Counterfactual prescriptions via hierarchical ML for missed chemotherapy appointment prevention.

Missed medical appointments, including cancellations and no-shows, disrupt clinical workflows, reduce resource efficiency, and can compromise patient care. This study develops a machine learning (ML) framework to jointly predict cancellations and no-shows using approximately 1.8 million chemotherapy appointments from Dana-Farber Cancer Institute in Boston, Massachusetts, USA. We implement a hierarchical ML approach that first detects cancellations and then predicts no-shows among the remaining cases. Combining static appointment features with dynamic temporal features derived from prior schedu

Recreational and Medical Cannabis Legalization and Opioid Prescriptions and Mortality.

IMPORTANCE: While some have argued that cannabis legalization has helped to reduce opioid-related morbidity and mortality in the US, evidence has been mixed. Moreover, existing studies did not account for biases that could arise when policy effects vary over time or across states or when multiple policies are assessed at the same time, as in the case of recreational and medical cannabis legalization. OBJECTIVE: To quantify changes in opioid prescriptions and opioid overdose deaths associated with recreational and medical cannabis legalization in the US. DESIGN, SETTING, AND PARTICIPANTS: Thi…

Prescriptions of opioid-containing drugs in patients with chronic cough.

BACKGROUND: Chronic cough (CC) affects about 10% of adults, but opioid use in CC is not well understood. OBJECTIVES: To determine the use of opioid-containing cough suppressant (OCCS) prescriptions in patients with CC using electronic health records. DESIGN: Retrospective cohort study. METHODS: Through retrospective analysis of Midwestern U.S. electronic health records, diagnoses, prescriptions, and natural language processing identified CC - at least three medical encounters with cough, with 56-120 days between first and last encounter - and a 'non-chronic cohort'. Student's t-test, Pearso…

Sociodemographic and pharmacological variables and the identification of prescriptions that were potentially inappropriate due to the risk of worsening acne were considered. RESULTS: A total of 21,604 patients with acne were identified. Median age was 20.8 years (interquartile range: 17.3-27.3 years), and 60.7 percent were female. Tr …

Ursodiol Prescriptions Following Bariatric Surgery: National Prescribing Trends and Outcomes.

BACKGROUND: Rapid weight loss after bariatric surgery is a risk factor for gallstone development. Numerous studies have shown that ursodiol after surgery decreases rates of gallstone formation and cholecystitis. Real-world prescribing practices are unknown. This study aimed to examine prescription patterns for ursodiol and reassess its impact on gallstone disease using a large administrative database. METHODS: The Mariner database (PearlDiver, Inc.) was queried using Current Procedural Terminology codes for Roux-en-Y gastric bypass (RYGB) and sleeve gastrectomy (SG) between 2011 and 2020. Onl

Predicting benzodiazepine prescriptions: A proof-of-concept machine learning approach.

INTRODUCTION: Benzodiazepines are the most commonly prescribed psychotropic medications, but they may place users at risk of serious adverse effects. Developing a method to predict benzodiazepine prescriptions could assist in prevention efforts. METHODS: The present study applies machine learning methods to de-identified electronic health record data, in order to develop algorithms for predicting benzodiazepine prescription receipt (yes/no) and number of benzodiazepine prescriptions (0, 1, 2+) at a given encounter. Support-vector machine (SVM) and random forest (RF) approaches were applied to…

Opioid prescriptions among the World Trade Center Health Program population.

BACKGROUND: The World Trade Center Health Program (Program) provides limited health care to those directly affected by the 9/11 terrorist attacks. Because of physical/mental trauma arising from the 9/11 attacks, Program members might be at high risk of opioid use. To prevent prescription opioid overuse, in 2018 the Program implemented various measures to improve opioid prescribing and expand access to non-opioid pain management among Program members. However, the characteristics of opioid prescriptions dispensed among this population has never been described. METHODS: Administrative and claim…

Safety Aspects of Extemporaneous Prescriptions Prescribed by Latvian Healthcare Specialists.

Until 1991, Latvia had been part of the Union of Soviet Socialist Republics, and medicines in Latvia were prepared in accordance with the Union of Soviet Socialist Republics Pharmacopoeia and standardized prescriptions. Latvia regained its independence 30 years ago, but Latvia still does not have its standardized prescriptions and official sources of information in the official language. So far, there have been no studies which would collect data about prescribed extemporaneous prescriptions and characterize the current situation in different Latvian regions. For this reason, we analyzed the e…

Clinical Evidence of Tai Chi Exercise Prescriptions: A Systematic Review.

OBJECTIVES: This systematic review aims to summarize the existing literature on Tai Chi randomized controlled trials (RCTs) and recommend Tai Chi exercise prescriptions for different diseases and populations. METHODS: A systematic search for Tai Chi RCTs was conducted in five electronic databases (PubMed, Cochrane Library, EMBASE, EBSCO, and Web of Science) from their inception to December 2019. SPSS 20.0 software and Microsoft Excel 2019 were used to analyze the data, and the risk of bias tool in the RevMan 5.3.5 software was used to evaluate the methodological quality of RCTs. Interprovincial Variation of Psychotropic Prescriptions Dispensed to Older Canadian Adults.

BACKGROUND: Utilization of psychotropic medications among the elderly has garnered attention due to concerns about safety and degree of efficacy, but may be used differently across regions. METHODS: We conducted a cross-sectional study of all antipsychotic, benzodiazepine, and trazodone prescriptions dispensed to seniors ( ≥ 65 years) leveraging IQVIA (Durham, NC) GPM data in 2013. We report the units dispensed (per 100 seniors) by province. RESULTS: Nationally, on average, 26,210 units of antipsychotics, 24,257 of benzodiazepines, and 7,519 of trazodone were dispensed in 2013 for every 100 …

Chronic Pain Opioid-Maintained Patients Receive Less Analgesic Opioid Prescriptions.

Treating pain and opioid use disorder represents a clinical challenge. While most studies that have assessed opioid analgesic use in opioid substitution treatment (OST) patients primarily address opioid analgesic misuse (1, 2), only few studies focused on OST patients assessed the prescription of analgesic opioids for chronic pain. We sought to compare the prevalence of analgesic opioid prescription (AOP) in two groups of chronic non-cancer pain (CNCP) patients: OST patients vs. the general population. This was a population-based cross-sectional study based on the French national healthcare cl

DESIGN AND METHODS: A register study, including data from the Swedish Forensic Pathology and Forensic Toxicology databases 2003-2010, the Prescribed Drug Register and the National Patient Register. RESULTS: A total of 1301 deaths, assessed as related to buprenorphine, methadone or heroin, or a combination of them, were studied. The largest number of fatalities was related to intake of heroin (n = 776), followed by methadone (n = 342) and buprenorphine (n = 168)…

[Off-label prescriptions in acute psychiatry: a practice-based evaluation].

BACKGROUND: In acute psychiatry the prescribed medication is frequently off-label. AIM: To investigate the indications for off-label prescriptions described in the literature and to evaluate the use of off-label medication in the emergency service of Altrecht, Utrecht, The Netherlands. METHOD: We performed a systematic search of the PubMed database and we evaluated all the records of patients examined by the emergency service during two assessment periods (April 2009 and November 2010). RESULTS: The literature makes no mention of the prescribing of off-label medication in acute psychiatry. …
