Despite advances in our understanding of surgical site infections following total joint arthroplasty, this serious surgical complication continues to represent a substantial economic burden for the patient, the treating institution and the healthcare system. After increasing for the past decade, infection rates have stabilized at 1.6%; however, the total cost is projected to increase with the total number of revision procedures performed. A systematic review of the literature was performed to identify studies that assess the efficacy of pre-, peri- and post-operative infection prevention strat

INTRODUCTION: Patients with lower extremity amputations using a classic socket prosthesis face many challenges related to the socket-limb interface. The adaptation of osseointegration has allowed for the attachment of a prosthesis directly to bone, eliminating this interface and providing mechanical benefits. Contrary to the socket prosthesis, the osseointegrated prosthesis requires reducing and minimizing the soft tissue envelope. Studies have shown that patients who have undergone placement of these implants have high rates of reoperation for soft tissue redundancy. The purpose of our study

BACKGROUND: Prosthetic joint infections (PJIs) are a growing matter of concern due to their economic and social burden on health systems. In Italy, surgical data on PJIs are available in a national registry, but microbiological data are still scarce.
MATERIALS AND METHODS: We performed a retrospective study at a single center with records of patients treated for primary PJIs of knee or hip from January 1, 2011, to May 30, 2018. Patients with infections of osteosynthesis means and external devices were excluded, as well as PJI recurrences and polytrauma patients. Infections were diagnosed acco

The risk of intravascular cardiac device infection (ICDI) has increased significantly over the last decade and is rising faster than the corresponding increase in device implantations. ICDIs may consist of pocket infections, wire infections, wire infections complicated by endocarditis, or combinations of these. In this review, we examine the microbiology of ICDIs, focusing on the risk of developing ICDI following bloodstream infections with key pathogens such as Staphylococcus aureus, coagulase-negative staphylococci, enterococci, etc., as well as the pathophysiology of these infections.

BACKGROUND: Despite substantial progress in graft materials and surgical techniques, arterial graft infections continue to pose significant clinical challenges. The optimal graft material remains debated, with prosthetic grafts being associated with elevated reinfection and mortality rates. Given the growing interest in xenogeneic pericardium, this study aimed to systematically evaluate its efficacy and safety in the management of prosthetic graft infections.
METHODS: We conducted a systematic review and meta-analysis in accordance with PRISMA guidelines. PubMed/MEDLINE and the Cochrane Libra

INTRODUCTION: Destructive aortic root endocarditis is associated with high mortality rates. The objective of this article was to characterize the clinical and microbiological profiles of these patients, especially concerning an already implanted aortic valve prosthesis. We also focused on prognostic factors.
METHODS: Eighty patients underwent aortic root replacement due to destructive endocarditis from 1999 to 2018 in our institution. We analyzed their pre, intra, and postoperative data, outcomes, and predictors of mortality.
RESULTS: Thirty-one patients had native valve endocarditis (NVE),

BACKGROUND: Bone and joint infection involving Granulicatella adiacens is rare, and mainly involved in cases of bacteremia and infectious endocarditis. Here we report three cases of prosthetic joint infection involving G. adiacens that were successfully treated with surgery and prolonged antimicrobial treatment. We also review the two cases of prosthetic joint infection involving G. adiacens that are reported in the literature.
CASE PRESENTATION: Not all five cases of prosthetic joint infection caused by G. adiacens were associated with bacteremia or infectious endocarditis. Dental care befor

Operative joint replacement to treat disabling joint conditions secondary to degenerative and inflammatory arthritides has become one of the most efficacious and cost-effective procedures to relieve pain and restore joint function. However, prosthetic implants are not built to last forever and osteolysis and aseptic loosening has been associated with prosthetic arthroplasties since their introduction. The functional life of a synthetic joint is influenced by many factors including the material of the implant, operation procedures and the surgeon involved, as well as patient-related factors. Al

BACKGROUND: Low-grade infections are caused by low-virulence pathogens. The course of these infections is often mild, which is why they are often delayed or not recognized at all. Chronic infections can lead to osteolysis and implant loosening. The rate of complications requiring revision, such as implant loosening or material failure, is known from the literature. However, the rate of low-grade infections in patients requiring spinal revision surgery remains unclear.
PURPOSE: The aim of this review is to present the latest treatment strategies for low-grade infections. The diagnostic and the

Osteomyelitis and infections associated with orthopedic implants represent a significant burden of disease worldwide. Ceragenins (CSAs) are a relatively new class of small-molecule antimicrobials that target a broad range of Gram-positive and Gram-negative bacteria as well as fungi, viruses, and parasites. This review sets the context of the need for new antimicrobial strategies by cataloging the common pathogens associated with orthopedic infection and highlighting the increasing challenges of managing antibiotic-resistant bacterial strains. It then comparatively describes the antimicrobial p

Escherichia coli endocarditis is a rare condition, even though bacteriemia by such agent is common. The infection, normally from a urinary origin, may, in fact, progress without major hemodynamic disturbance and minimal symptoms, regardless its ability of destruction of heart's valvular apparatus. We present a case report of a 68-year-old man, with a history of aortic valvular mechanic prosthesis, dyslipidemia and hypertension, admitted at the emergency room with refractory fever and urinary tract symptoms. On the hypothesis of endocarditis, he was submitted to transesofagic echocardiography t

Staphylococcus aureus and coagulase-negative staphylococci are the commonest bacterial causes of both vascular graft and stent infections. Infection may occur either from direct implantation or haematogenous spread, and occurs in less than 1% of aortic grafts, 2% to 5% of inguinal grafts, and rarely in stents. Death or amputation is common following these infections despite aggressive treatment. Infection with S. aureus is usually more acute than with coagulase-negative staphylococci but both cause systemic symptoms. Inguinal infections usually cause localized swelling often with a sinus tract

A rare case of Corynebacterium striatum endocarditis on a bioprosthetic aortic valve replacement, treated medically, is reported. The presentation was subacute, and initially endocarditis screening was negative. Because of the failure of symptoms to settle further screening was performed which confirmed the organism in several sets of blood cultures. This emphasises the importance of persistent screening for endocarditis if the history raises any suspicion of this potentially serious infection, especially in the presence of prosthetic valves.

Kytococcus schroeteri is a Gram positive cocci usually found as a coloniser of the human skin and mucous membrane. It is a bacterium that is usually considered a contaminant in blood cultures. It can rarely cause infective endocarditis and is usually associated with prosthetic heart valve endocarditis. We report a case of early prosthetic valve endocarditis in a 78-year-old female and provide a brief review of similar cases published in the literature.

Fungal prosthetic valve endocarditis is an extremely severe form of infective endocarditis, with poor prognosis and high mortality despite treatment. Candida albicans is the most common etiological agent for this rare but increasingly frequent condition. We present a case of fungal prosthetic valve endocarditis due to C. albicans following aortic and pulmonary valve replacement in a 38-year-old woman with a history of surgically corrected tetralogy of Fallot, prior infective endocarditis and acute renal failure with need for catheter-based hemodialysis. Antifungal therapy with liposomal amphot

Actinobaculum schaalii is an emerging pathogen particularly involved in urinary tract infection of elderly people and/or patient with urological risk factors of urinary tract infection. This microorganism is a difficult-to-diagnose pathogen and is rarely involved in systemic or deep infections. Here, we report the first case of prosthetic joint infection due to A. schaalii in an 84-year-old man with a benign prostatic hyperplasia associated with chronic retention of urine. This case underlines the importance to optimize the diagnosis of emerging uropathogens as A. schaalii, to prevent systemic

Nocardia causes several types of infection in both immunocompetent and immunocompromised hosts, and may affect any organ. Endocarditis due to infection by Nocardia species is rare and has a poor prognosis. To our knowledge, after a literature review, only 12 cases of nocardial endocarditis have been reported, most treated with valve replacement. We report the case of a patient with prosthetic valve endocarditis due to Nocardia species, who was successfully treated with antimicrobial chemotherapy only (trimethoprim-sulfamethoxazole), instead of valve replacement.

BACKGROUND: Fever is common in surgical patients. The list of potential causes is long and includes many noninfective etiologies.
CAUSES: Only about 40% of fever episodes in hospitalized patients are caused by infection. Any fever in a surgical patient is a cause for concern. New or continuing fever more than three days after surgery should raise a strong suspicion of persistent illness or a new complication. On rare occasions, these fevers can be attributed to potentially life-threatening occult infections, including acute cholecystitis, antibiotic-related pseudomembranous colitis, systemic
