A nationwide survey of prescribing patterns for thrombolytic drugs in acute myocardial infarction.

In November 1988, 164 hospitals enrolled in the Drug Surveillance Network participated in a nationwide survey of prescribing patterns for thrombolytic drugs for patients with an acute myocardial infarction. The results indicated that alteplase has made dramatic inroads, being used exclusively in 14.6% of the hospitals; in 64% of the hospitals both alteplase and streptokinase were on the formulary. Overall, however, only 17% of patients admitted with an acute myocardial infarction were treated with a thrombolytic, and use of these agents varied markedly across institutions. One of the reasons f

Impact of a pharmacist/physician cooperative target drug monitoring program on prophylactic antibiotic prescribing in obstetrics and gynecology.

The pharmacist's role in promoting rational, cost-effective use of drugs has been described in the literature. In a target drug monitoring program (TDMP), a single agent or group of agents becomes targeted for review. Antibiotics have been the primary focus of TDMP because of their therapeutic impact and cost considerations. The objectives of this project were to assess the prophylactic antibiotic prescribing habits of OB/GYN physicians and to evaluate the impact of a pharmacist/physician cooperative TDMP on prophylactic antibiotic prescribing and cost. The study was conducted in three phases:

Board certification and urban-rural migration of physicians in Japan.

BACKGROUND: The board certification system serves as a quality assurance system for physicians, and its design and operation are important health policy issues. In Japan, board certification was established and operated independently by academic societies and has not been directly linked to reimbursement systems. The phenomenon of younger physicians seeking specialist careers has raised concerns about acceleration of the tendency of fewer physicians working in rural areas and the maldistribution of physicians. Little is known about the associations between physicians' geographical migration pa

Insights into prescribing patterns for antidepressants: an evidence-based analysis.

BACKGROUND: Antidepressants are a primary treatment for depression, yet prescribing them poses significant challenges due to the absence of clear guidelines for selecting the most suitable option for individual patients. This study aimed to analyze prescribing patterns for antidepressants across healthcare providers, including physicians, physician assistants, nurse practitioners, and pharmacists, to better understand the complex factors influencing these patterns in the management of depression. METHODS: Least Absolute Shrinkage and Selection Operator (LASSO) regression was employed to ident

Effect of pharmacist intervention on physician prescribing in patients with chronic schizophrenia: a descriptive pre/post study.

BACKGROUND: Although pharmacotherapy is one of the most important treatments for schizophrenia, the prominent levels of antipsychotic polypharmacy and high-dose regimens used in Japan are thought to be inconsistent with treatment regimens used in other countries. In this study, we evaluated the effect of pharmacist intervention on physician prescribing in patients with chronic schizophrenia. METHODS: Participants comprised 52 inpatients at Sawa Hospital (Osaka, Japan), treated with at least one antipsychotic agent, who received pharmacist intervention for 1 year (2012). We compared the dose a

Nicotine pouches: Harm reduction or initiation pathway? A pharmacy-focused review.

BACKGROUND: Nicotine pouches (NPs) are a rapidly growing category of tobacco-free oral nicotine products increasingly available in the community, including pharmacies. Whether NPs primarily function as harm-reduction tools for existing tobacco users or as a novel initiation pathway into nicotine and tobacco use, particularly among youth, remains a critical and unresolved clinical and public health question. OBJECTIVES: To synthesize current evidence on the pharmacology, harm-reduction potential, and initiation risk of NPs; to compare NPs with FDA-approved nicotine replacement therapy (NRT) an

Who receives psychiatry-focused pharmacogenomic testing, and is it associated with prescribing patterns and acute care utilisation in depression? Real-world evidence from a large health system.

BACKGROUND: Depression is highly prevalent, and antidepressant response and tolerability vary widely. Pharmacogenomic (PGx) testing is increasingly used to support antidepressant selection, but its real-world uptake, equity, and associations with prescribing and healthcare utilisation remain incompletely characterised. METHODS: Using electronic health record data from a large health system (2013-2023), we identified 1563 patients with depression who received psychiatry-focused PGx testing and compared them with 248,017 indexed non-PGx controls. We examined demographic and clinical factors ass

Structured Pharmacist-Led Vaccine Recommendation and Coadministration in Community Pharmacies: Real-World Evidence from Poland.

Background and Objectives: Community pharmacies in Poland administer an expanding range of adult vaccines, including influenza, COVID-19, respiratory syncytial virus (RSV), herpes zoster and pneumococcal vaccines. While pharmacist vaccination services are increasingly available, evidence on structured approaches supporting vaccine coadministration remains limited. This study assessed whether a structured pharmacist-led recommendation and coadministration protocol was associated with greater uptake of multiple vaccines and same-day coadministration in routine community pharmacy practice. Oral lichen planus induced by long-term use of antimicrobials for recurrent aphthous ulcer: A case report and literature review.

The precise etiology of oral lichen planus (OLP) is still unclear, but the existing evidence suggests that drug intake, virus infection, fungal infection, psychological disorders, and immunodeficiency are closely associated with the pathogenesis of OLP. We report a case of OLP accompanied with candidiasis induced by long-term use of antimicrobials for recurrent aphthous ulcer (RAU) and update the literature, to discuss the possible association between OLP and misuse of antimicrobials, and to inform general dentists and pharmacists the importance for practice with optimal antimicrobial stewards

Antibiotic dispensing in Egyptian community pharmacies: an observational study.

BACKGROUND: Antibiotics are commonly dispensed medications from community pharmacies, and they are frequently prescribed for inappropriate indications. In many countries, they are easily accessible without prescriptions. The inappropriate use of antibiotics results in the emergence of resistant bacterial strains, which represents a considerable public health problem, particularly in developing countries. OBJECTIVE: This study aimed to describe the pattern of antibiotics dispensing from Egyptian community pharmacies and to collect baseline descriptive data on the antibiotics dispensed and thei
