Improving the communication of healthcare professionals with affected children and adolescents.

All communication with children and adolescents with disorders of sex development (DSD) must be age appropriate and tailored to their developmental levels and needs. Communication is as much about information concerning the DSD as it is about listening to the individual's thoughts, questions and anxieties. Especially small children are dependent on, and are best reached or helped through the parents. If the diagnosis is made early the child should 'grow into disclosure'. The parents and the child should be involved in a continuous educational interactive process. Sexual maturation and developm

De-escalating angry and violent clients.

Lately, encounters with angry clients seem to be more frequent and sometimes violent. While I do not claim to know why this is happening, in the course of 20 years of practice, I have developed insights into managing the angry and violent and de-escalating these situations.

Parent and adolescent communication with healthcare professionals about Type 1 diabetes management at adolescents' outpatient clinic appointments.

AIM: This study aimed to identify the ways in which adolescents living with Type 1 diabetes and their parents communicate about Type 1 diabetes management with healthcare professionals in a clinical setting. METHODS: Twenty-nine adolescents (aged 11-17 years) and their parents were purposively recruited from two outpatient clinics for non-participant observations. Outpatient clinic appointments, which consisted of multiple consultations with healthcare professionals, were observed and audio-recorded. Outpatient clinic appointments were categorized based on the nature and extent of communicati

Putting Your Trauma Lens On.

Trauma in childhood is now understood to cause long-term effects on the brain and body. The pediatric provider, using a "trauma lens," which constitutes observing a child's attachment, resilience, and stress response, is well poised to identify and support children and families at risk. Fortunately, resilience is a dynamic process that can be learned, enhanced, and supported. Familiarity with the most common symptoms of traumatic stress will help the medical provider quickly recognize which children are impacted or FRAYED (Fits, Frets, and Fear; Regulation disorders; Attachment problems; Yawni

Developing a novel typology of unprofessional behaviours between healthcare staff: a best fit framework synthesis.

BACKGROUND: Unprofessional behaviours such as bullying, harassment, and microaggressions negatively affect patient safety and staff psychological wellbeing in healthcare systems globally. These behaviours do so by: (i) inhibiting health care professionals’ abilities to speak up to raise safety concerns; (ii) impairing team communication and individuals’ concentration; and (iii) promoting tolerance of bad practice. Unfortunately, there is little consensus in practice or academia about how these behaviours are defined. This can lead to an underestimation of the prevalence of these behaviours, in

Catastrophe and Creativity: From Fragmentation to Emergence.

This paper explores Ferenczi's path towards truth: the honest emotional link between patient and analyst that occurs first and foremost on a somatic level. The body does not lie. However, the perceptual registration of the soma when used defensively can be destructive instead of creative. The paper explores truth as it relates to the beauty of love and the catastrophe of loss. It follows emotion in relationship, thought as a reflection of intersubjectivity, and the role these play in structuralizing of mind. All these processes are both catastrophic and creative. They are essential for truth t

Therapeutic Relationship: A Factor for Substantial Change in Psychodynamic Psychotherapies and Psychoanalysis.

Significant change in psychodynamic psychotherapy resides in the uncovering of unconscious conflicts as they manifest themselves in the lives of patients. My intent in this article is to illustrate the special power of the therapeutic relationship as a separate element beyond interpretation as a vehicle for a change. This is illustrated by three patients who revealed special feelings of affection for me in the context of our work together and the progress that they had made. The substrate of these feelings is a product of a consistent acceptance and valuation of the patient in all that they re

La Famiglia è Tutto: Why Families Must Be at the Bedside.

Open Intensive Care Units (ICUs) represent one of the most significant transformations in the humanization of health care by removing temporal, physical, and relational barriers. While limitations on visiting hours are often implemented to alleviate concerns about infection spread or issues with interference with care, numerous studies have demonstrated overwhelming benefits of family members at patients' bedside, including reduced incidence of delirium and shorter hospital stays. Family members are vital teammates in the patient recovery process and, with clear engagement methods, can help pr

Tools used to measure the therapeutic relationship between staff and service users in adult mental health care: A scoping review.

BACKGROUND: Therapeutic relationships are key to both service user recovery and the safety of staff and service users in adult mental health care. However, staff over-involvement (crossing professional boundaries including sexual and emotional exploitation) and under-involvement (staff disinterest, avoidance or neglect) is often a cause for concern within mental health care. Little is known about measuring and assessing over / under involvement. This scoping review provides a broad understanding of existing tools used to measure this in adult mental health care. OBJECTIVE: To explore what mea

Flexibility between immersion and distancing: Relationship with depressive symptoms and therapeutic alliance.

OBJECTIVES: High levels of change are linked to the flexibility between immersion and distancing, as well as to higher levels of therapeutic alliance. This study aims to explore the evolution of flexibility between immersion and distancing throughout the entire therapeutic process and its relationship with therapeutic alliance and depressive symptoms in a clinical case. METHOD: We analyzed five sessions of a good outcome case of depression undergoing cognitive-behavioral therapy. We assessed the distancing/immersion of these sessions using the Measure of Immersed and Distanced Speech, and the

A Commentary on "A Therapist's Guide to Issues of Intimacy and Hostility Viewed as Group-Level Phenomena".

The article by J. Dugo and A. Beck, "A therapist's guide to issues of intimacy and hostility viewed as group-level phenomena" (1984), International Journal of Group Psychotherapy 34, 1, 25-45, was reviewed to determine its applicability to current clinical practice and for instructions and training for leaders of psychotherapy groups. The authors write from the psychoanalytic perspective to propose four leader roles for differing group phases and describe the impact of each leader type on the functioning of the group, the leader, and the group members. Central to the thesis for the article are

Confidence in the therapist and confidence in the treatment predict symptomatic improvement week by week in therapy: A latent curve modeling approach.

Previous research suggests that common relationship factors are composed of two overarching factors, "Confidence in the therapist" and "Confidence in the treatment." The aim of this naturalistic process-outcome study was to investigate the reciprocal relationships between these two constructs and patients' symptom level across treatment. The sample consisted of 587 patients who were admitted to an inpatient program and treated with psychotherapy for a range of mental health disorders, such as chronic depression, anxiety disorders, and eating disorders. Our data consisted of weekly measures of

Patient personality dimensions, relational patterns and therapeutic alliance in clinical practice: An empirical investigation.

INTRODUCTION: Transference (meant in this context, as the patient relational patterns expressed towards the clinician) and therapeutic alliance play a crucial role in the treatment of personality pathology. To date, no empirical study examined the association between these two dimensions of the clinical relationship and patients' personality maladaptive traits in psychotherapy. METHODS: A national sample of therapists (N = 100) of different theoretical orientations assessed dysfunctional personality features of a patient in their care using a comprehensive and empirically grounded dimensional

Nonverbal synchrony as a marker of alliance ruptures.

Findings from the past 5 decades of empirical research on the working alliance suggest its importance in psychotherapy. Recent studies have sought to identify markers of the alliance, of which one of the most promising candidates is nonverbal synchrony. Delving into processes that constitute the alliance, such as alliance ruptures, may shed light on underlying mechanisms of the association between nonverbal synchrony and the therapeutic relationship. The present study examines whether nonverbal synchrony can serve as a marker of alliance ruptures. To achieve this aim, 418 sessions of 75 therap

Clients in context: Environment, class, race, and therapists' perceptions of generalized anxiety disorder.

UNLABELLED: Evidence-based practice necessitates the inclusion of client identity and contextual information when conceptualizing diagnosis. OBJECTIVE: To examine how therapists' perceptions of Generalized Anxiety Disorder is influenced by client environmental contextual and identity factors, like class and race. METHOD: Therapists (n = 138; 76% women; Mage  = 38.3) were randomly assigned three of six client vignettes and asked to provide diagnostic recommendations, confidence in diagnosis, and perception of client's concerns. Vignettes differed in their description of client class, race, an

Support for people who use Anabolic Androgenic Steroids: A Systematic Scoping Review into what they want and what they access.

BACKGROUND: Since there is a paucity of research on support for people using Anabolic Androgenic Steroids (AAS), we aimed to identify and synthesise the available evidence in this field. Gaining an understanding of the support both accessed and wanted by recreational AAS users will be of use to professionals who provide services to intravenous substance users and also to those working in the fields of public health and social care, with the aim to increase engagement of those using AAS. METHODS: A systematic scoping review of the literature to explore and identify the nature and scope of info

Malchish-Kibalchish: cultural factors in a clinical case.

In this paper, a Jungian understanding of cultural factors influencing individual analysis is illustrated with the case of a patient suffering from panic attacks. The analysis revealed that, in addition to the patient's personal background, the collectivistic attitudes of the Soviet culture, which had a moulding effect on the patient in his childhood and obstructed his individuation, should be taken into account. The concepts of the totalitarian object and the Russian cultural complex encompassing a grandiosity pole and an inferiority pole are used to explore the patient's condition, and the c

[Psychotherapeutic considerations regarding medication treatment for refractory cases].

The aim of this paper is to consider the psychotherapeutic approach to refractory cases in a psychiatric clinic. Although standardized, formulated psychotherapies, i. e., CBT and IPT, have a limited efficacy against antidepressant-resistant or chronic depression, psychotherapy and medication treatment may complement each other in combination. However, the first step in the psychotherapeutic consideration of refractory depression is to give up seeking "a specific medicine" which does not exist. On the other hand, a doctor should give his/her patient new hope for recovery and stimulate their mot
