Trabeculectomy combined with phacoemulsification and intraocular lens implantation.

It has recently become popular to use mitomycin C (MMC) with trabeculectomy. MMC helps to maintain effective filtration and long-term intraocular pressure (IOP) control, while the use of laser suture lysis has improved the safety of this procedure. We evaluated the outcome of trabeculectomy alone and of trabeculectomy plus phacoemulsification (PEA) and intraocular lens (IOL) implantation, each supplemented with MMC. The mean preoperative IOP for the trabeculectomy alone group was 22.4 +/- 3.2 mmHg(bleb+), 25.3 +/- 2.6 mmHg(bleb-), and that of the combined surgery group was 22.4 +/- 2.8 mmHg(bl

[New modified method of phacoemulsification in lenticular subluxation].

A new method for cataract removal by phacoemulsification in impaired lenticular joints consists in fixation during surgery of the anterior lenticular chamber along the edge of the capsulorrhexis by 4 retractor hooks and subsequent implantation of a soft intraocular lens. The operation was performed in 6 patients with cataracts of different types complicated by subluxation of the lens. The operation did not involve such complications as vitreous prolapse or displacement of the incompletely dislocated lens. All operations were finished with complete evacuation of lenticular mass and implantation

[Combination of phacoemulsification and trabeculectomy. Results of a retrospective study].

Combined trabeculectomy and phacoemulsification was performed on 21 eyes with poorly controlled glaucoma (15 open-angle glaucomas, 5 capsular glaucomas, and one secondary glaucoma) and coexisting cataract. In 20 of these eyes there was severe glaucomatous damage of the optic nerve with visual field loss. With the procedure used by the present authors there was a higher incidence of choroidal detachments, whereas other complications occurred at a lower rate than reported by other authors combining cataract extraction and trabeculectomy. Mean IOP was lowered from 25.7 +/- 6.1 to 17.1 +/- 3.2 mmH

Endothelial damage with phacoemulsification and intraocular lens implantation.

Specular microscopy was used to study the corneal endothelium in a series of patients undergoing phacoemulsification with and without intraocular lens implantation by one experienced surgeon. Cell loss after phacoemulsification alone was variable, but the mean was 18%, with a median of 11%. The patients undergoing phacoemulsification plus lens implantation showed a mean cell loss of 26%, with a median of 17%. This is considerably lower than has been reported with standard intracapsular extraction plus lens implantation. The decreased endothelial damage is attributed to the technique of inserti

Repeat Descemetopexy after Descemet's Membrane Detachment following Phacoemulsification.

Descemet's membrane detachment (DMD) is an uncommon condition with a wide range of possible etiologies. Probably the commonest cause is a localized detachment occurring after cataract extraction surgery. Descemetopexy gives good anatomic attachment rates and visual outcomes and has become the standard treatment for DMD. However, in cases with failed initial descemetopexy, the next step in the management of such cases remains unclear. Before initiating a complex surgical procedure like keratoplasty, which requires good postoperative care and regular follow-ups, repeat descemetopexy with a long-

Hydrodissection-free phacoemulsification surgery: mechanical cortical cleaving dissection.

UNLABELLED: We describe hydrodissection-free phacoemulsification surgery (hydro-free phaco), which eliminates conventional cortical-cleaving hydrodissection during cataract surgery. To replace hydrodissection, we devised a mechanical cortical-cleaving dissection technique that is performed during phacoemulsification. The new technique prevents the high pressure within the endocapsular space or the posterior chamber-anterior hyaloid membrane barrier area that is common with hydrodissection. This reduces hydrodissection-related complications such as posterior capsule rupture, anterior hyaloid me
