Finite element analysis of basicervical femoral neck fracture treated with proximal femoral bionic nail.

BACKGROUND: Dynamic hip screws (DHS) and proximal femoral nail anti-rotation (PFNA) were recommended for basicervical femoral neck fracture (BFNF), however, with high rate of postoperative femoral neck shortening. The proximal femoral bionic nail (PFBN) was designed to decrease the postoperative complications associated with DHS and PFNA. The aim of this study is to compare the biomechanical characters of DHS, PFNA, and PFBN for fixation of BFNF. METHODS: Using finite element analysis, we created a three-dimensional model of the BFNF for this investigation. The PFBN group, the PFNA group and

Comparison of proximal femoral universal nail and proximal femoral nail anti-rotation internal fixation for older patients with intertrochanteric femoral fracture: a retrospective cohort study.

INTRODUCTION: Intertrochanteric femoral fractures (IFF) are among the most common lower limb fractures in the older population. Our team has developed a new cephalomedullary nail design called the proximal femoral universal nail (PFUN). This study aimed to compare the clinical outcomes of PFUN and proximal femoral nail anti-rotation (PFNA) internal fixation for the treatment of IFF in older patients. METHODS: We retrospectively reviewed the clinical records of 435 older patients with IFF who were treated at Peking University Third Hospital between January 2022 and January 2024. Among them, 21

An algorithm for the treatment of Vancouver type B2 periprosthetic proximal femoral fractures.

The number of individuals with periprosthetic fractures of the proximal part of the femur is increasing. Multiple treatment methods have been described but none that correlate to fracture type and few with mostly good-to-excellent results. This article describes an algorithmic reconstruction tactic for treating patients with periprosthetic fractures associated with a loose femoral component. The stem is approached through the fracture fragments. Reconstructing the tube of the proximal part of the femur with 18G cerclage wires allows for canal preparation and implantation of the new stem. Appli

[Fractures caused by bone insufficiency of the proximal femoral epiphysis. Contribution of MRI].

The fractures by bony insufficiency are particularly well seen on MRI. On T1 weighted images they are detected as a very hypointense line surrounded by a hypointense area. On T2 weighted images, the hypointense line is also recognized and represents the fracture surrounded by a hyperintense area due to oedema or hemorrhage. Five cases of bony insufficiency fracture of the femoral head are reported. The features of the superior subchondral locations of these fractures and some primary and limited epiphyseal osteonecrosis are similar and consistent with a common pathogeny.

Low-dose heparin in proximal femoral fractures. Failure to prevent deep-vein thrombosis.

The effect of heparin, 5000 units every 8 hours, on deep venous thrombosis in patients with proximal femoral fractures was investigated in a controlled, double blind, randomised study. Heparin or placebo was administered as soon as possible after the fracture, and before 6 hours had passed, and was continued for 14 days. The diagnosis of deep venous thrombosis was made using daily I125 fibrinogen scans. A total of 130 patients endured the trial and the results were registered on a sequential diagram. This showed that the 0-hypothesis could not be rejected, and that consequently no difference i…

Occurrence and clinical outcome of lateral wall fractures in proximal femoral fractures whose fracture line runs from femoral basal neck to subtrochanteric area.

Treatment methods for proximal femoral fractures, when the fractures run from the femoral basal neck to the subtrochanteric area, have not yet been fully reported. Thus, we aimed to clarify osteosynthesis methods based on the fracture frequency and clinical results. We classified the proximal femoral fractures using the Area classification method based on the location (area) of the fracture line. The proximal femur has 4 areas with 3 boundaries; the center of the femoral neck, the boundary between femoral neck and trochanter, and the plane connecting the lower ends of the greater trochanter an…

[Lesion of the sciatic nerve after antegrade intramedullary nailing of proximal femoral fractures].

Proximal femoral fractures are frequent injuries that typically require surgical treatment. A rare complication is iatrogenic injury to the sciatic nerve, often caused by cerclages or suboptimally reduced fracture fragments. Our retrospective study of 224 cases in 39 months revealed an incidence of 1.8% for postoperative sensory motor deficits. The treatment includes revision surgery, such as neurolysis or improved reduction of fracture fragments. Despite surgical revision, long-term deficits often persist due to slow nerve regeneration. A precise operative technique and early diagnostic work-…

Concomitant valgus hip osteotomy in operative fixation of displaced proximal femoral neck fractures.

Total hip arthroplasty (THA) is considered the preferred treatment for displaced proximal femoral neck fractures. However, in many countries this option is economically unviable. To improve outcomes in financially disadvantaged populations, we studied the technique of concomitant valgus hip osteotomy and operative fixation (VOOF). This prospective serial study compares two treatment groups: VOOF versus operative fixation alone with cannulated compression screws (CCSs). In the first series, 98 hip fixation procedures were performed using CCS. After fluoroscopic reduction of the fracture, three
