A segmentation method with a large vision model for magnetic resonance imaging-guided adaptive radiotherapy.

BACKGROUND: Segmentation is the most effort-consuming step for magnetic resonance imaging guided adaptive radiotherapy (MRIgART). Although the segment anything model (SAM) exhibits impressive capabilities, its application in medical imaging necessitates clicks, bounding boxes, or providing mask prompts on each target image, which would still require complex human interactions. PURPOSE: This study introduces SAM-ART, a large vision model that integrates personalized information to enhance the segmentation accuracy of MRIgART. METHODS: This study utilized planning computed tomography (pCT), ap

Bowel tracking for MR-guided radiotherapy: simultaneous optimization of small bowel imaging and tracking.

Objective. The small bowel is one of the most radiosensitive organs-at-risk during radiotherapy in the pelvis. This is further complicated due to anatomical and physiological motion. Thus, its accurate tracking becomes of particular importance during therapy delivery, to obtain better dose-toxicity relations and/or to perform safe adaptive treatments. The aim of this work is to simultaneously optimize the MR imaging sequence and motion estimation solution towards improved small bowel tracking precision during radiotherapy delivery.Approach. An MRI sequence was optimized, to adhere to the respi

On the feasibility of cardiac substructure sparing in magnetic resonance imaging guided stereotactic lung radiotherapy.

BACKGROUND: Lung stereotactic body radiotherapy (SBRT) has proven an effective treatment for medically inoperable lung tumors, even for (ultra-)central tumors. Recently, there has been growing interest in radiation-induced cardiac toxicity in lung radiotherapy. More specifically, dose to cardiac (sub-)structures (CS) was found to correlate with survival after radiotherapy. PURPOSE: Our goal is first, to investigate the percentage of patients who require CS sparing in an magnetic resonance imaging guided lung SBRT workflow, and second, to quantify how successful implementation of cardiac spari

Virtual clinical trial-based study for clinical evaluation of projection-reduced low-dose cone-beam CT for image guided radiotherapy.

PURPOSE: Repeated cone-beam CT (CBCT) scans for image-guided radiotherapy (IGRT) increase the health risk of radiation-induced malignancies. Patient-enrolled studies to optimize scan protocols are inadequate. We proposed a virtual clinical trial-based approach to evaluate projection-reduced low-dose CBCT for IGRT. MATERIALS AND METHODS: A total of 71 patients were virtually enrolled with 26 head, 23 thorax and 22 pelvis scans. Projection numbers of full-dose CBCT scans were reduced to 1/2, 1/4, and 1/8 of the original to simulate low-dose scans. Contrast-to-noise ratio (CNR) values in fat and

Determining optimal GTV-to-PGTV margins for CT-guided dose-escalated radiotherapy with daily image guidance in locally advanced rectal cancer.

BACKGROUND: Neoadjuvant chemoradiotherapy for locally advanced rectal cancer yields pathological complete response rates of only 10%-20%. Dose-escalation strategies may improve outcomes, but optimal GTV-to-PGTV margins for CT-guided radiotherapy with daily IGRT remain undefined. METHODS: Twelve LARC patients undergoing CT-guided daily IGRT with a simultaneous integrated boost were included. Daily diagnostic-quality fan-beam CT (FBCT) scans were acquired for IGRT. GTV and CTV were delineated on planning CT and all FBCTs. Target coverage margins were assessed by isotropically expanding the plan

Comparative Evaluation of [68Ga]Ga-Fibroblast Activation Protein Inhibitor vs. [18F]FDG as a Novel Radiotracer for Biology-Guided Image Radiotherapy.

Background/Objectives: Biology-guided radiotherapy (BgRT) is a novel technology utilizing PET radiotracer emissions to deliver image-guided adaptive RT. 18F-Fluorodeoxyglucose ([18F]FDG) is the most common PET radiotracer but has background in the liver and brain because it is taken up by viable tumor cells as well as inflammatory cells. Fibroblast activation protein (FAP) is overexpressed in cancer-associated fibroblasts with minimal expression in normal tissues. FAP inhibitors (FAPIs) bind to FAP, and a FAPI labeled with Gallium-68 (68Ga) is a novel radiotracer with high tumor selectivity. M
