BACKGROUND: Necrotizing fasciitis caused by Klebsiella pneumoniae is rare, with 11 documented cases in the literature, all occurring in Asia and the Middle East. These reports have become more frequent in recent years in association with the emergence of the highly virulent K1 capsular serotype of Klebsiella. We report a case of Klebsiella necrotizing fasciitis and con-current septic arthritis in the Western hemisphere.
METHODS: Case report and review of the literature.
RESULTS: Our patient, a 50-year-old Cambodian man with advanced hepatitis B-associated cirrhosis, who had last visited an e

INTRODUCTION: Cavitary necrosis remains a rare complication of bacterial pneumonia in children. Conservative medical treatment and radical surgical treatment with lung resection are the current therapeutic choices. Evaluation of thoracoscopy for this pathology has not yet been reported. We describe 3 cases.
MATERIALS AND METHODS: Between January 2001 and January 2002, 3 children (1, 2, and 3 years old) were admitted to our institution with necrotizing pneumonia. The diagnosis was based on injected computed tomography (CT) scan showing pulmonary condensation, intra-parenchymal bullae, and hypo

Abstract A 42-year-old man was hospitalized because of chills, fever, and severe polyarthralgia. He had a 5-year history of mixed connective tissue disease (MCTD) with polyarthritis which had been treated with a nonsteroidal anti-inflammatory drug alone. On the second day of hospitalization, necrotizing fasciitis of the right leg developed. Streptococcus pneumoniae was later detected in a blood culture. Pneumococcal infections of the soft tissues are uncommon, and have been reported in immunosuppressed patients and patients with systemic lupus erythematosus. This is the first report of a case

BACKGROUND: Some studies have observed an increased incidence of necrotizing pneumonia (NP) in recent years. This might be related to the emergence of non-vaccine S. pneumoniae serotypes after PCV7 introduction although it is suggested that evolutionary factors may have modified the virulence and the interactions of pneumococci. The aim of this study was to clinically and microbiologically define NP in the population served by the three major paediatric hospitals in Barcelona (Catalonia, Spain).
METHODS: A prospective observational study was conducted in patients <18 years hospitalized due to

Mycoplasma pneumoniae (MP) is a bacterium commonly known to cause mild respiratory infections, especially in young children. Epstein-Barr virus (EBV) is a herpesvirus that causes infectious mononucleosis, typically a mild illness in younger individuals. However, in its severe form, EBV can cause pneumonia. Severe pneumonia resulting from co-infection with MP and EBV is rare, with few cases reported in the literature, mostly in the pediatric population. We report a case of a 20-year-old man who presented with a one-week history of progressively worsening right-sided pleuritic chest pain, subjec

BACKGROUND: Necrotizing pneumonia (NP) is a severe but relatively uncommon complication of pediatric community-acquired pneumonia. In recent years, increasing clinical attention has been directed toward pediatric NP. This study aimed to characterize the clinical features, chest CT findings, and serum inflammatory markers of pediatric NP, and to examine pathogen-associated differences in imaging patterns and inflammatory profiles.
MATERIALS AND METHODS: We retrospectively reviewed 54 children diagnosed with NP between January 2018 and January 2024.Etiologic agents, chest CT findings, and serum

OBJECTIVE: This study aims to enhance understanding of necrotizing pneumonia and toxic shock syndrome by analyzing an adult case of community-acquired necrotizing pneumonia caused by co-infection of Influenza A (H1N1) and Staphylococcus aureus with LukS-PV and LukF-PV virulence factor genes.
METHOD: The clinical data of one patient admitted to the intensive care unit (ICU) with co-infection of Influenza A (H1N1) and Staphylococcus aureus was retrospectively analyzed.
RESULTS: The patient exhibited typical clinical manifestations of viral and Staphylococcus aureus co-infection, including necr

Necrotizing pneumonia (NP) represents an uncommon but severe complication of community-acquired pneumonia (CAP) in children, characterized by the destruction of lung parenchyma and formation of multiple thin-walled cavities. Despite its rarity, the incidence of pediatric NP has increased substantially over the past two decades, creating major diagnostic and treatment challenges. The COVID-19 pandemic has further altered the epidemiological landscape, with unique patterns of respiratory pathogen circulation. This narrative review synthesizes current evidence on pediatric necrotizing pneumonia t

Saprochaete capitata is a yeast-like fungus of the Dipodascaceae family, capable of colonizing the skin and the respiratory and gastrointestinal tracts. We present a 56-year-old man with diabetes mellitus who was admitted to the hospital presenting with fever, cough and hemoptysis. The diagnosis of necrotizing pneumonia was made by direct microscopy of the bronchoalveolar lavage fluid showed and Saprochaete capitata was identified by Matrix-Assisted Laser Desorption/Ionization Time-Of-Flight (MALDI-TOF MS®). Treatment consisted of itraconazole 200 mg every 12 hours orally for 30 days, leading

A 58-year-old man with a history of diabetes mellitus and end-stage renal disease acquired pneumonia with acute respiratory failure during his stay in an intensive care unit (ICU). Empirical antimicrobial therapy with ceftazidime and vancomycin was initiated, and imipenem replaced ceftazidime 2 days later due to the patients pulmonary condition failed to improve. However, within 5 days, pulmonary consolidation rapidly progressed to necrotizing pneumonia complicated by lung abscess, empyema, pyopneumothorax, and tension pneumothorax, leading to the patient's death. After the patient had died, a

BACKGROUND: Mycobacterium lentiflavum (M. lentiflavum), a slow growing nontuberculous mycobacterium (NTM), has recently been described as an emerging human pathogen regardless of the immune status of the host. Previous reports have demonstrated that cervical lymphadenitis of children is the most frequent pathology of M. lentiflavum. However, there are little reports regarding pulmonary diseases by M. lentiflavum specifically in immunocompetent patients.
CASE PRESENTATION: A 60-year-old man having prolonged productive cough and dyspnea with fever was initially diagnosed as pneumonia with parap

A chest X-ray of a young healthy African-American man with acute respiratory failure revealed bilateral multiple nodular shadows in the lungs, while community-acquired methicillin-resistant Staphylococcus aureus (CA-MRSA) was detected in blood and sputum cultures. Magnetic resonance imaging showed osteomyelitis of the left thigh, and computed tomography revealed bilateral cavitary lesions in the chest, indicating necrotizing pneumonia with pulmonary embolism caused by osteomyelitis as a result of infection with CA-MRSA. CA-MRSA should be suspected as a causative agent of severe community-acqui

Pulmonary alveolar proteinosis (PAP), which was first described by Rosen in 1958, is a rare disease characterized by impaired surfactant metabolism that provokes the accumulation of proteinaceous material in the alveoli. PAP is usually an auto-immune disease though, less commonly, may be congenital or secondary to another underlying disorder, such as infection, an immunodeficiency or a haematological disease. A positive diagnosis can be made with the appearance of "crazy-paving" on a computed tomography scan, with a milky fluid bronchial aspiration. A cytological examination will also show eos

Tularemia is an unusual disease caused by the gram-negative coccobacillus Francisella tularensis. The clinical features of the disease depend on the route of inoculation. Ulceroglandular and typhoidal forms have been recognized as occurring in tularemia, however primary or secondary pneumonic infections have also been reported. Symptoms, laboratory markers and radiological features are non-specific in tularemic pneumonia. Diagnosis is made on clinical grounds and evidence of elevated agglutinating antibodies to F. tularensis (> 1:128). We report a case of primary tularemic pneumonia presenting

A rare case of a severe anaerobic necrotizing pneumonia complicated by pyopneumothorax and monoarthritis due to anaerobic Peptostreptococcus magnus is described in a 20-year-old soldier. The patient's immunological status was normal. There were no predisposing underlying factors, and he was treated successfully without any residual damage. To our knowledge, a similar case has never been reported in the literature.

BACKGROUND: Necrotizing pneumonia is a life-threatening complication of community-acquired pneumonia managed in an intensive care setting with a multitude of diagnostic modalities and numerous therapeutic interventions. Pseudomonas aeruginosa remains an extremely uncommon organism known to cause necrotizing pneumonia, and of the cases reported, are almost entirely found in immunocompromised hosts. CASE DETAILS: Here, we report a case of Pseudomonas aeruginosa community-acquired necrotizing pneumonia in a host receiving adalimumab therapy that ultimately was not able to be healed despite target
