INTRODUCTION: Preterm infants are commonly treated by non-invasive ventilation (NIV) and nasal continuous positive airway pressure (CPAP), which are associated with fewer complications than nasotracheal intubation. BACKGROUND: A palatal fistula is a common complication of cleft palate repair. Although a buccal musculomucosal flap (BMMF) is effective for fistula repair, it does have the following problems: a second operation may be required to release the pedicle on the oral side and unilateral BMMF cannot close mucosal defects of both the nasal and oral sides. We describe a case of a palatal tic resembling palatal tremor (PT) in a young female patient with a previously unrecognized mild Tourette syndrome. At the time of her visit, the patient complained about ear clicks that were audible to others. We discuss the differential diagnoses of hyperkinetic palatal movements emphasizing the ongoing discussion about essential PT representing a more heterogeneous disorder than previously thought.

Palatal snoring produces explosive peaks of sound at very low frequency (approximately 20 Hz). Using a digital sound trace a ratio of peak amplitude to root mean square amplitude can be calculated. This Peak Factor Ratio is significantly higher for palatal snores than non-palatal snores (P < 0.01). This acoustic method will be useful for selecting patients for palatal surgery as it is non-invasive and could be used in a home monitor.
