In diabetes mellitus, pain and weakness in a lower extremity can result from lumbosacral radiculopathy. However, abdominal pain caused by a similar type of thoracic nerve root disease is not generally recognized. Four diabetic patients with abdominal pain were studied. In none of the patients was the actual cause--radiculopathy--suspected until after extensive negative diagnostic investigation, which frequently included invasive procedures. Their clinical features were suggestive of malignant disease. Electromyographic examination revealed the correct diagnosis, and all four of the patients ha…

BACKGROUND: This study was performed to investigate the incidence and clinical features of acute radiculopathy after anterior cervical discectomy and fusion (ACDF) in patients of cervical spondylotic radiculopathy (CSR) combined with cervical kyphosis, to analyze the causes, and to identify preventive measures.
METHODS: We analyzed 39 ACDF patients, stratified into three kyphosis groups (n = 29) and a lordotic control group (n = 10). Kyphosis Group-1 (n = 8) had acute radiculopathy with SAP displacement; Group-2 (n = 13) had displacement without radiculopathy; Group-3 (n = 8) had neither. Ana…

Cervical radiculopathy presents with symptoms involving the head, neck, and upper limb that can significantly impact athletic performance. While the presence of radiating pain and/or paresthesias may increase the clinical suspicion for a cervical radiculopathy, a variety of alternative neurogenic and musculoskeletal disorders can mimic cervical radicular patterns. Familiarity with key differentiating variables in the history and physical examination is essential for early and accurate diagnosis. Evaluation may require the use of imaging, electrodiagnostic testing, and/or diagnostic injections.…

Low back pain is a common condition, which affects millions of people around the world. While there are several treatment options available, conservative treatment has been widely used as an initial approach to managing these conditions. The objective of this article was to highlight the potential of conservative approaches in the treatment and prognosis of low back pain. The search on the Pubmed platform was performed using the following search terms: "lumbar radiculopathy", "low back pain AND Sciatic pain", "conservative treatment". Articles published between 2014 and 2024 in English and Por…

BACKGROUND: Tarlov cysts rarely cause radiculopathy and typically do not warrant operative intervention.
CASE DESCRIPTION: A 50-year-old female with chronic low back pain radiating into both lower extremities had revealed perineural cysts from S1 to S3 on her magnetic resonance imaging scan. Treatment with nonsteroidal anti-inflammatory drugs and gabapentin resulted in good symptomatic improvement.
CONCLUSION: Tarlov cysts are typically incidental findings and rarely cause radiculopathy. Conservative treatment is the standard management, with very few warranting surgical intervention.

Lumbar disk herniation is the most common surgical condition of the spine. High-level athletes participate in activities that place extreme loads on the intervertebral disks. These repetitive loads may lead to an elevated risk for degenerative disk disease, which in turn predisposes to disk herniations. Treatment algorithms for athletes with disk herniations are similar to those in the nonathletic population; however, success in the athletic population is often measured in the ability to return to play. Both nonoperative and operative treatment show a high success rate in return to play in ath

Compression of roots/nerves can disrupt some of their functions, but does not necessarily cause pain. This is illustrated by the frequency of nearly asymptomatic spinal stenosis or disc herniations. In fact, pain of radiculopathies (and nerve entrapments) may mostly be the consequence of intraneural oedema induced by microscopical venous stasis around roots/spinal ganglia (or nerves) not or poorly shown by imaging. This narrative review first lists arguments for a role of congestion of vasa-nervorum in the pathophysiology of radiculopathies, including those induced by disc herniation and spina

A 48-year-old man reported right hip pain and low back pain and was referred to physical therapy by an orthopaedic surgeon after magnetic resonance imaging revealed an L1-2 symmetrical disc bulge. His chief complaint was worsening right groin pain. Following examination, the patient attended 3 treatment visits; however, each visit exacerbated the patient's groin pain, so the physical therapist recommended the patient return to his surgeon for hip imaging. Radiographs revealed a right proximal femur osseous lesion. Magnetic resonance imaging was subsequently performed, and a biopsy established

We report the first extraforaminal location of a lumbar discal cyst. The patient was treated by hemilaminectomy, arthrectomy, cyst resection, and unilateral arthrodesis, achieving complete release of the nerve root. Extraforaminal lumbar discal cyst may represent an unexpected cause of sciatic pain with favorable outcome after surgical resection.

This article is a guideline covering a wide array of cervical conditions seen in the workers' compensation, as well as the nonworkers' compensation, population. The guideline is intended to provide a diagnostic and treatment algorithm to commonly seen cervical conditions such as single-level and multilevel radiculopathies and myelopathies.

INTRODUCTION: We sought to determine whether electrical impedance myography (EIM) could serve as a diagnostic procedure for evaluation of radiculopathy.
METHODS: Twenty-seven patients with clinically and radiologically diagnosed cervical or lumbosacral radiculopathy who met a "gold standard" definition underwent EIM and standard needle electromyography (EMG) of multiple upper or lower extremity muscles.
RESULTS: EIM reactance values revealed consistent reductions in the radiculopathy-affected myotomal muscles as compared with those on the unaffected side; the degree of asymmetry was associat…

Cervical spondylotic myelopathy and radiculopathy are common disorders which can lead to significant clinical morbidity. Conservative management, such as physical therapy, cervical immobilisation, or anti-inflammatory medications, is the preferred and often only required intervention. Surgical intervention is reserved for those patients who have intractable pain or progressive neurological symptoms. The goals of surgical treatment are decompression of the spinal cord and nerve roots and deformity prevention by maintaining or supplementing spinal stability and alleviating pain. Numerous surgica…

Cervical spondylosis is a common problem encountered in modern orthopaedic practice. It is associated with significant patient morbidity related to the consequent radiculopathic and myelopathic symptoms. Operative intervention for this condition is generally indicated if conservative measures fail; however there are some circumstances in which urgent surgical intervention is necessary. Planning any surgical intervention must take into account a number of variables including, but not limited to, the nature, location and extent of the pathology, a history of previous operative interventions, and
