A framework for improving quality. Using project study teams, Providence Health System tackles problem areas.

To identify issues for quality improvement and monitoring, an organization first articulates its service mission and objectives. The organization uses internal measurements and self-examination with external accountability and benchmarks to select a manageable number of projects for study. Supported by collected evidence, published data, and broad-based approval, projects focus on the process or outcomes of care, frequency of services, or patient populations. An accountable leader selects team members from the affected disciplines, who review evidence, agree on what and how to measure, determi

Recovery-oriented service provision and clinical outcomes in assertive community treatment.

OBJECTIVE: While the term "recovery" is routinely referenced in clinical services and health policy, few studies have examined the relationship between recovery-oriented service provision and client outcomes. The present study was designed to examine the relationship between recovery-orientation of service provision for persons with severe mental illnesses and outcomes in Assertive Community Treatment (ACT). METHODS: Client, family, staff, and manager ratings of service recovery-orientation and outcomes across a range of service utilization and community functioning indicators were examined a

[Dupuytren's contracture].

For an uncomplicated Dupuytren's contracture (no pain or paresthesias), the currently accepted optimal treatment option is surgery in stage II-III. The fewer the secondary alterations to tendons, skin and joints, the better are the functional results that may be expected. Further factors impacting on the surgical outcome are the choice of incision and the occurrence of early complications (hematomas, graft necrosis, wound healing disorders, infections).

Mixed venous oxygen saturation. Its role in the assessment of the critically ill patient.

Except in the presence of left to right shunts, slow aspiration from the distal lumen of a catheter placed in the pulmonary artery can provide a sample of mixed venous blood. The oxygen saturation of such a sample (SvO2) is dependent on arterial oxygen saturation, hemoglobin concentration, cardiac output, and tissue oxygen demands. A fall in SvO2 usually implies anemia, arterial oxygen desaturation, and/or decreased cardiac output; however, a normal or high value does not exclude such disturbances. When employed in conjunction with the other indicators of tissue oxygenation available in an int

The quality of clinical maternal and neonatal healthcare - a strategy for identifying 'routine care signal functions'.

BACKGROUND: A variety of clinical process indicators exists to measure the quality of care provided by maternal and neonatal health (MNH) programs. To allow comparison across MNH programs in low- and middle-income countries (LMICs), a core set of essential process indicators is needed. Although such a core set is available for emergency obstetric care (EmOC), the 'EmOC signal functions', a similar approach is currently missing for MNH routine care evaluation. We describe a strategy for identifying core process indicators for routine care and illustrate their usefulness in a field example. MET

Myocardial Infarction Quality of Care and Outcomes in Asian Ethnic Groups in the United States.

BACKGROUND: National-level differences in myocardial infarction (MI) quality of care among Asian patients in the United States are unclear. We assessed the quality of MI care in the 6 largest US Asian ethnic groups. METHODS: Patients aged ≥18 years with ST-segment-elevation MI or non-ST-segment-elevation MI in the Get With The Guidelines-Coronary Artery Disease registry (711 US hospitals, 2015-2021) were assessed. The odds of MI-related quality of care and process outcomes were evaluated in Asian Indian, Chinese, Filipino, Japanese, Korean, Vietnamese, and other Asian adults compared with non

Implementation of a fluid balance control strategy in critically ill patients: POINCARE-2 trial process evaluation.

BACKGROUND: POINCARE-2 trial aimed to assess the effectiveness of a strategy designed to tackle fluid overload through daily weighing and subsequent administration of treatments in critically ill patients. Even in highly standardized care settings, such as intensive care units, effectiveness of such a complex intervention depends on its actual efficacy but also on the extent of its implementation. Using a process evaluation, we aimed to provide understanding of the implementation, context, and mechanisms of change of POINCARE-2 strategy during the trial, to gain insight on its effectiveness an

Pre-pyloric versus post-pyloric feeding.

Optimal management of the critically ill patient involves the initiation and rapid advancement of early enteral nutrition (EN). Compared to parenteral nutrition or no nutritional support, early enteral feeding favorably impacts patient outcome by reducing infectious morbidity and shortening hospital length of stay. Controversy exists over the true risks and benefits of pre-pyloric versus post-pyloric feeding. Placement of nasogastric tubes is easier than nasojejunal tubes, initiation of EN is more expedient, and intragastric feeds may provide greater physiologic benefits. Post-pyloric feeding,

Home versus hospital phototherapy for term infants with hyperbilirubinemia: a comparative study.

Twenty-five term infants with hyperbilirubinemia were selected for phototherapy administered at home if they fulfilled clinical and laboratory criteria designed to exclude any pathologic cause of hyperbilirubinemia. Ongoing nursing surveillance of home phototherapy was provided to assure adequate nursing care. No phototherapy-related complications were noted in the study infants. Effectiveness of phototherapy at home was compared with results in a similar group of 33 infants who underwent standard hospital phototherapy. The average decrease in serum bilirubin concentration was similar for the

Monitoring of the folic acid supplementation program in the Netherlands.

In November of 1993, the Dutch government recommended daily folic acid supplementation of 0.4 or 0.5 mg for all women planning pregnancy, starting 4 weeks before conception until 8 weeks after. In 1995, a one-time mass media campaign was conducted, and due to this campaign, the use of folic acid in this recommended period increased from 4.8% in 1995 to 21% in 1996. Subsequently, no structural strategies were undertaken until 2003, and at that time, 22% of lower-educated women used folic acid in the recommended period. The prevalence of neural tube defects (NTD) decreased from 11.44/10,000 befo

The experience of sexual assault: findings from a statewide victim needs assessment.

A statewide assessment was conducted of assaults, experiences, needs, and recommendations of 125 adult victims receiving care at 19 sexual assault centers (SACs) in the State of Maryland. More than one half of the victims (55.6%) waited years before disclosing, with delays in reporting especially likely if the assault was perpetrated by a family member (the most frequent perpetrators at 42.4% of respondents). About one half of the victims (51.3%) had been previously sexually assaulted, yet only 9% of these victims had sought treatment. The majority of respondents (69.4%) indicated they would n

From the field: a maternal-child health nursing competence validation model.

This group of Vermont community health nurses from different agencies collaborated to develop a competence validation framework for maternal and child health nursing in the practice areas of perinatal client teaching, breastfeeding, and prenatal, postpartum, and newborn nursing care. The framework is based on the work of Benner, using the "competent" level of nursing practice, and delineates three parameters of competence: technical skills, interpersonal skills, and critical thinking skills. Learning resource materials, including newborn and maternal assessment guidelines, were developed for e

Outcome Prediction in Clinical Treatment Processes.

Clinical outcome prediction, as strong implications for health service delivery of clinical treatment processes (CTPs), is important for both patients and healthcare providers. Prior studies typically use a priori knowledge, such as demographics or patient physical factors, to estimate clinical outcomes at early stages of CTPs (e.g., admission). They lack the ability to deal with temporal evolution of CTPs. In addition, most of the existing studies employ data mining or machine learning methods to generate a prediction model for a specific type of clinical outcome, however, a mathematical mode

The use of collaboration science to define consensus outcome measures: a telemental health case study.

The purpose of this document is to provide an overview of a collaboration science process used to develop recommendations for the field of telemental health (TMH) in the selection of outcome measures that best reflect programmatic impacts. A common use of group development techniques in medicine is the development of clinical guidelines, which typically occurs using one of two methods: the nominal group or the Delphi method. Both processes have been faulted for limited transparency, reliability, and sustainability. Recommendations to improve the traditional process include making goals explici

Systematic review of cardiac rehabilitation guidelines: Quality and scope.

Cardiac rehabilitation is a comprehensive model of secondary prevention proven to reduce mortality and morbidity. The World Health Organization is developing a Package of Rehabilitation Interventions for implementation by ministries of health as part of universal healthcare across the continuum. Through a systematic review, we sought to identify the best-quality cardiac rehabilitation guidelines, and extract their recommendations for implementation by member states. A systematic search was undertaken of academic databases and guideline repositories, among other sources, through to April 2019,

Proposal of quality indicators for cardiac rehabilitation after acute coronary syndrome in Japan: a modified Delphi method and practice test.

OBJECTIVES: Cardiac rehabilitation is underused and its quality in practice is unclear. A quality indicator is a measurable element of clinical practice performance. This study aimed to propose a set of quality indicators for cardiac rehabilitation following an acute coronary event in the Japanese population and conduct a small-size practice test to confirm feasibility and applicability of the indicators in real-world clinical practice. DESIGN AND SETTING: This study used a modified Delphi technique (the RAND/UCLA appropriateness method), a consensus method which involves an evidence review,

Borderline personality disorder and group therapy.

This study examined the process and outcome of group therapy for community health center clients with borderline personality disorder. The results indicated that group members rated Yalom's curative factors to be increasingly helpful over time, and that universality and existential factors were consistently ranked as very helpful group process variables. The group sessions also were associated with positive outcomes, as evidenced by significant improvement in Goal Attainment Scale scores and the depression and hostility scales of the Brief Symptom Inventory. The results of this study suggest t

Interventions to enhance breast self-examination practice: a review.

Breast self-examination (BSE) is an important component of any program for the early detection of breast cancer. Compared to clinical breast examination and mammography, BSE is relatively safe, low cost, offers monthly assessment, and does not require overcoming barriers associated with access to the medical care system (unless an abnormality is discovered). Over the past decade, considerable research has been directed at encouraging women to perform BSE. This paper reviews 33 intervention studies that vary in the type and intensity of strategies employed, and in the BSE behaviors assessed. In
