Vulvar Lichen Sclerosus: Outcomes Important to Patients in Assessing Disease Severity.

OBJECTIVE: The aim of the study was to determine outcome measures that women with vulvar lichen sclerosus (LS) rate as important in assessing disease severity with the ultimate goal of including these items in a disease severity rating tool. METHODS: An online survey of women older than 18 years with a diagnosis of vulvar LS was performed. The survey was posted in Facebook LS support groups. Participants rated items on a scale from 1 to 5 (not important to include to essential to include) in a disease severity scale. Participants also rated how often they were affected by various symptoms on

Assessment of Quality of Life and Treatment Outcomes of Patients With Persistent Postchemotherapy Alopecia.

IMPORTANCE: Persistent alopecia occurs in a subset of patients undergoing chemotherapy, yet the quality of life (QOL) of these patients and their response to therapy have not been described in a large patient cohort. OBJECTIVE: To characterize the clinical presentation of patients with persistent chemotherapy-induced alopecia (pCIA) or endocrine therapy-induced alopecia after chemotherapy (EIAC) and their QOL and treatment outcomes. DESIGN, SETTING, AND PARTICIPANTS: A retrospective multicenter cohort of 192 women with cancer treated with cytotoxic agents who received a clinical diagnosis of

Emergency Department Volume and Outcomes for Patients After Chest Pain Assessment.

BACKGROUND: Chest pain is one of the most common reasons for emergency department (ED) visits in developed countries. Whether higher volume EDs have better outcomes, specifically for patients with chest pain, is unknown and pertinent. METHODS AND RESULTS: We conducted a study using population-based data on 498 291 patients ≥40 years old, presenting to ED in Ontario, Canada from 2008 to 2014, with chest pain and were discharged after assessment. We evaluated processes of care after discharge from ED. The primary outcome was a composite of all-cause death or hospitalization for acute coronary s

A Patient-Assessed Morbidity to Evaluate Outcome in Surgically Treated Vestibular Schwannomas.

OBJECTIVE: Outcome after treatment of vestibular schwannomas can be evaluated by health providers as mortality, recurrence, performance, and morbidity. Because mortality and recurrence are rare events, evaluation has to focus on performance and morbidity. The latter has mostly been reported by health providers. In the present study, we validate 2 new scales for patient-assessed performance and morbidity in comparison with different outcome tools, such as quality of life (QOL) (European Quality of Life-5 dimensions [EQ-5D]), facial nerve score, and work capacity. METHODS: There were 167 total

Preoperative Cardiac Risk Assessment and Surgical Outcomes of Patients with Gastric Cancer.

PURPOSE: To evaluate the incidence of major adverse cardiac events (MACE) and long-term surgical outcomes after gastrectomy for gastric cancer using the revised cardiac risk index (RCRI), which is based on preoperative insulin use, serum creatinine >2.0 mg/dL, and history of ischemic or congestive heart failure or cerebrovascular disease. METHODS: We allocated 1000 patients who underwent elective gastrectomy to three groups with ≥3 (group A, n = 32), 2 (group B, n = 142), or 1 (group C, n = 826) of these factors and compared surgical complications and prognoses. RESULTS: Groups A and B had o

Assessing depression outcome in patients with moderate dementia: sensitivity of the HoNOS65+ scale.

To date, there is no widely accepted clinical scale to monitor the evolution of depressive symptoms in demented patients. We assessed the sensitivity to treatment of a validated French version of the Health of the Nation Outcome Scale (HoNOS) 65+ compared to five routinely used scales. Thirty elderly inpatients with ICD-10 diagnosis of dementia and depression were evaluated at admission and discharge using paired t-test. Using the Brief Psychiatric Rating Scale (BPRS) "depressive mood" item as gold standard, a receiver operating characteristic curve (ROC) analysis assessed the validity of HoNO

The development and validation of a patient-reported questionnaire to assess outcomes of elbow surgery.

We developed a questionnaire to assess patient-reported outcome after surgery of the elbow from interviews with patients. Initially, 17 possible items with five response options were included. A prospective study of 104 patients (107 elbow operations) was carried out to analyse the underlying factor structure, dimensionality, internal and test-retest reliability, construct validity and responsiveness of the questionnaire items. This was compared with the Mayo Elbow performance score clinical scale, the Disabilities of the Arm, Shoulder and Hand questionnaire, and the Short-Form (SF-36) General

Lateral epicondylitis: in vivo assessment of arthroscopic debridement and correlation with patient outcomes.

BACKGROUND: Surgical management of lateral epicondylitis has traditionally consisted of an open incision with debridement of the affected extensor tendon tissue. More recently, arthroscopy has been reported as a surgical option for this condition. PURPOSE: To evaluate the effectiveness of arthroscopic debridement in excising the characteristic tendinopathy of chronic lateral epicondylitis and determine if residual tendinopathy correlated with poorer patient outcomes. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: Data were prospectively collected on all patients who underwent art

Evaluation of patient satisfaction and outcomes after assessment for acute ankle injuries.

Physicians argue that patient preferences influence their test ordering and their potential for compliance with clinical practice guidelines (CPG). This study was conducted to evaluate patient satisfaction with clinical practice in emergency department (ED) settings using a validated and widely publicized set of CPGs. Patients presenting to 4 hospital EDs were eligible if they had sustained acute ankle or foot injuries. All sites were involved with the dissemination of radiography CPG, and use of radiography was determined by treating physicians. Telephone follow-up was attempted for all patie

Assessment of Clinician- and Patient-Reported Outcomes After Mohs Micrographic Surgery.

BACKGROUND: Evaluating postoperative outcomes after Mohs micrographic surgery (MMS) necessitates the use of patient-reported outcome measures (PROMs) and clinician (± patient) scar assessment tools. OBJECTIVE: To review patient- and clinician-reported outcome tools that have been developed to evaluate postoperative outcomes that are relevant to MMS. METHODS: A literature search was conducted to identify patients and clinician-reported outcome tools developed for surgery on the head and neck and for skin cancer surgery (including MMS). RESULTS: Sixteen articles were included. Five PROMs (der

Assessment of patient-centered outcomes in young children with differences of sex development.

Patient-reported outcome measures (PROMs) are key to evaluating the real-life impact of care, particularly for individuals with differences/disorders of sex development (DSD). DSDs are congenital and life-long conditions, most often ascertained in infancy or adolescence, and are characterized by atypical chromosomal, gonadal, or phenotypic sex. PROMs capture vital aspects of health-related quality of life (HRQoL) and psychosocial well-being from the perspective of patients and caregivers, complementing traditional clinical outcomes. Despite their importance, there is a paucity of validated, DS

Radiologically Assessed Stapes Footplate Thickness and Audiologic Outcomes in Patients with Otosclerosis.

UNLABELLED: <p>Introduction: Otosclerosis is a known cause of particularly conductive hearing loss of a variable extent. Radiological examination reveals footplate thickening in addition to heterogeneously distributed hypodense foci. The objective of this study was to investigate the correlation between a thickened stapes footplate and its association with perioperative audiometric findings and postoperative complications. METHODS: This was a retrospective data analysis conducted at a single tertiary referral center on 63 surgically confirmed otosclerotic ears from 56 patients. Stapes footpla

Assessment of epidemiology and outcomes of adult patients with kidney-limited thrombotic microangiopathies.

Thrombotic microangiopathies (TMA) are usually associated with hematological features (RH-TMA). The epidemiology of TMA limited to kidneys (RL-TMA) is unclear Therefore, patients with TMA and native kidney biopsies were identified during 2009-2022 in 20 French hospitals and results evaluated. RL-TMA was present in 341/757 (45%) patients and associated with lower creatinine levels (median 184 vs 346 μmol/L) than RH-TMA. RL-TMA resulted from virtually all identified causes, more frequently from anti-VEGF treatment and hematological malignancies but less frequently from shigatoxin-associated hemo

A Tool for Rapid Assessment of Functional Outcomes in Patients with Head and Neck Cancer.

Head and neck cancer (HNC) and its treatment can lead to various functional impairments. We developed and validated an instrument for rapid physician-rated assessment of basic functional outcomes in HNC patients. HNC-relevant functional domains were identified through a literature review and assigned to verbal ratings based on observable criteria. The instrument draft was subjected to systematic expert review to assess its face and content validity. Finally, the empirical validity, reliability, and responsiveness of the expert-adapted Functional Integrity in Head and Neck Cancer (HNC-FIT) scal
