We report three patients with a history of neuroleptic malignant syndrome for whom modified electroconvulsive therapy (m-ECT) was scheduled. Two patients suffered from schizophrenia, and one suffered from depression. Their symptoms, such as hyperthermia, consciousness disturbance, myotonus, tremor, sweating, and tachycardia, improved gradually with administration of dantrolene and fluid infusion. However, their psychotic state was exacerbated. Therefore, m-ECT was scheduled. When patients were restless at the hospital ward, they were sedated with propofol and transferred to the operating room.

BACKGROUND: Propofol emulsion, a widely used anesthetic in clinical practice, can lead to propofol infusion syndrome (PRIS) when improperly administered. PRIS is characterized by dyslipidemia and cytotoxic damage, particularly affecting peripheral neutrophils.
METHODS: Using an ex vivo whole blood model followed by flow cytometric analysis, this study elucidates the mechanisms underlying PRIS-induced cytotoxicity.
RESULTS: Significant lipid droplet accumulation and increased neutrophil cell death were observed, with cell granularity correlating strongly with lipid droplet levels. Apoptosis w…

We present an emergency anesthetic management of craniotomy for a 22-year-old man with congenital cyanotic heart disease due to brain abscess. Pulmonary blood flow was completely supplied via major arteriopulmonay collatelal artery (MAPCA). This patient complicated with Eisenmenger syndrome, has no history of cardiac surgery but several times of craniotomy due to repeated brain abscess. Total intravenous anesthesia with propofol and remifentanil was induced and maintained. SpO2 and PaO2 were elevated after oxygen administration via mask. Although systemic blood pressure level was decreased by

: The anaesthetic management of patients with myopathies is challenging. Considering the low incidence and heterogeneity of these disorders, most anaesthetists are unfamiliar with key symptoms, associated co-morbidities and implications for anaesthesia. The pre-anaesthetic assessment aims at the detection of potentially undiagnosed myopathic patients and, in case of known or suspected muscular disease, on the quantification of disease progression. Ancillary testing (e.g. echocardiography, ECG, lung function testing etc.) is frequently indicated, even at a young patient age. One must differenti

BACKGROUND: Spinal cord stimulation is a validated approach for managing chronic pain syndromes. The stimulator placement typically requires sedation, and an awake phase is needed to ensure optimal lead positioning. We describe a novel multimodal sedation approach using target-controlled infusions of propofol, remifentanil, and dexmedetomidine, combined with boluses of ketamine, guided by electroencephalography and nociception-antinociception balance monitoring.
METHODS: This retrospective, single-center cohort study reviewed all spinal cord stimulator procedures, including both trials and pe

Case report of a five year-old boy who was admitted for elective squint surgery. The medical history revealed some delay in his behavioural and mental development as well as an unspecific dysmorphic syndrome with suspected muscular weakness. Tonsillectomy and adenotomy have been performed six months prior to admission. During examination on the day before surgery, the child presented only mild symptoms of a subsiding upper respiratory infection. Because of the uncooperativeness of the child the pharyngeal inspection was not performed. On the day of surgery total intravenous anaesthesia was ind

We detail a rare cause of forearm compartment syndrome that occurred in an 18-year-old patient who presented with a Glasgow Coma Scale of 13/15 after a mixed drug overdose and subsequently required intubation. She suffered extravasation of her propofol infusion, which resulted in intrinsic compression within her forearm muscle compartments. Fortunately, the diagnosis of compartment syndrome was made swiftly and the patient was taken to theatre within 3 h where she underwent an emergency forearm fasciotomy. She made an uneventful recovery and at follow-up her wounds had healed well with no asso

OBJECTIVE: We aim to describe management of a patient receiving renal transplantation for chronic renal failure due to Alport syndrome with low dose of intrathecal bupivacaine and continuous epidural infusion of local anesthetic.
CASE REPORT: A 38-years-old man with chronic renal failure secondary to Alport syndrome underwent kidney transplantation. Because of a high risk of respiratory and cardiovascular complications related to the patient's baseline lung disease and abnormalities in heart conduction, we selected combined spinal-epidural anesthesia. The block was ultrasound-guided and perfo

We report a case of paroxysmal supraventricular tachycardia (PSVT) that occurred during video-assisted thoracoscopic (VATS) lobectomy in a patient with concealed Wolff-Parkinson-White (WPW) syndrome. A 59-year-old man with lung cancer was scheduled for VATS lobectomy under general anesthesia. After inserting a thoracic epidural catheter, general anesthesia was induced with intravenous administration of propofol. Anesthesia was maintained with inhalation of desfurane in an air/oxygen mixture and intravenous infusion of remifentanil. Recurrent PSVT occurred three times, and the last episode of P

BACKGROUND: A higher-than-usual resistance to standard sedation regimens in COVID-19 patients suffering from acute respiratory distress syndrome (ARDS) has led to the frequent use of the second-line anaesthetic agent ketamine. Simultaneously, an increased incidence of cholangiopathies in mechanically ventilated patients receiving prolonged infusion of high-dose ketamine has been noted. Therefore, the objective of this study was to investigate a potential dose-response relationship between ketamine and bilirubin levels.
METHODS: Post hoc analysis of a prospective observational cohort of patien

Obesity and sleep apnea syndrome (SAS) are significant risk factors for airway management, necessitating comprehensive preoperative evaluation, careful preparation, and adaptive strategies to prevent complications, such as hypoxemia, during sedation and general anesthesia. We report the use of intravenous moderate/deep sedation to manage a patient with morbid obesity and severe SAS undergoing dental treatment using a high-flow nasal cannula (HFNC) system. The sedation was performed with a propofol infusion and was closely monitored using bispectral index (BIS) monitoring and capnography. Prepa

Reversible cerebral vasoconstriction syndrome (RCVS) is a group of disorders characterized by prolonged reversible cerebral vasoconstriction, associated with acute-onset, severe, episodic "thunderclap" headaches with or without other neurologic clinical features. We describe the first reported case, to our knowledge, of a patient with RCVS who was managed under general anesthesia in the field of oral and maxillofacial surgery. A 67-year-old woman with a diagnosis of RCVS made 2 months earlier was scheduled to undergo surgical extraction of the mandibular right third molar. Standard monitoring

Mitochondrial disorders encompass a group of syndromes produced by genetic defects that disrupt mitochondrial energy production. The impaired mitochondrial energy supply affects nearly all organs and tissues leading to a variable clinical presentation. The possible multisystem involvement complicates the management of anaesthesia and perioperative care. Exact knowledge of the path physiology of mitochondrial diseases may help to avoid perioperative anaesthesiological complications. This report describes the anaesthetic management of a patient with a mitochondrial disorder during combined pancr

Implantable cardioverter defibrillators (ICD) have become indispensable in managing life-threatening ventricular arrhythmias. On average, 50%-70% of the patients receive a device-based therapy within the first two years post implantation. A few patients experience the electrical storm (ES). ES is a syndrome of recurrent ventricular tachycardia or fibrillation occurring two or more times in a 24-hour period, calling for the need of electrical cardioversion or defibrillation to stabilize the patient. We present the case of a patient with severe cardiomyopathy who presented with resistant ES afte

Sedation and analgesia are a constant challenging issue in paediatric intensive care units, for ethical reasons among others. Basically, goa … longer than several hours should be strongly discouraged in infants and children due to the risk of Propofol Infusion Syndrome.
