OBJECTIVE: Because propofol is known to reduce vascular resistance, the objective of this study was to compare the indices of hepatosplanchnic circulation and oxygenation during cardiopulmonary bypass (CPB) in patients anesthetized with either propofol or midazolam/halothane.

Extreme premature triplets were scheduled for PDA ligation through median sternatomy. Anesthesia was maintained with fentanyl, ketamine, pancuronium and sevoflurane or isoflurane with N2O, oxygen and air. During operation, steady ventilation with low oxygen concentration was maintained. Monitoring size and contraction of hearts in the face, 5% albumine solution could be administered safely for hypotension after PDA ligation. PDA ligation through median sternatomy might be a less invasive surgical procedure.

A case of Lambert-Eaton myasthenic syndrome (LEMS) revealed by prolonged apnea following the administration of pancuronium during surgery for squamous cell carcinoma of the lung is reported. The postoperative respiratory failure allowed the correct interpretation of a poor clinical picture:slight weakness of the lower limbs and no other signs of neuromuscular disease. In the authors' opinion prolonged apnea always requires accurate neurophysiological investigation in order to detect latent neuromuscular transmission defects which are frequently underevaluated.

Total I.V. anesthesia was given to 20 patients using an Etomidate continuous infusion to maintain sleep, combined to Fentanyl analgesia, Droperidol, Pancuronium for muscular relaxation and artificial ventilation with an oxygen-air mixture. All these patients were carefully observed during and for several hours after the anesthesia and the results noted. With the Fentanyl dosages used in this technique, peroperative analgesia was frequently insufficient. More Fentanyl would probably be needed with the inherent dangers of prolonged postoperative depression.

A 30-yr-old patient with fulminant ulcerative colitis was anaesthetized on three occasions, for colectomy and rectal stump removal. The first anaesthesia, with droperidol and fentanyl and pancuronium neuromuscular block, was uneventful. Addition of suxamethonium on the second occasion and use of the original drugs plus nitrous oxide for the third anaesthesia produced symptoms of MH, which was confirmed by muscle biopsy. There was no hyperthermia. The patient's admitted anxiety before the last two operations is believed to have played a role in the genesis of MH.

A 57-year-old woman ingested 15 to 17 tablets of carbidopa-levodopa 10/100 tablets (carbidopa 150 mg and levodopa 1,500 mg) along with ibuprofen, carisoprodol, hydrocodone, and acetaminophen. The patient developed choreiform movements that persisted despite obtundation and attempts to extinguish them with naloxone, morphine, and diazepam. When the patient developed a rising level of creatine phosphokinase and myoglobinuria, she was treated with ventilatory support and pancuronium. She required paralysis for 60 hours, when her chorea resolved.

A 74-year-old male was operated for sigmoid colon cancer. Because of an agglutination of the patient's platelet, it was difficult to measure his platelet count under ethylene diamine tetra acetate (EDTA), heparin or citrate as anticoagulants with an automated cell counter. Even though there was a strong possibility of pseudothrombocytopenia, anesthetic management for the patient was safely conducted. His condition was stable throughout the perioperative course and no bleeding tendency was observed. Nitrous oxide, oxygen, sevoflurane, propofol and pancuronium were useful in this case.

In 10 premature infants (1123 +/- 263 g), fentanyl citrate (30 to 50 microgram/kg) was used in conjunction with pancuronium (0.1 mg/kg) as the sole anesthetic for transthoracic ligation of patent ductus arteriosus. Ventilation was controlled with air and oxygen in concentrations sufficient to maintain transcutaneous PO2 between 50 and 70 torr. Circulatory stability was easily maintained throughout the procedure. "Stiff chest" was avoided by the use of muscle relaxants, and the infants were awake within 1 hour after the procedure.

Two comparable groups of ten patients were studied. After nitrous oxide-oxygen fentanyl-pancuronium anesthesia, half the patients were reversed with a titrated dose of naloxone. Even in titrated doses naloxone rapidly abolished residual post-operative fentanyl analgesia in 80 p. 100 of the patients. In the control group none of the patients complained of pain for an average of six to eight hours. Blood gases in the recovery room were practically the same in reversed and non-reversed patients and were satisfactory.
