Health system readiness-the neglected catalyst in Malaria elimination.

BACKGROUND: While strong political commitment and robust technical strategies are essential to drive malaria elimination, the readiness of national health systems ultimately determines whether these commitments can be successfully translated into action. MAIN TEXT: Without adequate preparation, gaps in the health system can undermine the transition from malaria burden reduction to elimination policies, reduce the effectiveness of interventions, and jeopardise progress toward achieving zero local transmission of malaria. Despite its critical importance, there is currently no systematic and com

Substance abuse treatment organizations as mediators of social policy: slowing the adoption of a congressionally approved medication.

Most substance abuse treatment occurs in outpatient treatment centers, necessitating an understanding of what motivates organizations to adopt new treatment modalities. Tichy's framework of organizations as being comprised of three intertwined internal systems (technical, cultural, and political) was used to explain treatment organizations' slow adoption of buprenorphine, a new medication for opiate dependence. Primary data were collected from substance abuse treatment organizations in four of the ten metropolitan areas with the largest number of heroin users. Only about one fifth offered bupr

"To do so in a patient-centred way is not particularly lucrative": The effects of neoliberal health care on PrEP implementation and delivery.

BACKGROUND: HIV pre-exposure prophylaxis (PrEP) is a highly effective biomedical intervention used by HIV-negative people to prevent HIV acquisition. Despite increased use of PrEP worldwide, several barriers to PrEP implementation have resulted in insufficient uptake, inadequate adherence, and frequent discontinuation. Our objective was to interrogate the social, political, and economic conditions shaping PrEP implementation and delivery among gay, bisexual, queer and other men who have sex with men (GBQM) in Ontario, Canada. METHODS: Six focus groups and three interviews with 20 stakeholders

Beyond procedural justice: Power, identity and the asymmetric architecture of public trust in security services.

Public trust in security services is a critical social-psychological phenomenon, yet existing theoretical accounts remain fragmented and insufficiently attentive to the structural power asymmetries characterising security-public relations. This article argues that the dominant paradigm, procedural justice theory, requires fundamental revision within intergroup and institutional power dynamics. Drawing upon procedural justice theory (Why people obey the law, 1990, Yale University Press; Why people obey the law, 2nd ed., 2006, Princeton University Press), social identity and self-categorisation

What race means for health among Black Americans: How microaggressions shape self-efficacy and readiness for behavior change.

OBJECTIVES: Racial microaggressions in health care are an underexamined mechanism through which structural racism shapes health behavior change among Black Americans. Drawing on microaggression and ecological systems theory, this research examines how essentialist health messages and clinical microaggressions relate to self-efficacy and readiness for cardiovascular health behaviors. METHOD: Across two experimental mixed methods studies, a nationally representative sample of Black American adults viewed health messages varying in racial essentialism, from absent to ambiguous to explicit microa

Re-emergence of trichinellosis in southeastern Europe due to political and economic changes.

The countries of southeastern Europe including the Balkan region and bordering countries - Albania, Bulgaria, Bosnia and Herzegovina, Croati … sis is sporadic. The re-emergence of trichinellosis is connected with the changes in the social and political systems in Bulgaria and Romania. In Serbia and Montenegro as well in Croatia, however, a re-emergence of trichinellosis was due not only to political and social changes but also to wars that took place in these countries during the last years of the 20th ce …

Political conservatism and social distancing from people living with HIV among Medical students: mediating roles of negative stereotypes and negative Intergroup emotions.

BACKGROUND: HIV-related stigma within the health care system is a major barrier preventing people living with HIV (PLWH) from accessing and continuing treatment. Psychosocial factors such as political orientation, personality characteristics, and personal moral values of health care providers have not been adequately investigated. Furthermore, a deeper understanding of the mechanisms and effects of these drivers on social distancing from PLWH is needed. The present study aims to fill these gaps in the literature by examining the stigmatizing attitudes of medical students from the perspective o

Plundering the poor: the role of the World Bank in the Third World.

The World Bank, the most important so-called development assistance agency, annually dispenses billions of dollars to Third World governments, ostensibly to "develop" their economics through a variety of loan projects. But even a superficial analysis reveals that the Bank is the perfect mechanism to help (i.e., subsidize) the large transnational corporations from the industrial countries to expand their industrial, commercial, and financial activities in the Third World, at the expense of the latter and particularly at the expense of the rural and urban proletariat. This article discusses Cher

All Smiles are Not Created Equal: Morphology and Timing of Smiles Perceived as Amused, Polite, and Embarrassed/Nervous.

We investigated the correspondence between perceived meanings of smiles and their morphological and dynamic characteristics. Morphological characteristics included co-activation of Orbicularis oculi (AU 6), smile controls, mouth opening, amplitude, and asymmetry of amplitude. Dynamic characteristics included duration, onset and offset velocity, asymmetry of velocity, and head movements. Smile characteristics were measured using the Facial Action Coding System (Ekman, Friesen, & Hager, 2002) and Automated Facial Image Analysis (Cohn & Kanade, 2007). Observers judged 122 smiles as amused, embarr

The emergence of political priority for safe motherhood in Honduras.

Each year an estimated 500,000 to 600,000 women die due to complications from childbirth, making this one of the leading causes of death globally for women in their reproductive years. In 1987 a global initiative was launched to address the problem, but few developing countries since then have experienced a documented significant decline in maternal mortality levels. Honduras represents an exception. Between 1990 and 1997 the country's maternal mortality ratio--the number of deaths due to complications during pregnancy, childbirth and the postpartum period per 100,000 live births--declined 40%

When health meets politics: Selective gatekeeping of health experts in Chinese media during COVID-19.

Recent global health crises have underscored the need for adaptive policymaking amid evolving evidence, alongside the political vulnerability of science communication. This study investigates the politics of health policy communication in a system where the government, media, and experts closely coordinate their messaging. We propose a Selective Gatekeeping framework to explain how state and media actors collaborate to amplify scientific voices aligned with policy objectives while marginalizing alternative perspectives. Using China's COVID-19 policy (2020-2022) as a case study, we analyze over

The political economy of national health insurance schemes: evidence from Zambia.

Governments in low and middle-income countries (LMICs) are increasingly considering the introduction of national health insurance scheme (NHIS) as a strategy to achieve universal health coverage (UHC) targets. The literature has widely documented the technical challenges associated with implementing UHC policies in LMICs but much less is known about the political process necessary to pass UHC legislation. In this article, we document the political economy issues surrounding the establishment of the Zambia NHIS in 2018. We adapted a political economy framework incorporating, semi-structured int

Lessons learned from the Global Polio Eradication Initiative: A roadmap for the international COVID-19 vaccination campaign.

The COVID-19 vaccine is lauded by many as one of the greatest accomplishments in modern medicine, with the potential to definitively contain the deadliest pandemic of the last century. With the vaccine rollout now underway in the developing world, a robust, methodical, and swift global distribution effort is required to ensure that it will be done in an equitable manner. Taking into account the vast geographic, socioeconomic, cultural, and political diversity of countries around the world, global vaccination efforts have historically required multifaceted, time consuming, and labor-intensive a

[Transgender care in the USA, advances and setbacks, and how it impacts Europe].

What is happening in the United States in terms of support for trans people, as in other areas, is regularly observed and a source of inspiration at the international level; the recent political turmoil around affirmative trans care in some States, especially for minors, has a tremendous impact in Europe, being regularly used by the "camps" that argue today on this issue. However, the political context and the organization of the health and health insurance system color this support in a very specific way in the United States, and omitting these aspects or simplifying them is a source of erron

One-health approach as counter-measure against "autoimmune" responses in biosecurity.

This Swine flu pandemic of 2009 and the potential Avian flu threat of 2011-2012 have revived a most challenging debate on protection against infectious diseases. The response to the Swine flu pandemic has been ambivalent, both on the societal (political) and the scientific level. While some scientists warned against potential massive loss of human lives and urged for immediate and large-scale vaccination, others accused them of unnecessary scaremongering, arguing that the pandemic would not be that severe. The lab-created virulent Avian flu virus - which has been created in order to 'fight' a

[Transcultural aspects of family therapy].

The transcultural view is evident throughout all of Positive Family-Therapy. It helps us to understand the individual conflicts and can also be important in dealing with such social issues as the treatment of illegal aliens and refugees, foreign aid for the Third World countries, problems in dealing with members of other cultural systems, interracial and transcultural marriages, prejudices, alternative life-styles societies and political problems. 4 ways of dealing with conflict and the 5-stage Positive Family-Therapy are therapeutic strategies in which therapy and self-help are intertwined.

[Health sector reform in Colombia and its effects on tuberculosis control and immunization programs].

This paper analyzes the effects of health reform in Colombia on public health programs at the local level, particularly the Expanded Program of Immunization (EPI) and the tuberculosis control program. The research was developed in three health districts in two States by analysis of documents, direct observation, and longitudinal follow-up of the transition process. The health districts were unprepared for the change, resulting in insufficient technical skills among staff as well as lack of awareness of important elements in the new system, associated with a reduction in immunization coverage a

Quantifying racial inequality in transit access across New York City.

Urban transit networks are crucial for sustainable cities, yet their uneven distribution often results in unequal access to urban resources. Prior studies primarily focused on accessibility within transit networks while overlooking its implications for residents' behaviors and the historical and political roots of inequality. Using door-to-door transit information and over 66 million mobility records, we examine racial disparities in New York City's transit accessibility and their impact on mobility behaviors. We find that ethnic minorities have 4.9% lower neighborhood accessibility, 15.7% les
