Prurigo nodularis is an uncommon pediatric cutaneous disorder that is associated with intense pruritus. We report a teenager with prurigo nodularis as the presenting sign of stage IIA Hodgkin's disease, primarily in the mediastinum. The pruritus and associated cutaneous lesions resolved with combination chemotherapy directed at the malignancy. In patients with prurigo nodularis and pruritus of unclear etiology, the possibility of underlying Hodgkin's disease must be considered.

Electron microscopic studies were carried out on cutaneous lesions in one case of prurigo nodularis Hyde. The cells of the basal layer partly showed large paranuclear vacuoles, which were sometimes divided by thin membrane structures. The intercellular space of the stratum basale and stratum spinosum was usually dilated, exhibiting acantholytic phenomen. Furthermore, numerous cells of these layers contain vacuolic-like mitochondria and polymorphous granular structures. Melanocytes and Langerhans-cells have not been observed, however, Merkel-cells were found in the basal layer, containing chara…

Prurigo nodularis (PN) is a chronic skin disease that manifests with severe itchy, firm, hyperkeratotic nodules distributed on the trunk and the extremities symmetrically. Here, we report a unique presentation of PN. A 26-year-old male presented with multiple itchy nodules over the hands and feet sparing the trunk, which were confirmed histologically as PN. This is the first reported case of PN with exclusive acral distribution.

Prurigo nodularis (PN) is a chronic, recalcitrant inflammatory skin condition characterized by the presence of pruritic nodules. The exact pathogenesis of the disease is unknown, although immune and neural dysregulation are indicated in driving the itchscratch cycle. Specifically, interleukin-4 and interleukin-31 pathways have been recently implicated in transmission of the pruritic sensation. There are currently no US FDA-approved targeted therapies for the treatment of PN. This article aims to review our present understanding of the disease pathogenesis and treatments, with a focus on emergi…

Prurigo nodularis (PN) is a chronic inflammatory skin disease characterized by intensely pruritic, hyperkeratotic nodules that favor the extensor surfaces of the extremities and the trunk. In addition to its significant impact on quality of life, many patients with PN are recalcitrant to therapy because there are currently no therapies approved by the US Food and Drug Administration. In the first article of this 2-part continuing medical education series, we describe the broader epidemiology, patient demographics, physical examination findings, and symptoms to aid in the timely recognition and…

Prurigo pigmentosa is an uncommon inflammatory skin disease predominately affecting young women. Clinically the disease presents with erythematous and urticarial papules arranged in a reticular pattern. Lesions heal with reticulated hyperpigmentation. Strict ketogenic diet is one of many factors that might trigger the disease. In this article, we present a case of prurigo pigmentosa following a complicated laparoscopic gastric sleeve with the resolution of the rash after improvement of the patient's diet.

Most patients with localized nodules should receive topical treatment first. But disappointing results or specific findings described here could necessitate additional or alternative options.

BACKGROUND/AIMS: Prurigo nodularis (PN) has multiple reported disease associations, and this study analyzes the demographics, clinical distribution and underlying etiologies in an Asian cohort.
METHODS: A 1-year retrospective study was carried out on PN patients with extensive lesions (≥10) who attended a specialized itch clinic.
RESULTS: 37 patients (46% male, median age 53 years) were included. In all patients, an underlying etiologic factor associated with pruritus could be identified. 49% (n = 18) of patients had a single attributable etiology, whilst 51% (n = 19) had multiple etiologies…
