Selective termination of aneuploidy utilizing rapid fluorescence in situ hybridization detection techniques.

Twin pregnancy following assisted reproductive technology with a euploid fetus and a coexisting aneuploid co-twin constitutes a conflicting situation; therefore, it is important for the genetic constitution of each co-twin to be diagnosed accurately and promptly for parental genetic counseling and subsequent aggressive management. A 35-year-old woman, gravida 1, with a 2-year history of infertility, presented bilateral fallopian tubal obstruction at her infertility workups, for which she received in vitro fertilization; subsequently she conceived a twin pregnancy. She underwent genetic amnioce

Limb body wall complex complicating a dichorionic diamniotic twin pregnancy: MRI for demonstration of fetal morphology.

Limb body wall complex (LBWC) is a rare, lethal malformation characterised by body wall defects, craniofacial and limb anomalies with or without various other organ anomalies. We report a case of dichorionic diamniotic twin pregnancy discordant for LBWC, diagnosed by ultrasound (US) and confirmed by MRI at 21 weeks' gestation, managed expectantly and delivered at 35 weeks by emergency caesarean section with a favourable outcome of the unaffected twin. The anomalous twin, who died soon after birth, had a sizeable thoracoabdominal wall defect, eviscerated liver and bowel loops attached to the pl

Heterotopic cervical pregnancy management after a high-complexity assisted reproduction procedure.

Heterotopic cervical pregnancies with a viable intrauterine pregnancy are rare, the conservative and prudent management of these cases should be focused on the selective reduction of cervical pregnancies in order to maintain eutopic pregnancy. This paper reports the first case in Ecuador of a heterotopic cervical pregnancy after ICSI procedure and its proper management by ultrasound-guided curettage, allowing the normal development as an intrauterine pregnancy.

Prenatal diagnosis of thalassemia in twin pregnancies in mainland China.

The aim of this study was to describe our experience with prenatal diagnosis of thalassemia in twin pregnancies at a tertiary referral centre in mainland China. All cases of twin pregnancies at risk of α- or β-thalassemias were followed over an eight-year period. There were 32 twin pregnancies at a mean gestation of 14 weeks (range, 11-21 weeks) at risk of thalassemias, including 19 at risk of α-thalassemia and 13 at risk of β-thalassemia. Chorionicity was determined by ultrasound before prenatal diagnosis. In 19 twin pregnancies at risk of α-thalassemia, there were four where both foetuses we

Selective Reduction Using Intravascular Potassium Chloride Injection after Laser Surgery for Twin-Twin Transfusion Syndrome.

Selective reduction (SR) via intravascular potassium chloride (KCl) injection is contraindicated in monochorionic twins due to the presence of placental vascular communications, which may serve as a conduit for inter-twin passage of KCl or allow exsanguination of the living twin into the demised twin. After successful selective laser photocoagulation of communicating vessels (SLPCV) for twin-twin transfusion syndrome (TTTS), the twins' circulatory systems are rendered independent. Theoretically, intravascular KCl injection into one twin after successful SLPCV should not result in passage of th

Obstetrical outcomes of embryo reduction and fetal reduction compared to non-reduced twin pregnancies.

PURPOSE: To prevent perinatal morbidity and mortality of high-order multiple pregnancy (HOMP), multifetal pregnancy reduction (MPR) is offered to some patients. In this study, we investigated whether twin pregnancies derived from MPRs carry a higher adverse obstetrical outcome compared to non-reduced control group of twins. METHODS: We retrospectively analyzed the data from HOMPs on which transvaginal ER (n = 153) at a mean gestational age of 7.6 weeks or transabdominal FR (n = 59) at a mean gestational age of 12.4 weeks was performed between December 2006 and January 2018. Nonsurgical management of heterotopic abdominal pregnancy.

BACKGROUND: Heterotopic abdominal pregnancies with coexisting intrauterine pregnancies pose unique therapeutic challenges, and management options, particularly nonsurgical approaches, are limited. CASE: We present a case in which selective reduction of a heterotopic abdominal pregnancy during the second trimester using fetal intracardiac injection with potassium chloride enabled subsequent vaginal delivery of the intrauterine pregnancy at term. In addition, we summarize nine cases of nonsurgical management of heterotopic abdominal pregnancies, four of which involve potassium chloride selectiv

Fetal reduction by bipolar cord coagulation in managing complicated monochorionic multiple pregnancies: preliminary experience in China.

BACKGROUND: Monochorionic multiple pregnancies (MMPs) are associated with higher rates of perinatal morbidity and mortality caused by interfetal vascular anastomoses in the monochorionic placenta, which can lead to fetal health interactions. In some circumstances, selective feticide of the affected fetus is necessary to save the healthy co-twin. We evaluated the effects and safety of our initial experiences using bipolar cord coagulation for the management of complicated MMPs. METHODS: Using ultrasound-guided bipolar cord coagulation, we performed selective feticide on 14 complicated MMPs (5

Intrauterine twin pregnancy complicated by cornual heterotopic pregnancy: A case report and narrative review.

BACKGROUND: Intrauterine twin pregnancy complicated by cornual heterotopic pregnancy is a rare pregnancy situation that demands effective intervention to avert rupture of the cornual pregnancy, while also striving to preserve the intrauterine fetus. CASE DESCRIPTION: A 31-year-old woman presented with a complex pregnancy scenario involving an intrauterine twin gestation alongside a cornual heterotopic pregnancy subsequent to in vitro fertilization (IVF) treatment. Under the guidance of abdominal ultrasound, a transabdominal puncture was performed to administer potassium chloride directly at t

Selective termination: a life-saving procedure for complicated monochorionic gestations.

Monochorionic twin pregnancies are a subset of twin pregnancies that face potential complications related to a shared circulation between the fetuses. These complications are related to anastomotic placental vessels connecting the cardiovascular systems of the two fetuses, which can result in significant sequela if one twin experiences intrauterine death. The sudden cardiovascular collapse in this scenario leads to a massive blood shift away from the healthy co-twin, significantly jeopardizing its life and long-term neurodevelopmental outcome. Such conditions include selective fetal growth res

Twin pregnancy, fetal reduction and the 'all or nothing problem'.

Fetal reduction is the practice of reducing the number of fetuses in a multiple pregnancy, such as quadruplets, to a twin or singleton pregnancy. Use of assisted reproductive technologies increases the likelihood of multiple pregnancies, and many fetal reductions are done after in vitro fertilisation and embryo transfer, either because of social or health-related reasons. In this paper, I apply Joe Horton's all or nothing problem to the ethics of fetal reduction in the case of a twin pregnancy. I argue that in the case of a twin pregnancy, there are two intuitively plausible claims: (1) aborti

[Trisomy 18 in a bichorial-biamniotic twin pregnancy].

Screening for chromosomal abnormality such as trisomy 18 in a bichorial-biamniotic twin pregnancy is based on ultrasound and a non-invasive prenatal test (NIPT) from 12 weeks of gestation. An invasive examination such as amniocentesis is necessary for a diagnostic confirmation. The management of these complicated cases consists in performing a selective feticide in the first or third trimester of pregnancy. Trisomy 18 most often results from a chromosomal nondisjunction of maternal origin. Indeed, advanced maternal age is a major cause of chromosomal abnormality and a promoting factor of twin
