, leading to heterogeneous range of symptoms and associated with increased metabolic and cardiovascular morbidity. This review focuses on androgen biosynthesis, use, metabolism in PCOS and clinical consequences of hyperandrogenism. RECENT FINDINGS: Controversial definition of the disorder and different phenotypic subgroups present a challenge for clinical and basic research. Further investigation of different phenotypes highlights the fact that PCOS pro…

Acarbose for polycystic ovary syndrome.

OBJECTIVE: To review the evidence for use of acarbose in the management of polycystic ovary syndrome (PCOS). DATA SOURCES: Relevant publications were identified through a systematic search of PubMed English-language literature (1950-February 2008) using the MeSH terms and key words acarbose and polycystic ovary syndrome. STUDY SELECTION AND DATA EXTRACTION: The literature search retrieved 6 primary literature citations. Three randomized controlled clinical trials and one open-label study were evaluated. The other 2 citations were not evaluated due to only a peripheral mention of PCOS in rela…

Diagnosis of polycystic ovary syndrome.

Polycystic ovary syndrome (PCOS) is a diagnosis of exclusion, with other androgen excess and ovulatory dysfunction disorders to be ruled out. There are 3 principal features of the syndrome, including hyperandrogenism, ovulatory dysfunction, and polycystic ovarian morphology. These features have been included in slightly different permutations in the 3 criteria currently available to diagnose PCOS, including that of the National Institutes of Health (NIH) 1990, Rotterdam 2003, and the Androgen Excess Society 2006. Overall, at least 1 in 15 women of reproductive age will be affected by PCOS, mak…

Laparoscopic detorsion of twisted ovary.

An 18-year-old female presented with a 6-day history of lower abdominal pain. Transabdominal ultrasonography revealed a right adnexal cyst measuring 9 cm. Laparoscopic examination demonstrated a 10-cm right paratubal cyst and a 5-cm right ovarian torsion with gangrenous discoloration. After removal of the right paratubal cyst, untwisting of the right ovary was performed. The postoperative period was uneventful. During follow-up, ultrasonography revealed restoration of right ovarian size with follicular growth. Arterial and venous blood flows were seen on Doppler examination. The right ovary ap…

Pelvic echinococcus mimicking multicystic ovary.

An unusual case of pelvic echinococcus cyst is presented, appearing initially on transvaginal ultrasound as a pelvic mass mimicking a multicystic ovary. A similar mass in the liver raised preoperatively the suspicion of echinococcosis, making an open surgical procedure preferable to laparoscopy. Diagnosis was confirmed pathologically after removal of the cyst. Though their location in the pelvis is rare, echinococcal cysts should be considered in the differential diagnosis of pelvic masses, especially in patients from endemic areas. Evaluation of previous medical history and current symptoms, …
