OBJECTIVE: The objective of this study is to evaluate current means of monitoring of pregnant women victims of abdominal trauma and to deter … for 7 patients (8%). Three patients (3%) presented fetal complications: one fetal death, one fetal porencephaly, one premature birth at 34 weeks gestation for premature rupture of the membranes and abruptio placentae. These three women were traffic accident victims who also suffered extra-abdominal trauma. Ultrasonographic signs (npv=99%) and anomalies foetal …

Ommaya reservoirs, commonly used in the management of intrathecal chemotherapy, can sometimes lead to complications, including cerebral edema and porencephaly. We present the case of a 65-year-old woman with a suprasellar craniopharyngioma who developed these rare complications after the placement of an Ommaya reservoir. The initial CT and MRI findings revealed a hypodense area in the right frontal lobe along the Ommaya catheter, consistent with a vasogenic edema. Laboratory work-ups, including inflammatory markers, were normal, and the patient did not exhibit fever. This case highlights the n…

Disruptions of fetal structures can create a variety of congenital anomalies. Some apparent prenatal vascular disruptions associate strongly with decreased maternal age, and are rare with older mothers. This is well-documented for gastroschisis, but similar findings with hydranencephaly suggest a general phenomenon that may also involve porencephaly, septo-optic dysplasia, early body stalk disruptions, certain hemifacial anomalies, and other findings. Prenatal vascular disruption may be a common cause of congenital anomalies, but its nature is unknown, and obvious environmental confounders ass…

Since 2005, several neurological diseases, including porencephaly, leukoencephalopathy and intracerebral hemorrhage without hypertension, and retinal arteriolar tortuosity have been linked to mutations in the COL4A1 gene, which encodes the alpha1 chain of type IV collagen, the main constituent of basement membranes. In three families, we observed a new syndrome that we called HANAC, for hereditary angiopathy with nephropathy, aneurysms and muscle cramps, which is associated with morphological alterations of cutaneous and renal basement membranes. This novel "basalopathy" is caused by glycine m…

Maternal chronic ethanol abuse during pregnancy causes malformations of the offspring. Three children (aged 6 months, 9 months, 4 1/2 years) and 3 fetuses (17th, 18th, and 20th gestational week) showed a wide spectrum of disorders ranging from severe dysraphic state, arhinencephaly, porencephaly, agenesis of corpus callosum, a range from hydranencephaly to microdysplasias (p.e. reduced gyration of dentate nucleus and inferior olives), and a range from gastrochisis or congenital heart defects to craniofacial dysmorphogenesis and palmar crease anomalies. The patterns of the cerebral malformation…

Six children with an oro-facio-digital syndrome (Papillon-Leage and Psaume) underwent pneumoencephalography. In 3 cases, the examination was normal. In 3 cases, severe abnormalities were discovered. These abnormalities included: agenesis of the corpus callosum (3/3); a markedly asymmetric skull with enormous porencephalic cavities (2/3); heterotopia of the grey matter (2/3). There were no differences in neurological state nor I.Q. between the children with cerebral malformations and those with a normal pneumo-encephalogram. The association of agenesis of the corpus callosum and porencephaly is…

The author discusses the value of isotope cisternography and ventriculography in the diagnosis of a variety of congenital anomalies of the central nervous system (CNS) associated with either hydrocephalus or a disturbance in cerebrospinal fluid (CSF) flow and absorption. In sincipital encephaloceles the techniques demonstrate CSF communication with the anomalous sac and the frequently associated hydrocephalus with occipital lesions. In porencephaly and cerebral agenesis, the isotope demonstrates ventricular reflux and remains in the cystic dilatation indicating abnormal CSF absorption; isotope…

We investigated the developmental outcome and features of brain imaging in 33 infants with moderate or severe depression of background EEG activities immediately after birth. Lesions due to perinatal injury were observed in 25 infants on neonatal ultrasonography and/or childhood MRI. These findings strongly correlated with their gestational age. Periventricular leukomalacia and posthermorrhagic porencephaly were observed in preterm infants. On the other hand, perinatal injury of mature brain type, such as multicystic encephalomalacia, parasagittal infarct, bilateral basal ganglia/thalamic lesi…

This study investigates the clinical features of epilepsy in 20 patients with brain malformation. Epileptic seizures were recognized in 15 patients, 12 of whom had their first seizure by 1 year of age. Partial seizure was the initial seizure type in 10 patients. Epileptic seizures were controlled in only four patients. Patients with holoprosencephaly and lissencephaly had seizure onset by 3 months of age, resulting in the most severe neurologic outcome. Only two patients with porencephaly had epileptic seizures, and in one of those patients the seizures were well controlled. A wide variety of …

We describe a 22-week female fetus after pregnancy was terminated because of fetal magnetic resonance imaging showing a large left cerebral hemispheric cleft suggestive of porencephaly or schizencephaly. Postmortem examination revealed a large cavernous angioma of the left opercular region with evidence of previous hemorrhage and extensive cerebral infarction. In the right hemisphere, another vascular malformation within the frontal germinal matrix consisted of an aggregate of primitive vessels not yet canalized. Selective dysgenesis of the right subiculum also was demonstrated. This case illu…

It has been suggested that the surviving cotwin and the mother are seriously affected when intrautrine fetal death of one monozygotic twin occurs. Several authors have reported brain damage in the surviving twin coexisting in utero with dead monozygotic cotwin. Most of such instances are monochorionic and show vascular communication between paired twins in the placenta. In a total of 133 pairs of monochorionic twins, death of one twin during pregnancy occurred in 33 cases (24.8%). Of 33 surviving twins, 8 babies suffered from porencephaly, cerebral palsy and other abnormalities. More unfavorab…

In children at risk of neurological damage, such as those delivered prematurely or with history of birth asphyxia, cranial ultrasonography i … ent with porencephalic cysts. Brain magnetic resonance imaging is the gold standard for identifying porencephaly and distinguishing it from other cysts. However, because of time restrictions in emergencies, cranial ultrasonography for a quick assessment in neonates and infants can be appropriate, as was the case in our encounter.

Collagen type IV, alpha-1 (COL4A1) variants can cause cerebrovascular diseases, such as porencephaly and cerebral hemorrhage, in addition to other autosomal dominant hereditary diseases. Patients with COL4A1 variants can present with epilepsy, most commonly focal epilepsy. In this paper, we present five patients, three of whom were examined by the authors, and two who were previously reported. Clinically, these five patients were characterized by the presence of West syndrome (WS), periventricular leukomalacia (PVL), and microcephaly, but none had a history of premature birth or hypoxic ischem…

Mutations in the COL4A1 gene, which encodes one of the chains of type IV collagen, affect the basement membrane of various organs, including the brain, eyes, kidneys, and skeletal muscle. These abnormalities can manifest as early as fetal life with a highly variable clinical spectrum, particularly in the central nervous system, where conditions such as intracerebral hemorrhages, porencephaly, hydranencephaly, schizencephaly, hydrocephalus, and periventricular leukomalacia are observed. Extracerebral manifestations include congenital cataracts, intraocular hypertension, hematuria, and arrhythmi…

The author describes 2 cases of children with growing skull fractures (GSFs). Surgical exploration of the widened fracture shortly after the head injury failed to reveal a dural tear because the neuroimaging studies (MR images, CT scans, and skull radiographs) had not been accurately interpreted, thereby allowing the development of a GSF at the site of the actual dural injury. In both cases, the dural and bony defect and the leptomeningeal cyst were successfully repaired. To prevent GSFs associated with progressive neurological deficit, seizure, ventricular porencephaly, and encephalomalacia, …

Congenital and acquired cerebrovascular diseases in the very young age group are reviewed and discussed. Whilst saccular aneurysms are rare … cavernous hemangiomas. Acquired arterial lesions may develop during fetal life by embolism, causing porencephaly or unilateral brain atrophy. The "Moyamoya" syndrome represents a frequent multi-arterial lesion causing characteristic ischemic episodes. Etiology is still obscure. We do not even know if the disease is acquired or congenital.
