Sexuality and sexual identity are developmental milestones that occur throughout the life span with sexual maturation usually occurring during adolescence. Parents and health care providers must prepare children with chronic conditions for this important component of adulthood to which they are entitled.

Characteristics by which to classify a woman as authentically transsexual are offered from earlier investigations and supplemented by findings from recent work with this type of patient. Developmental issues are discussed and six psychodynamic accompaniments listed. A case demonstrates how to identify the true female transsexual and how to understand her psychological processes.

Although the importance of the anal zone is well known in psychoanalysis, pruritus ani has not been studied recently in the psychoanalytic literature. This paper is presented to show how bisexuality in a male patient has been analysed through pruritus ani as the main presenting symptom. This article illustrates how the assessment of the level and degree of dependency in psychotherapy can be used as a diagnostic and therapeutic tool. Dependency varies with the stage of psychosexual development, with the degree and state of illness, and with the stage of treatment. The syndrome of male pseudohermaphroditism, caused by a deficiency of the enzyme 5-alpha reductase, results in a selective decrease in dihyrotestosterone. Culture is an important part of the context in which decisions are made on sex assignment of patients with abnormalities of the external genitalia. In Turkey, patients with ambiguious genitalia are diagnosed very late and corrective surgery is usually performed during or after puberty. Here we present a case with 5-alpha reductase deficiency who was diagnosed at puberty and followed-up for 10 years.

Modern surgical treatment of hypospadias is reviewed. Early microsurgical correction of hypospadias is gaining ground in view of early presentation of the patient; this means minute tissue relations that require very precise reconstruction. The operative correction is described as an example of a complex treatment: psychosexual adjustment, endocrinological development and sexual function must all be taken into increasingly frequent account.
