A radioimmunoassay for serum immunoreactive parathyroid hormone (iPTH), which has had widespread clinical use for five years, is described in detail. The iPTH results in large groups of patients are reported, and are discussed in relation to the specificity of the assay and in relation to other assays. Parathyroid hormone is concerned with urolithiasis, and regulated by serum ionized calcium concentration. We thought that parathyroid hormone might also be regulated by a hormone. 1 mg of ACTH injection was given intramuscularly to 6 patients with primary hyperparathyroidism, 6 patients with urolithiasis, and 5 control subjects. Reliable measurements of the concentration of parathyroid hormone (PTH) in serum or plasma are crucial for the effective clinical management of patients with chronic kidney disease (CKD). New PTH assays that increase the specificity of such measurements are now available and are widely utilized. The current review summarizes key technical developments in the evolution of PTH assays. We also discuss the diagnostic value of various methods for measuring PTH in serum or plasma for the assessment of patients with renal bone disease.

PURPOSE: To find out if the whole parathyroid hormone (wPTH) assay has practical advantages over the intact (iPTH) assay in patients with Graves' disease.
METHODS: We measured iPTH and wPTH levels before and after subtotal thyroidectomy in 111 consecutive patients (94 women and 17 men) with Graves' disease. Blood samples for assays were obtained after the induction of anesthesia (basal) and following skin closure (postoperative).
