[Image analysis of the contour of paralyzed vocal cord during phonation].

The development of digital radiography (DR) has made it possible to analyze the contour of the laryngeal soft tissue structures in more detail than the conventional screen-film method. We analyzed the contour of the paralyzed vocal cord during phonation using motion subtraction technique. Forty four patients with unilateral recurrent nerve palsy were examined. The images were obtained by means of frontal tomography of the larynx during quiet inspiration and normal (expiratory) phonation. Tomography during inspiratory phonation was also examined in ten patients who can perform it. The thickness…

CT in carcinoma of the larynx and pyriform sinus: value of phonation scans.

The structural and functional information obtained from CT performed during quiet breathing and phonation of the letter E was investigated in 25 patients with carcinoma of the larynx or pyriform sinus. Significant additional information was obtained from the phonation scans in all patients. In 16 patients, vocal cord dysfunction was found on the phonation scans. In 14 patients, phonation CT demonstrated an abnormal aryepiglottic fold better than CT during quiet breathing. A pyriform sinus was distorted or displaced in 11 patients and CT during phonation was more accurate than laryngoscopy or C…

Perception of vocal tremor during sustained phonation compared with sentence context.

OBJECTIVES/HYPOTHESIS: Vocal tremor is an acoustical phenomenon characterized by relatively periodic modulations in fundamental frequency and intensity. Although vocal tremor is considered easier to perceive during sustained phonation than during connected speech, systematic comparison between these speech contexts has not been investigated. This investigation compared vocal tremor perception during sustained phonation and connected speech contexts. STUDY DESIGN: This is a prospective, controlled study with randomized conditions. METHODS: Audio recordings from five speakers diagnosed with es…

Can maximum phonation time predict voice outcome after thyroplasty type I?

BACKGROUND: Thyroplasty type I, as introduced by Isshiki and colleagues almost 30 years ago, has become the gold standard of improving glottal incompetence caused by unilateral vocal fold paralysis. Intraoperative assessment of the adequacy of glottal closure is subjective and based on the perceptual judgment of vocal quality and degree of improvement in glottal gap size. OBJECTIVE/HYPOTHESES: The primary purpose of this study was to investigate whether the intraoperative measurement of maximum phonation time (MPT) is an adequate predictor of voice outcome after thyroplasty type I. To assess …

Acoustic Features of Sustained Phonation for Schizophrenia Classification: A Feasibility Study.

Voice and speech are increasingly studied as indicators of mental health, but the acoustic features of sustained phonation in schizophrenia remain underexplored. This feasibility study examined whether a 500 ms sustained vowel /a/ contains meaningful information for distinguishing patients with schizophrenia from healthy controls. Recordings from 84 participants (41 patients, 43 controls) were analyzed using features from eight acoustic domains. Random Forest, XGBoost, and logistic regression were evaluated under four train-test configurations with data augmentation and participant-grouped cro…

Computational analysis of asymmetric thyroarytenoid and cricothyroid activation in human phonation.

Asymmetric activation of intrinsic laryngeal muscles is common in human phonation, yet its effects on vocal fold (VF) vibration and acoustic output remain unclear. In this study, we used a high-fidelity, anatomically resolved 3D computational framework to examine how asymmetric thyroarytenoid (TA) and cricothyroid (CT) activation regulates VF strain, vibratory dynamics, and flow-related acoustics. The results show that TA activation is the primary source of left-right mechanical imbalance. In contrast, CT asymmetry produces limited vibratory asymmetry due to the bilateral anatomical constraint…

Stuttering: the nature of the speech disruptions-a multimodal study of articulation and phonation.

Stuttering is a disorder characterized by transient disruptions in speech motor production. This article is focused on the characteristics of stuttering and the immediate vocal tract mechanisms resulting in stuttered speech disruptions. A range of observations from an initial series of studies on the motor characteristics of stuttering events in adults is presented, combined with a narrative review of published data regarding objective and subjective aspects of instances of stuttering. The aims of the empirical studies were to develop methods for data collection and analysis, as well as collec

Influence of Reduced Saliva Production on Phonation in Patients With Ectodermal Dysplasia.

OBJECTIVE: Patients with ectodermal dysplasia (ED) suffer from an inherited disorder in the development of the ectodermal structures. Besides the main symptoms, i.e. significantly reduced formation/expression of teeth, hair and sweat glands, a decreased saliva production is objectively accounted. In addition to difficulties with chewing/swallowing, ED patients frequently report on the subjective impression of rough and hoarse voices. A correlation between the reduced production of saliva and an affliction of the voice has not yet been investigated objectively for this rare disease. METHODS: F

Hybrid aeroacoustic approach for the efficient numerical simulation of human phonation.

A hybrid aeroacoustic approach was developed for the efficient numerical computation of human phonation. In the first step, an incompressible flow simulation on a three-dimensional (3 D) computational grid, which is capable of resolving all relevant turbulent scales, is performed using STARCCM+ and finite volume method. In the second step, the acoustic source terms on the flow grid are computed and a conservative interpolation to the acoustic grid is performed. Finally, the perturbed convective wave equation is solved to obtain the acoustic field in 3 D with the finite element solver CFS++. Th…

In Vivo Quantification of the Intraglottal Pressure: Modal Phonation and Voice Onset.

Intraglottal pressure is the driving force of vocal fold vibration. Its time course during the open phase of the vibratory cycle is essential in the mechanics of phonation, but measuring it directly is difficult and may hinder spontaneous voicing. However, it can be computed from the in vivo measured transglottal flow and glottal area (hence the air particle velocity) on the basis of the Bernoulli energy law and the interaction with the inertance of the vocal tract. As to sustained modal phonation, calculations are presented for the two possible shapes of glottal duct: convergent and divergent…

Vocal fold nodules: A disorder of phonation organs or auditory feedback?

OBJECTIVE: Increasing evidence supports that auditory feedback of one's own voice closely relates to real-time adjustments of vocal control. Previous studies highlighted that the low-frequency modulations of below 3 Hz (LFM) embedded in vocal fundamental frequency (F0) showed a reflex-like response to altered auditory inputs. However, the auditory feedback control of different vocal disorders remains unclear. DESIGN: A cross-sectional, case-controlled study. SETTING: A tertiary medical centre. PARTICIPANTS: Sustained vocalisations of vowel/a/ from adult healthy controls and patients with vo

[Quantitative study of phonation by aerophonoscopy: Reproducibility study on healthy volunteers].

INTRODUCTION: The aerophonoscope allows for recording buccal and nasal airflow during breathing and speech and the sounds emitted by the patient. It is known to be useful in the postoperative follow-up of cleft lip and palate children, but there are currently no studies that quantitatively validate its reliability in pathological or non-pathological situations. The aim of our study was to measure the reliability of aerophonoscopic measures in adult healthy volunteers. MATERIAL AND METHODS: A quantitative evaluation of the reliability of aerophonoscopy has been carried out in 30 healthy adult
