Epidemiological studies of atomic bomb radiation at the Radiation Effects Research Foundation.

Epidemiological studies of people who were exposed to atomic bomb radiation and their children who were conceived after parental exposure to radiation (F1) have investigated late health effects of atomic bomb radiation and its transgenerational effects. Those studies were initiated by the Atomic Bomb Casualty Commission (ABCC) in the 1950s. ABCC was reorganized to the Radiation Effects Research Foundation (RERF) in 1975, which continued the work of the ABCC. Follow-up of vital status and cause of death is performed for all RERF cohorts, including the atomic bomb survivors (the Life Span Study:…

Radiation-induced insufficiency fracture of the femur 18 years after radiation therapy.

Advances in oncologic treatment have improved survival rates, allowing late effects of radiotherapy to become more prevalent. Our patient, an 82-year-old woman with a remote history of right thigh basal cell carcinoma treated with resection and radiation therapy 18 years prior, presented with severe right thigh pain and inability to bear weight as she had suffered a femur fracture after a fall from standing. Initial imaging was suspicious for pathologic fracture secondary to malignancy due to imaging findings and because radiation-induced fractures have rarely been reported beyond 44 months fr…

Radiation optic neuropathy and retinopathy with low dose (20 Gy) radiation treatment.

PURPOSE: To report a case of optic neuropathy and retinopathy from a dose of radiation traditionally thought to be safe to the visual system and discuss strategies for preventing vision loss when using radiation to treat intraocular tumors. OBSERVATIONS: A 44-year-old woman presented with new, painless vision loss in the left eye eighteen months after receiving proton beam radiotherapy (20 Gy dose delivered in two 10 Gy fractions) for a uveal metastasis of lung cancer. The dilated funduscopic examination revealed optic disc swelling and retinal hemorrhages and an MRI of the brain and orbits d…

Radiation Exposure Dose Trends and Radiation Dose Reduction Strategies in Medical Imaging.

Since the discovery of x-rays, many investigations of radiation effects on humans have taken place. From the discovery of severe x-ray dermatitis near the end of 1896, the multiple radiation burns endured by Thomas Edison's assistant, Clarence Dally, to the establishment of early radiation regulation in 1929, radiation exposure has been a continuing concern. Studies from the 1940s to the 1960s looked at effects such as leukemia and other cancers that appeared to be caused by exposure to radiation; outcomes of these studies resulted in increased regulation, establishment of dose tolerances and …

Radiation dose and radiation protection for patients and physicians during interventional procedure.

Although the wide acceptance of interventional radiology (IVR) procedures has led to increasing numbers of interventions being performed, the radiation doses from IVR are higher. Increasing numbers of case reports of patient radiation injury resulting from IVR are being published. Therefore, radiation protection during IVR poses a very important problem. To protect against radiation injury, the evaluation of radiation dose is essential. The radiation dose must be evaluated for each IVR x-ray machine and each laboratory, because it varies greatly. To obtain this information easily, and to ensur…

[Two cases of radiation-induced sarcoma after radiation therapy in nasopharyngeal carcinoma].

We report two cases of radiation-induced sarcoma after chemoradiation therapy in nasopharyngeal carcinoma. Case 1: A 40-year-old man developed a malignant peripheral nerve sheath tumor (MPNST) in the posterior floor of the nasal cavity 10 years after treatment for nasopharyngeal cancer. Case 2: A 64 year-old man developed a malignant fibrous histiocytoma (MFH) of the lower gum 10 years after treatment of nasopharyngeal cancer. Despite radical surgery, the man with MPNST had a recurrent tumor and died of the disease.

Radiation dose to otologic structures during head and neck cancer radiation therapy.

BACKGROUND: Otologic structures are often contained within head and neck cancer radiation treatment ports. The dosimetry to otologic structures has not been routinely analyzed and radiation treatment planning does not currently attempt to specifically avoid the inner ear structures when dosimetry is calculated. Recent studies demonstrate that up to 30% of patients experience sensorineural hearing loss on multimodality therapy with cisplatin and radiation. METHODS: In the current case series, radiation dosimetry to otologic structures was calculated from computed tomogram treatment plans on pa…

The influence of infrared radiation on short-term ultraviolet-radiation-induced injuries.

Because heat has been reported to influence adversely short- and long-term ultraviolet (UV)-radiation-induced skin damage in animals, we investigated the short-term effects of infrared radiation on sunburn and on phototoxic reactions to topical methoxsalen and anthracene in human volunteers. Prior heating of the skin caused suppression of the phototoxic response to methoxsalen as evidenced by an increase in the threshold erythema dose. Heat administered either before or after exposure to UV radiation had no detectable influence on sunburn erythema or on phototoxic reactions provoked by anthrac…

Peritoneal Post-Radiation Angiosarcoma in a Patient With a History of Radiation for Cervical Cancer.

BACKGROUND Post-radiation angiosarcoma (PRA) is a rare, invasive mesenchymal tumor associated with prior cancer radiotherapy. The diagnostic criteria for PRA include a history of radiation exposure, the development of a new malignant tumor with a long latency period within the irradiated field, and a histological type different from that of the primary malignancy if radiation was administered for the original cancer. Although PRA can arise in any part of the body, it most commonly occurs in the skin. Only a few cases of vascular sarcoma secondary to radiotherapy for cervical cancer have been r…

A systematic approach for successful repair of radiated and non-radiated ureteral injuries.

BACKGROUND: Successful ureteral reconstruction is challenging, particularly in radiated fields. We characterize and directly compare surgical outcomes in modern cohorts of radiated and non-radiated patients undergoing ureteral reconstruction utilizing a systematic approach to pre-operative assessment. We hypothesize that radiated patients will undergo more complex ureteral reconstructions and experience higher rates of surgical failure and complications compared to nonradiated patients. METHODS: Consecutive cases of ureteral reconstruction for acquired ureteral injury performed by a single su

The Implications of Genetic Testing on Radiation Therapy Decisions: A Guide for Radiation Oncologists.

The advent of affordable and rapid next-generation DNA sequencing technology, along with the US Supreme Court ruling invalidating gene patents, has led to a deluge of germline and tumor genetic variant tests that are being rapidly incorporated into clinical cancer decision-making. A major concern for clinicians is whether the presence of germline mutations may increase the risk of radiation toxicity or secondary malignancies. Because scarce clinical data exist to inform decisions at this time, the American Society for Radiation Oncology convened a group of radiation science experts and clinici…

The enhancement of biological ocular UV radiation on beaches compared to the radiation on grass.

BACKGROUND: The influence of albedo on ocular UV exposure has seldom been reported. This paper aimed to explore the enhancement effect on measured ocular UV radiation due to a sand surface compared to measured ocular UV radiation due to a grass surface. METHODS: We measured ambient and ocular UV radiation over the beach and grass surface in Sanya City of China (18.4°N, 109.7°E). The experimental apparatus was composed of a manikin and a dual-detector spectrometer. Integration of both UVA and UVB radiation was used to denote UV radiation. Then biologically effective ocular UVB radiation (UVBE)…

Nuking the radiation: minimizing radiation exposure in the evaluation of pediatric blunt trauma.

PURPOSE OF REVIEW: Our objective is to highlight recent literature investigating low-radiation diagnostic strategies in the evaluation of pediatric trauma. RECENT FINDINGS: In the area of minor head injury, research has focused on implementation of validated clinical decision rules into practice to reduce unnecessary computed tomography scans. Clinical observation may also serve as an adjunct to initial assessment and a potential substitute for computed tomography imaging. Subgroups of children with special needs or severe injury mechanisms may also be safely characterized by the clinical dec…

Radiation in the workplace-a review of studies of the risks of occupational exposure to ionising radiation.

Many individuals are, or have been, exposed to ionising radiation in the course of their work and the epidemiological study of occupationally irradiated groups offers an important opportunity to complement the estimates of risks to health resulting from exposure to radiation that are obtained from other populations, such as the Japanese survivors of the atomic bombings of Hiroshima and Nagasaki in 1945. Moreover, workplace exposure to radiation usually involves irradiation conditions that are of direct relevance to the principal concern of radiological protection: protracted exposure to low le…

Radiation-induced lung disease and the impact of radiation methods on imaging features.

Although radiologic findings in radiation-induced lung disease are well described in the literature, the influence exerted on these findings by different radiation methods is not well understood. Radiation treatment of non-small cell lung cancer varies depending on the location and extent of disease. Irradiation with oblique beam angles results in unusual distribution of radiation-induced lung disease. Small cell lung cancer is treated with irradiation concurrent with or following chemotherapy, and portal arrangements are controversial. In breast cancer, use of tangential beam portals may indu…

Ionizing radiation and the conceptus: neurophysiologic effects of prenatal X-radiation on offspring.

A brief review of the literature precedes the presentation of a radiation behavioral teratology study. The various types of radiation and the units of measure used in radiation biology are discussed. The concept of the radiation-induced teratogenic "triad" of growth retardation, malformation, and death is presented. A discussion of stage- and dose-dependent sensitivity to prenatal irradiation is followed by an introduction to behavioral teratology as a new interdisciplinary area of investigation, emphasizing postnatal psychophysiologic analyses of the effects of prenatal exposure. In the prese…

Overcoming Radiation Resistance: Ferroptosis Induction to Sensitize Solid Tumors to Radiation Therapy.

Radiation therapy (RT) is a mainstay of treatment for a myriad of cancers, often utilized for tumors that are unable to be resected, as well as an adjunct to surgery and chemotherapy. Unfortunately, many cancers are resistant to RT-induced damage and subsequent cell death. This has spurred the pursuit of novel radiation sensitizers that could potentiate the effects of this widely used and important treatment modality. Since its discovery as a regulated cell death mechanism in 2012, ferroptosis has been studied for its connection to cancer and other known oxidative pathways. With this, the inte…
