Persistent pulmonary consolidation treated with intrapulmonary percussive ventilation: a preliminary report.

Intrapulmonary percussive ventilation (IPV) is a novel form of chest physiotherapy delivered by a percussive pneumatic device (IPV, Percussionaire, Sand Point, ID). There are few published reports about the use of IPV for diseases other than cystic fibrosis. We report our experience with three pediatric patients and one adult patient with persistent pulmonary consolidation refractory to conventional therapies. Three of the four patients had neuromuscular disease; one patient had segmental atelectasis due to aspiration. Three of the four patients showed clinical and radiographic improvement wit

Quantitative evaluation of pulmonary stretch receptor activity during high-frequency ventilation.

The purpose of this study was to determine the neural output of pulmonary stretch receptors (PSRs) in response to conditions that, in previous studies (J. Appl. Physiol. 65: 179-186, 1988 and Respir. Physiol. 80: 307-322, 1990), produced apnea in anesthetized cats. These conditions included changes in airway pressure (Paw; 2 or 6 cmH2O), stroke or tidal volume (1-4 ml/kg), frequency [conventional mechanical ventilation (CMV) vs. high-frequency ventilation (HFV) at 10, 15, and 20 Hz], and levels of inspired CO2 (0, 2, and 5%). These data were needed to assess properly the specific contribution

Central hemodynamics and pulmonary ventilation after trauma: effect of portacaval transposition.

Immediate reaction to traumatic shock involves, among other factors, trapping of platelets in the lungs. It has previously been shown that this trapping is associated with an increase in pulmonary vascular resistance (PVR). To elucidate whether this increase in PVR was due to cellular sequestration or other factors, we performed a portacaval transposition in a group of pigs (n = 9), making the liver the first receiving capillary bed for blood from the lower extremities. One week later, these animals and a control group (n = 6) were subjected to a standardized soft tissue trauma to the hind lim

Ventilation-perfusion scans in neonatal regional pulmonary emphysema complicating ventilatory assistance.

Two cases of ventilator-related neonatal lobar overexpansion with similar radiographic appearance, but probably different pathogenesis, are presented. In one infant, persistent interstitial lobar emphysema was confirmed by markedly decreased perfusion shown on scintigraphy; this information was of great value in predicting the beneficial effect of lobectomy. In the other case, ventilation and perfusion scans indicated functional value of the emphysematous lobe and correctly suggested conservative management. Radioisotope lung scans may provide valuable information regarding lung function in re

[Do the severity and duration of rheumatoid arthritis influence pulmonary ventilation?].

Pulmonary ventilation was studied in 100 patients with rheumatoid arthritis of an outpatient clinic, using whole-body plethysmography, spirography, and pneumometry. The patients with more or less severe joint involvement, as well as the patients with shorter or longer duration of their disease, did not show any significant difference in their dynamic and static pulmonary fuction. Patients with serious joint destructions showed a bronchial obstruction in 29%. 36 patients who were treated with thioglucose gold had a significantly better expiration capacity. A causal connection with the underlyin…

Automated patient-ventilator interaction analysis during neurally adjusted non-invasive ventilation and pressure support ventilation in chronic obstructive pulmonary disease.

INTRODUCTION: Delivering synchronous assist during non-invasive ventilation (NIV) is challenging with flow- or pressure-controlled ventilators, especially in patients with chronic obstructive pulmonary disease (COPD). Neurally adjusted ventilatory assist (NAVA) uses diaphragm electrical activity (EAdi) to control the ventilator. We evaluated patient-ventilator interaction in patients with COPD during NIV with pressure support ventilation (PSV) and NAVA using a recently introduced automated analysis. METHODS: Twelve COPD patients underwent three 30-minute trials: 1) PSV with dedicated NIV vent

Retrospective analysis of ventilation strategies for blunt pulmonary trauma patients: comparing the effectiveness of synchronized intermittent mandatory ventilation, continuous positive airway pressure, and pressure-controlled ventilation.

OBJECTIVE: To compare the efficacy of various ventilation approaches to the treatment of patient with patients with severe blunt chest trauma. METHODS: The retrospective study was conducted at the Cardiorespiratory Syndrome Ward of the First People's Hospital of Linping District, Hangzhou, China, and comprised data of patients with blunt pulmonary trauma admitted from December 2019 to February 2023. The patients were divided into three groups based on the ventilation support mode. Group 1 received synchronised intermittent mandatory ventilation + positive endexpiratory pressure, Group 2 recei

Outcomes of mechanically ventilated hematology patients with invasive pulmonary aspergillosis.

BACKGROUND: Invasive pulmonary aspergillosis (IPA) is a life-threatening infection documented in up to 15% of hematology patients who require intensive care for acute respiratory failure. We report outcomes in hematology patients given mechanical ventilation (MV) with IPA. METHODS: Retrospective study of all hematology patients given MV with IPA between January 1998 and March 2011 at a single center. Predictors of 6-month survival or mortality were identified using multivariable analysis. RESULTS: We studied 67 patients including 49 (73%) with neutropenia, 23 (34%) with long-term steroid the

[Pulmonary ventilation in patients with a varying degree of chronic kidney insufficiency].

Lung ventilation was determined in 116 patients with various degrees of chronic renal insufficiency. The patients examined were subdivided into 4 groups according to the values of serum creatinine and glomerular filtration. Lung ventilation was increased in all patient groups, mainly on the account of the increased inspiration reserve volume. The changes in respiration rate were insignificant. PaCO2 values in arterial blood were adopted, by the author, as a sign of an existing hyperventilation. A high correlation interdependence was established between CO2 pressure in arterial blood (PaCO2) an

Fatal re-expansion pulmonary oedema in a mechanically ventilated patient.

Fatal course of re-expansion pulmonary oedema (REPO) is infrequent and very rarely documented in mechanically ventilated patients. We report a case of fatal REPO following tube thoracostomy for a right-sided pneumothorax in an elderly patient of chronic obstructive pulmonary disease (COPD) with respiratory failure on mechanical ventilation.

Effect of general anesthesia and controlled mechanical ventilation on pulmonary ventilation distribution assessed by electrical impedance tomography in healthy children.

INTRODUCTION: General anesthesia is associated with the development of atelectasis, which may affect lung ventilation. Electrical impedance tomography (EIT) is a noninvasive imaging tool that allows monitoring in real time the topographical changes in aeration and ventilation. OBJECTIVE: To evaluate the pattern of distribution of pulmonary ventilation through EIT before and after anesthesia induction in pediatric patients without lung disease undergoing nonthoracic surgery. METHODS: This was a prospective observational study including healthy children younger than 5 years who underwent nonth…

A New and Safe Mode of Ventilation for Interventional Pulmonary Medicine: The Ease of Nasal Superimposed High Frequency Jet Ventilation.

We use pulmonary interventional procedures for the diagnosis of pulmonary diseases either for benign or malignant lesions. Flexible bronchoscopy with or without radial endobronchial ultrasound, convex-probe endobronchial ultrasound and electromagnetic navigation are procedures performed in centers with experience in diagnostic pulmonary medicine. The method of sedation and ventilation is very important in order to avoid or handle with success complications. Proper respiration during pulmonary (or other interventional) procedures is a key factor. Apart from the proper sedation method we have to
