BACKGROUND: Photochemotherapy is a safe and effective treatment for patients with drug-resistant severe GvHD. The technique involves the exposure of MNC to psoralen and UVA light (PUVA). We have investigated the effect of in vitro PUVA on MNC adhesion to extracellular matrix (ECM) proteins.
METHODS: MNC were isolated from peripheral blood (PB) and umbilical cord blood (UCB), and treated with PUVA. After labeling by a chemiluminescent probe, MNC were plated on ECM proteins (collagen, fibronectin, vitronectin and laminin) and the number of adherent cells was measured.

The vitamin D3 analogue calcipotriol (calcipotriene) is an effective topical treatment for psoriasis. In combination with other antipsoriatic agents, such as ultraviolet radiation, calcipotriol is reported to improve the overall efficacy of the treatment. Here we describe two patients treated with a combination treatment of calcipotriol and bath psoralens and ultraviolet A who developed hyperpigmentation at the lesional sites where calcipotriol ointment was applied.

In the Skin Clinic of Neurnberg about 1,500 patients have been treated since 1972 with blacklight and 8-methoxypsoralen, approximately 120-150 every day. This new therapy has proved very good in psoriasis, acne, bacterial disseminated eczema, parapsoriasis and mycosis fungoides. The technique and the rate of relapses, the side effects and their endogenous as well as exogenous causes will be discussed.
