Phantom limb pain and etiology of amputation in unilateral lower extremity amputees.

The relationships between phantom limb pain and both the etiology of amputation (blood clots, nonclot diabetes, and miscellaneous) and the occurrence of gangrene and/or infection were investigated in 92 unilateral lower extremity (LE) amputees who were seen sequentially. There were 55 above-knee (AK) and 37 below-knee (BK) amputations in 61 men and 31 women. The mean age was 51.4 years. The blood-clot etiology had the highest levels of phantom pain both pre- and postrehabilitation, the longest time interval between amputation and prosthetic fitting, and the greatest number of medical condition…

Psychological factors influencing chronic phantom limb pain: an analysis of the literature.

The behavioral literature concerning chronic phantom limb pain was reanalyzed in order to determine the role of psychological factors in initiating and controlling the intensity of its episodes. Some of the behavioral literature presents an inaccurate picture of amputees who have phantom pain. This apparently happened because many of the data were gathered from those amputees requesting treatment for phantom pain who were referred to mental health professionals. We conclude that phantom pain is similar to other chronic pain syndromes in that episodes are greatly influenced by psychological fac…

Experiences of individuals who underwent Phantom Motor Imagery treatment to decrease phantom limb pain and the consequences on their daily functioning.

PURPOSE: Phantom Limb Pain (PLP) frequently occurs after limb amputation and often negatively influences daily functioning. Phantom Motor Imagery (PMI) treatment intends to decrease PLP by imagining phantom movements guided by extended reality techniques. The aim of this study was to investigate the experiences of individuals with PLP after limb amputation who underwent PMI treatment, and to explore the consequences of PMI treatment on daily functioning. MATERIALS AND METHODS: A qualitative study was performed, consisting of semi-structured interviews one month after PMI treatment among eight…

Modulations in high-density EEG during the suppression of phantom-limb pain with neurostimulation in upper limb amputees.

Phantom limb pain (PLP) is a distressing and persistent sensation that occurs after the amputation of a limb. While medication-based treatments have limitations and adverse effects, neurostimulation is a promising alternative approach whose mechanism of action needs research, including electroencephalographic (EEG) recordings for the assessment of cortical manifestation of PLP relieving effects. Here we collected and analyzed high-density EEG data in 3 patients (P01, P02, and P03). Peripheral nerve stimulation suppressed PLP in P01 but was ineffective in P02. In contrast, transcutaneous electr…

Preemptive Treatment of Phantom and Residual Limb Pain with Targeted Muscle Reinnervation at the Time of Major Limb Amputation.

BACKGROUND: A majority of the nearly 2 million Americans living with limb loss suffer from chronic pain in the form of neuroma-related residual limb and phantom limb pain (PLP). Targeted muscle reinnervation (TMR) surgically transfers amputated nerves to nearby motor nerves for prevention of neuroma. The objective of this study was to determine whether TMR at the time of major limb amputation decreases the incidence and severity of PLP and residual limb pain. STUDY DESIGN: A multi-institutional cohort study was conducted between 2012 and 2018. Fifty-one patients undergoing major limb amputati…

Auricular Therapy for Treating Phantom Limb Pain Accompanied by Jumping Residual Limb: A Short Review and Case Study.

Phantom limb pain (PLP) is a common complaint among patients after amputation, while jumping residual limb is a rare post-amputation complication, they rarely happen at the same time and both remain difficult to manage. At present, there is a paucity of literature on this topic, and no treatment has been proven effective for treating both of them. In the present brief report, we described a patient who developed severe PLP accompanied by jumping residual limb after below-the-knee amputation and she was treated by auricular therapy (AT) with satisfactory effect.

Tactile, thermal, and electrical thresholds in patients with and without phantom limb pain after traumatic lower limb amputation.

PURPOSE: To examine whether there is central sensitization in patients with phantom limb pain (PLP) after traumatic limb amputation. METHODS: Seventeen patients after unilateral lower limb amputation secondary to trauma were enrolled. Ten patients had chronic PLP, while the other seven patients had no PLP. Tactile-sensation threshold, cold- and warm-sensation thresholds, cold- and heat-pain thresholds, electrical-sensation threshold (EST), and electrical-pain threshold on the distal residual limb and the symmetrical site on the sound limb were measured in all tested patients. Their thresholds…

Identifying discriminant factors between phantom limb pain, residual limb pain, and both in people with lower limb amputations: a cross-sectional study.

Postamputation pain is a common condition in patients with lower limb amputation (LLA), which compromises amputees' rehabilitation, use of the prosthesis, and quality of life. The aim of our study was to investigate the prevalence of phantom limb pain (PLP), residual limb pain (RLP), or both types of pain among individuals with LLA, and to identify the factors associated with the presence of one type of pain versus the other. Patients who underwent amputation for traumatic or vascular reasons and who reported on RLP or PLP were analyzed and divided into three groups: PLP, RLP, or a group of su…

Supernumerary phantom limb appearing in association with motor intention or pain in the paralyzed upper limb: a case report.

We describe a case of supernumerary phantom limb (SPL) persisting into the chronic phase of a right putaminal hemorrhage. The individual, a forced right-handed female in her 40's, was admitted to the hospital 9 months after onset with clear consciousness, well-preserved cognitive function, severe left hemiparesis, and deep sensory impairment. When attempting to move the paralyzed upper limb, she perceived an SPL and felt as if it assisted the motion. Subsequently, the perceived SPL became associated with pain in the paralyzed upper limb. She reported that the SPL wrapped and tightened around h…

Exploring phantom phenomena following brachial plexus block in intact limbs.

BACKGROUND AND OBJECTIVE: Phantom limb experiences, including phantom limb sensations (PLS) and phantom limb pain (PLP), are common after limb amputation or deafferentation, with PLP significantly impacting quality of life. However, the mechanisms underlying PLP remain unclear, complicating treatment development. Investigating phantom phenomena has been proposed to gain insights into the mechanisms behind the insurgence of PLS and PLP, potentially informing new therapeutic approaches. However, small, heterogeneous samples and a lack of objective pain metrics often limit research on individuals…

Mirror Therapy for the Management of Phantom Limb Pain: A Single- Center Experience.

BACKGROUND: Phantom limb pain (PLP) can have devastating consequences, affecting up to 90% of amputees. PLP is associated with analgesia dependence and impaired quality of life. Mirror therapy (MT) is a novel treatment that has been applied in other pain syndromes. We prospectively evaluated MT in the management of PLP. METHODS: A prospective study of patients recruited between 2008 and 2020 who underwent unilateral major limb amputation, with a healthy contralateral limb. Participants were invited to attend weekly MT sessions. Pain in the 7 days prior to each MT session was scored on a Visua…

A Survey of Frozen Phantom Limb Experiences: Are Experiences Compatible With Current Theories.

There are over two million individuals living with amputations in the United States. Almost all will experience the feeling of the amputated limb as still present, termed phantom limb sensation (PLS). Over 85% will also experience excruciatingly painful sensations known as phantom limb pain (PLP). Additionally some amputees also experience a sensation of the phantom limb in which the limb is immobile or stuck in a normal or abnormal anatomical position, termed frozen phantom sensations. When an amputee experiences a frozen limb they report that they are unable to move the limb, and sometimes r…

Gamma oscillations in the somatosensory thalamus of a patient with a phantom limb: case report.

The amputation of an extremity is commonly followed by phantom sensations that are perceived to originate from the missing limb. The mechanism underlying the generation of these sensations is still not clear although the development of abnormal oscillatory bursting in thalamic neurons may be involved. The theory of thalamocortical dysrhythmia implicates gamma oscillations in phantom pathophysiology although this rhythm has not been previously observed in the phantom limb thalamus. In this study, the authors report the novel observation of widespread 38-Hz gamma oscillatory activity in spike an…

The deafferentation syndrome in genetically blind rats: a model of the painful phantom limb.

The hypothesis was tested which states that the somatic deafferentation syndrome is a visually prompted response to sensorimotor loss. The d … ses from an existing central representation of the limb and its relationship with the total body; a phantom limb. Consideration of other reports regarding the deafferentation syndrome leads to the view that it is motivated by disturbing abnormal sensations (pain) of central neural origin.

Spinal Cord Stimulation for the Treatment of Phantom Limb Pain: A Narrative Review.

BACKGROUND: The annual number of amputations is estimated to be between 200 and 500 million worldwide. The different etiologies of these amputations can be sorted into the following categories: traumatic, vascular, infectious, and tumoral. Since the twentieth century, progress has been made in stump management, surgical techniques to treat amputation, and rehabilitation, with improvements made in the prostheses designed for amputees. Among amputees, 10-79% will suffer from stump pain and phantom limb pain (PLP). Several studies have highlighted the various neuromodulation techniques available …

Phantom limb pain in daily practice--still a lot of work to do!

OBJECTIVES: Effective treatment of phantom limb pain (PLP, pain felt in the part of the body of an amputated limb) is still difficult to achieve, and improved treatment is needed. It is therefore of paramount interest to understand the current practice of PLP therapy outside pain centers. DESIGN: As a part of a nationwide survey, 537 amputees were asked 11 questions related to their treatment experiences and the pain relief. Furthermore, the patients' opinion about the quality of medical care was also asked. RESULTS: Five hundred thirty-seven out of 1088 amputees returned the questionnaire (…
