Cardiotoxicity model-based patient selection for Hodgkin lymphoma proton therapy.

INTRODUCTION: Hodgkin lymphoma (HL) is a highly curable hematological malignancy. Consolidation radiation therapy techniques have made significant progresses to improve organ-at-risk sparing in order to reduce late radiation-induced toxicity. Recent technical breakthroughs notably include intensity modulated proton therapy (IMPT), which has demonstrated a major dosimetric benefit at the cardiac level for mediastinal HL patients. However, its implementation in clinical practice is still challenging, notably due to the limited access to proton therapy facilities. In this context, the purpose of …

Modelling of late side-effects following cranial proton beam therapy.

BACKGROUND: The limited availability of proton beam therapy (PBT) requires individual treatment selection strategies that can be based on normal tissue complication probability (NTCP) models. We developed and externally validated NTCP models for common late side-effects following PBT in brain tumour patients to optimise patients' quality of life. METHODS: Cohorts from three PBT centres (216 patients) were investigated for several physician-rated endpoints at 12 and 24 months after PBT: alopecia, dry eye syndrome, fatigue, headache, hearing and memory impairment, and optic neuropathy. Dose-vol

Proton therapy for newly diagnosed pediatric diffuse intrinsic pontine glioma.

INTRODUCTION: Diffuse intrinsic pontine glioma (DIPG) is a type of brain malignancy with a very poor prognosis. Although various radiation and chemotherapy protocols have been attempted, only conventional radiotherapy has yielded improvements in survival. In this study, we aimed to compare proton therapy versus conventional photon radiotherapy in terms of the outcomes of pediatric patients with DIPG. METHODS: This retrospective review included 12 pediatric patients with newly diagnosed DIPG who received a total proton therapy dose of 54 Gy (relative biological effectiveness) in 30 fractions a…

Potential Morbidity Reduction With Proton Radiation Therapy for Breast Cancer.

Proton radiotherapy confers significant dosimetric advantages in the treatment of malignancies that arise adjacent to critical radiosensitive structures. To date, these advantages have been most prominent in the treatment of pediatric and central nervous system malignancies, although emerging data support the use of protons among other anatomical sites in which radiotherapy plays an important role. With advances in the overall treatment paradigm for breast cancer, most patients with localized disease now exhibit long-term disease control and, consequently, may manifest the late toxicities of a

Persistent gastro-oesophageal reflux symptoms despite proton pump inhibitor therapy.

About one-third of patients with suspected gastro-oesophageal reflux disease (GERD) do not respond symptomatically to proton pump inhibitors (PPIs). Many of these patients do not suffer from GERD, but may have underlying functional heartburn or atypical chest pain. Other causes of failure to respond to PPIs include inadequate acid suppression, non-acid reflux, oesophageal hypersensitivity, oesophageal dysmotility and psychological comorbidities. Functional oesophageal tests can exclude cardiac and structural causes, as well as help to confi rm or exclude GERD. The use of PPIs should only be co

[Ring recurrence of ciliary body melanoma after proton-beam therapy].

A malignant ciliary body melanoma was treated by proton-beam irradiation. Ring-shaped recurrence with extraocular extension appeared thirty-six months later and required enucleation. Histologic study revealed a predominant epithelioid cell type tumor, flattened in previously treated area. Proton-beam irradiation efficiency depends on clinical evaluation, which remains questioned in ciliary body locations where circumferential extension could be misevaluated.

Comparing the Robustness of Intensity-modulated Proton Therapy and Proton-arc Therapy Against Interplay Effects of 4D Robust-optimised Plans for Lung Stereotactic Body Radiotherapy.

AIMS: To assess the robustness of 4D-optimised IMPT and PAT plans against interplay effects in non-small cell lung cancer (NSCLC) patients with respiratory motion over 10 mm, and to provide insights into the use of proton-based stereotactic body radiotherapy (SBRT) for lung cancer with significant tumour movement. MATERIALS AND METHODS: Fourteen patients with early-stage NSCLC and tumour motion >10 mm were selected. Three hypofraction regimens were generated using 4D robust optimisation with the IMPT and PAT techniques. The nominal plan qualities for both techniques were compared, and their r

Selection of Mediastinal Lymphoma Patients for Proton Therapy Within the Proton Collaborative Group Registry: Concordance With the ILROG Guidelines.

PURPOSE: As patients with mediastinal lymphoma are typically young with curable disease, advanced radiation techniques such as proton therapy are often considered to minimize subacute and late toxicity. However, it is unclear which mediastinal lymphoma patients are treated with proton therapy. Within a prospective, multi-institutional proton registry, we characterized mediastinal lymphoma patients treated with proton therapy and assessed concordance with consensus recommendations published in 2018 by the International Lymphoma Radiation Oncology Group (ILROG). METHODS: Eligible patients inclu…

Proton Therapy for Muscle-Invasive Bladder Cancer: Outcomes from the Multicenter Proton Collaborative Group PCG 001-09 Registry Trial.

BACKGROUND: Muscle-invasive bladder cancer (MIBC) is treated with radical cystectomy or trimodality therapy (TMT), which yield comparable survival outcomes. Many patients undergoing TMT experience significant toxicity related to chemoradiation. The unique physics of proton radiation may allow for a more tolerable regimen. We herein report on the safety and efficacy of proton therapy for patients with MIBC. METHODS: Patients with non-metastatic MIBC treated with curative-intent proton radiation were enrolled in the multi-center Proton Collaborative Group (PCG) prospective, observational regist…

Dosimetric advantages of proton beam therapy compared to intensity-modulated radiation therapy for retroperitoneal chordoma.

Retroperitoneal chordomas are exceedingly rare and account for less than 5% of all primary bone malignancies. Their etiology remains unknown. We report a rare case of an extravertebral chordoma of the retroperitoneum in a 71-year-old man treated with surgical resection and post-operative spot-scanning proton beam therapy. We describe how to safely treat a retroperitoneal target to a prescription dose over 70 Gy (relative biological effectiveness) with spot-scanning proton beam therapy and also report a dosimetric comparison of spot-scanning proton beam therapy versus intensity-modulated radiat

Postoperative proton therapy for chordomas and chondrosarcomas of the spine: adjuvant versus salvage radiation therapy.

STUDY DESIGN: Retrospective comparative cohort series. OBJECTIVE: The aim of this study was to evaluate patients treated with proton therapy for chordoma and chondrosarcoma of the spine in the postoperative setting and to report local control, relapse-free, and overall survival outcomes. SUMMARY OF BACKGROUND DATA: Margin-negative resection of spinal chordomas and chondrosarcomas can be challenging, so adjuvant radiotherapy is often recommended. However, delivery of adequate radiotherapy is complicated by the relative radioresistance of these tumors, necessitating high doses, as well as the …
