The objective of this national audit was to examine the use of platelet transfusions against audit standards developed from national guidelines. With data on 91 alloimmunized thrombocytopenic patients and 389 donor-recipient pairs matched or selectively mismatched for HLA antigens, it was observed that ABO incompatibility significantly reduced the effectiveness of platelet transfusions. Visceral leishmaniasis (VL; kala-azar), a life-threatening infection of the mononuclear phagocytic system, is transmitted by the bite of infected sand flies. Though peripheral parasitaemia is documented for Leishmania spp. causing VL, reports of transfusion-transmitted infections are rare. A case of probable platelet transfusion-acquired VL is reported from India and issues related to transfusion safety in endemic areas are discussed.

OBJECTIVES: To provide the lowest effective dose and minimize toxicity, our transfusion service initiated the use of a half-dose for all prophylactic inpatient platelet transfusions. The main rationale for this change in dose was the lack of evidence of increased bleeding and mortality in lower-dose platelet transfusion. Since the current platelet dose is not evidence based, and dose and toxicity are linked for most therapeutics, this approach to reducing adverse effects made scientific and clinical sense. The occurrence of post-transfusion purpura (PTP) in a 16-year-old girl with sickle/beta-thalassaemia is described. Clinically this was a typical case of PTP, but it was unusual serologically. Anti-Baka and anti-PIA2 platelet-specific antibodies were identified and the patient's platelets were typed as homozygous PIA1-positive and Baka-negative. The patient also developed red-cell, granulocyte and lymphocytotoxic antibodies in response to the blood transfusion and had a delayed haemolytic transfusion reaction.
