BACKGROUND: Both the anterior chamber and posterior chamber phakic intraocular lenses (pIOLs) implantation are acceptable refractive surgical approaches in keratoconus patients with high anisometropia, contact lens intolerance, or who prefer spectacle and contact lens independent. They are beneficial for correcting anisometropia in stable keratoconus cases or following corneal procedures such as intrastromal corneal ring segments (ICRS), collagen cross-linking (CXL), and keratoplasty. They are suitable for eyes without advanced keratoconus with acceptable best-corrected distance visual acuity

Cataract surgery is the most commonly performed refractive procedure in the world today. Improvements in the calculation of intraocular lens power and design have allowed complete spherical correction of preexisting refractive error with intraocular lens implantation. Advances in incision construction have improved the refractive results of cataract surgery by minimizing surgically induced astigmatism. Astigmatism and pseudophakic presbyopia continue as the primary indications for spectacle correction following cataract surgery. Improvements in the technique of correcting preexisting astigmati

PURPOSE: Femtosecond laser-assisted small incision lenticule extraction (SMILE) was performed to correct myopic astigmatism in a 39-year-old patient who had previously undergone deep anterior lamellar keratoplasty (DALK) for keratoconus, with clinically significant anisometropia and contact lens intolerance.
METHODS: Case report.
RESULTS: SMILE was planned in the right eye to reduce the refractive error and to allow spectacle correction. The surgical procedure was centered on the visual axis, a 5.20-mm optical zone was used, and the refractive lenticule was extracted through a 3.25-mm incisi

PURPOSE OF REVIEW: Keratorefractive procedures designed to decrease refractive errors have gained enormous popularity among ophthalmologists and patients. As the post-refractive surgery patient population ages, visually significant cataracts will develop. With advances in techniques for cataract extraction and intraocular lens implantation, cataract surgery has evolved into a refractive surgical procedure as well as an operation to improve best corrected visual acuity. This raises expectations in terms of desired postoperative refractive status and uncorrected visual acuity. Although performin

PURPOSE OF REVIEW: Advances in artificial intelligence have integrated into modern medicine decision making and diagnostics. Artificial intelligence in ophthalmology has become more ingrained in refractive surgery because of extensive use of diagnostic modalities including interpretation of anterior segment imaging used in refractive surgery. The role of artificial intelligence has increased to include biometry for the accurate selection of intraocular lenses (IOLs), detection of pathology such as cornea ectasia and keratoconus, sizing of phakic IOLs, as well as surgical decision making for th

PURPOSE: The aim of this study was to evaluate clinical outcomes of the combined surgery (triple procedure) of deep anterior lamellar keratoplasty (DALK), phacoemulsification, and intraocular lens (IOL) implantation using a new surgical strategy.
METHODS: The study included 43 eyes of 43 patients who underwent a triple procedure. Twenty-six eyes with good visibility through the cornea underwent phacoemulsification before the stromal dissection. Seventeen eyes with bad visibility underwent the DALK procedure first until achieving enough visibility to perform the lensectomy. IOL power calculati

PURPOSE: To determine demographic, surgical, and preoperative visual factors affecting the level of static and dynamic cyclotorsion (SC and DC) in eyes undergoing single-step transepithelial photorefractive keratectomy (TransPRK).
METHODS: In this cross-sectional study, 386 patients with different types of refractive errors scheduled for single-step TransPRK were enrolled. A comprehensive profile of personal, optic, and visual characteristics of patients as surgical parameters were collected. Statistical modeling was used to explore correlates of SC and DC before and during the refractive pro

TOPIC: To compare the outcomes of surgical approaches to correct ametropia after cataract and lens surgery.
CLINICAL RELEVANCE: Despite advancements in the field of biometry and intraocular lens (IOL) power calculation formulas, complete elimination of refractive surprises after cataract and lens surgery is impossible. Preferred Practice Patterns acknowledges the possibility of refractive surprise after cataract surgery; however, no recommendations regarding the preferred treatment have been given.
METHODS: PubMed and Scopus were used to search the literature as of November 14, 2024. For the

INTRODUCTION: Presbyopia is the most prevalent refractive error. With a progressive aging population, its surgical correction is an important challenge for the ophthalmology community, as well as the millions of patients who suffer from it, and who are increasingly demanding alternatives to its correction with glasses or contact lenses.
MATERIAL AND METHODS: A review is presented with a synthesised discussion on the pathophysiological theories of presbyopia and an updated and analytical description of the non-lens involvement surgical techniques used to treat presbyopia.
RESULTS: Corneal pro

Corneal ectasia comprises keratoconus, keratoglobus, pellucid marginal degeneration, and iatrogenic keratectasia. In all forms of corneal ectasia, there is a thinning of the cornea, usually accompanied by steepening of the cornea, leading to irregular astigmatism. Here, we provide an overview of histopathologic alterations of the various corneal ectasias. Furthermore, histologic changes after surgical procedures that are performed in severe cases of corneal ectasia, such as corneal cross-linking and penetrating keratoplasty (pKPL), as well as refractive surgical procedures that may lead to pos…

With the increase in corneal laser refractive surgery, there is a greater need for precise evaluation of the corneal surface. Articles published in the past year have reported possible use of confocal scanning laser technology-based devices to image the cornea. Other studies have compared existing instruments and software in an effort to determine if data from different instruments are comparable. Topographic evaluation also has served to highlight long-term complications of procedures like radial keratotomy and the promise of newer surgical procedures like the intrastromal corneal ring segmen

Pars plana vitrectomy is today a common first-line procedure for treatment of rhegmatogenous retinal detachment (RRD). Removal or preservation of the natural lens at the time of vitrectomy is associated with both advantages and disadvantages. The combination of cataract extraction (i.e., phacoemulsification) with pars plana vitrectomy (PPVc) enhances visualization of the peripheral retina and the surgical management of the vitreous base. However, PPVc prolongs the surgical time and is associated with iatrogenic loss of the accommodation function in younger patients, possible postoperative anis

BACKGROUND: Standardization for reporting medical outcomes enables clinical study comparisons and has a fundamental role in research reproducibility. In this context, we present mEYEstro, a free novel standalone application for automated standardized refractive surgery graphs. mEYEstro can be used for single and multiple group comparisons in corneal and intraocular refractive surgery patients. In less than 30 s and with minimal user manipulation, mEYEstro automatically creates the required journal standard graphs while simultaneously performing valid statistical analyses.
RESULTS: The softwar

Post refractive corneal ulcers is a disastrous complication, can affect healthy individuals, is cumbersome to treat, and sometimes has a poor prognosis with corneal scarring. Accurate diagnosis and prompt treatment of corneal infection is very important; however, until now, there has been no specific protocol for the management of this common eye disease and severe cases may require a corneal transplant. The patient is a 42-year-old male who suffered a corneal ulcer after photo refractive keratectomy (PRK) surgery in which the cornea was completely destroyed. None of the routine treatments wer

Refractive cataract surgery has evolved considerably in recent years owing to the rapid development in precise ocular biometry, surgical technique and instrumentation, and various astigmatism-correcting and multifocal intraocular lenses. However, inherent defects of these premium intraocular lenses should not be underestimated and neither should imperfections of measuring devices. Cataract surgery complications such as posterior capsular opacification and dry eye, which were originally infrequent or minor, are becoming increasingly prominent. Surgeons must properly understand and conduct refra
